Situational fears: Association with negative affect-related smoking cognition among treatment seeking smokers.

Despite the consistent clinically-significant relation between smoking and anxiety and its disorders, there is limited understanding of how specific fears relate to smoking processes. To isolate therapeutic targets for smoking-anxiety treatment development, there is a need to identify the underlying situational fears most related to smoking processes. Thus, the present study examined the association between interoceptive, agoraphobic, and social fears in terms of clinically significant negative affect-related smoking cognitions including negative affect reduction expectancies, coping motives,

Attentional bias in adolescents with panic disorder: changes over an 8-day intensive treatment program.

BACKGROUND: The present study evaluated attentional bias in adolescents diagnosed with panic disorder. Although a large body of research exists in the area of attentional bias in adults, this feature of panic disorder is not well understood in adolescents. METHOD: Twenty-five adolescents, aged 12-17, with a panic disorder diagnosis were included in the study. An emotional Stroop task was utilized to assess whether: (1) adolescents with panic disorder exhibit an attentional bias to panic-relevant stimuli; (2) this bias diminishes after completing a course of CBT; and (3) a specific attentional

Anxiety in middle adulthood: effects of age and time on the 14-year course of panic disorder, social phobia and generalized anxiety disorder.

BACKGROUND: Much about the long-term course of anxiety disorders is unknown. The present study utilizes a naturalistic, longitudinal, short-interval follow-up design to elucidate the course of anxiety disorders over 14 years in a largely middle-aged adult sample recruited from out-patient psychiatry and primary care facilities. METHOD: The sample consisted of 453 participants with a diagnosis of panic disorder (PD), social phobia (SP) and/or generalized anxiety disorder (GAD). Anxiety symptom ratings were tracked using weekly psychiatric status ratings (PSRs). Controlling for demographic and

Dimensions of perfectionism across the anxiety disorders.

To explore the role of perfectionism across anxiety disorders, 175 patients with either panic disorder (PD), obsessive compulsive disorder (OCD), social phobia, or specific phobia, as well as 49 nonclinical volunteers, completed two measures [Frost, R. Trait anxiety, coping with stress, and norharman.

Plasma levels of the beta-carboline norharman, concentration of platelet 5-HT, trait measures of anxiety, and measures of coping and defense mechanisms were compared for 15 patients with panic disorder and 24 healthy volunteers. Patients indicated that they made less use of the defense mechanism of principalization than control subjects. No other differences between patients and controls were significant. Platelet 5-HT concentration was positively correlated with the subjectively reported anxiety. Plasma norharman concentration was negatively correlated with the defense mechanisms of principal

Quantitative EEG correlates of panic disorder.

Quantitative analysis of electroencephalographic (EEG) signals recorded from multiple scalp sites was used to compare panic disorder patients (n = 34) with normal healthy controls. Patients exhibited greater overall absolute power in the delta, theta, and alpha frequency bands and less relative power in the beta band. Discriminant analysis of absolute power indices correctly classified 75% of the subjects, while relative power indices exhibited a 69% correct-classification rate. Absolute delta and theta power were positively correlated with observer ratings of anxiety, while relative beta powe

Panic disorder with or without concomitant depression 5 years after treatment: a prospective follow-up.

50 patients with panic disorder (30 without and 20 with concomitant depression) were enrolled in a controlled treatment study using either imipramine or doxepin in addition to supportive psychotherapy and were then studied under naturalistic treatment conditions over a 5-year period. While patients with concomitant depression scored higher in overall measures of illness severity (as measured by HAMA, HAMD and GAS), no differences were detected between the groups with regard to panic disorder symptoms and degree of impairment. Our data suggest that comorbidity of panic disorder and depression i

Coping strategies in major depressed, agoraphobic and comorbid in-patients: a longitudinal study.

The first aim of this study was to explore the diagnostic specificity of coping styles by comparing ways of coping in non-anxious major depressed, non-depressed agoraphobic, and both major depressed and agoraphobic (comorbid) in-patients. The second aim was to investigate whether a vulnerability model, a state model, or a combined vulnerability-state model of coping accounted best for the data. On admission and when discharged, 95 patients completed the Way of Coping Checklist and were evaluated on several symptom scales. Self-report symptom scales were completed at one-year follow-up as well.

Four Systems Anxiety Questionnaire (FSAQ): a self-report measure of somatic, cognitive, behavioral, and feeling components.

This article presents the development of a 60-item self-report known as the Four Systems Anxiety Questionnaire (FSAQ). The FSAQ assesses anxiety on four components: somatic, cognitive, behavioral, and feeling. The FSAQ was given to 218 first-year university students, 54 anxiety patients attending clinical psychology departments, and 14 agoraphobic patients pre- and postpsychological treatment. Results reveal satisfactory reliability and validity levels. In addition, we proved that the FSAQ is sensitive to change following psychological treatment.

The effect of long-term imipramine treatment on carbon dioxide-induced anxiety in panic disorder patients.

