Evolving diagnostic criteria in primary lateral sclerosis: The clinical and radiological basis of "probable PLS".

INTRODUCTION: Primary lateral sclerosis is a rare neurodegenerative disorder of the upper motor neurons. Diagnostic criteria have changed considerably over the years, and the recent consensus criteria introduced 'probable PLS' for patients with a symptom duration of 2-4 years. The objective of this study is the systematic evaluation of clinical and neuroimaging characteristics in early PLS by studying a group of 'probable PLS patients' in comparison to a cohort of established PLS patients. METHODS: In a prospective neuroimaging study, thirty-nine patients were stratified by the new consensus

Irrigation endoscopic decompressive laminotomy. A new endoscopic approach for spinal stenosis decompression.

BACKGROUND CONTEXT: The classic surgical treatment of spinal stenosis involves bilateral dissection of paraspinal muscles to expose all the involved levels, wide laminectomy, and medial facetectomy and foraminotomy. The surgical morbidity of the procedure is further magnified by being more common in elderly with associated medical comorbidities and being usually global involving multiple levels. To address this problem, several less invasive techniques have been introduced over the past decade including the microendoscopic decompression. PURPOSE: The aim was to describe and evaluate a new end

One Year Results of the Randomized BiPOWR Trial Comparing the Spring Distraction System (SDS) and the One Way Self-Expanding Rod (OWSER) for the Correction of Neuromuscular and Syndromic Early Onset Scoliosis.

BACKGROUND: Current "growth-friendly" implants for treatment of Early Onset Scoliosis (EOS) have limitations that