The authors examined whether long-term treatment with imipramine would decrease CO2-induced anxiety in 10 patients with panic disorder. The patients underwent CO2 testing after single-blind placebo testing and again after imipramine treatment. Scores on self-rated visual analog mood scales and panic attack symptom scales showed that the anxiogenic effects of CO2 were significantly reduced during long-term imipramine treatment. These results suggest that the mechanisms underlying CO2-induced anxiety may be similar to those involved in the pathophysiology of panic disorder.

Delimitation of generalized anxiety disorder: clinical comparisons with panic and major depressive disorders.

We compared 40 outpatients with "pure" generalized anxiety disorder (GAD) with 152 panic disordered patients with varying degrees of phobic avoidance, and 241 primary major depressives with single and recurrent episodic patterns. Despite sociodemographic and symptomatologic overlaps with these comparison groups, GAD emerged as a relatively distinct disorder, characterized by chronic low-grade symptomatology with observed anxiety at interview, as well as nausea, headache, tension, and insomnia. These anxious "traits," which appear to be part of the habitual self of the patient, are subject to f

The significance of HPA axis disturbance in panic disorder.

Agoraphobic and panic disorder patients underwent 1-mg Dexamethasone Suppression Tests (DST) before, during, and after an 8-week trial of diazepam, alprazolam, or placebo. Previously described, never-ill controls underwent similar testing. At baseline, 21 of 82 (25.6%) panic disorder and 5 of 38 (13.2%) controls were nonsuppressors. This difference grew more marked with multiple testing over a 2-month period; 18 of 44 (40.9%) panic disorder patients were nonsuppressors on at least 1 of 3 tests compared with only 5 of 35 (14.3%) controls (p = 0.006). DST results were related to severity, but no

Dehydroepiandrosterone-sulfate/cortisol ratio in panic disorder.

We hypothesized that the dehydroepiandrosterone-sulfate (DHEA-S)/cortisol ratio, which has been used as an index of adrenocortical function, would be altered in panic disorder patients and would change after treatment. We evaluated 10 male and 14 female outpatients meeting DSM-III-R criteria for panic disorder. Of these 24 subjects, 13 were treated with clonazepam, 8 were treated with alprazolam, and 3 were treated with placebo as part of a double-blind study. The DHEA-S/cortisol ratio values in the 24 patients with panic disorder (mean = 20.5, SD = 11.6) were significantly higher than those o

A 3H-flunitrazepam binding inhibitor is present in psychiatric patients' sera.

We investigated the possible existence of endogenous compounds acting on benzodiazepine central receptors in the serum of patients with panic attacks or depression. Our results show the presence of a substance which inhibits the 3H-flunitrazepam binding specifically in the samples taken from the patients' groups, and which is not present in normal controls, in the range of concentrations used. This compound has a molecular weight below 1,000 daltons, is heat-stable, and resistant to proteolytic degradation. The demonstration of this inhibitor opens new perspectives in the study of the biochemi

Life events obtained via interview: the effect of time of recall on data obtained in controls and patients with panic disorder.

Because previous studies have documented the distorting effect of time of recall on the reporting of life events over a one-year period of time, the relation between various life event measures and time of recall over time periods greater than one year was examined in 44 patients with panic disorder and 44 control subjects. For both groups, time of recall was unrelated to any of nine life event measures. The implications of these findings for life event studies requiring retrospection greater than one year are discussed.

An update on treatment of the anxiety disorders.

The recently discovered high prevalence of the anxiety disorders, as well as studies demonstrating their strong association with psychophysiologic symptoms, will probably increase referrals for these disorders to psychiatric clinicians. The authors review developments since 1983 in the psychopharmacologic and psychologic-behavioral treatment of the anxiety disorders. They find a growing body of empirical evidence that a variety of pharmacologic and cognitively oriented behavioral therapies are effective treatments. Less evidence is available for the utility of psychodynamic therapies, although

The social ecology of anxiety. Theoretical and quantitative perspectives.

Experience sampling is a relatively new research approach that captures relevant variations in experience and symptoms in the course of everyday life. In this paper, a socioecological frame of reference that integrates psychological, historical, cultural, and developmental theories about anxiety phenomena will be illustrated using quantitative descriptions of anxiety and avoidance behaviors gathered with the Experience-Sampling Method. The interaction between anxiety symptoms and the environment in which they occurred is explored in a case study that points to possibilities for future research

Antipanic drug effects during lactate infusion in lactate-refractory panic patients.

In previous articles we have reported on the total Acute Panic Inventory (API) score during lactate-induced panic. Even patients with panic disorder who do not panic during lactate infusion have higher API scores during the infusion than normal controls. In this post hoc analysis, we wished to determine whether specific API items were more sensitive to the beneficial effects of antipanic medication, either at baseline or during the infusion, in these lactate-insensitive panic patients. Four of the 17 items showed specific drug blockade of baseline severity. Five additional items showed specifi
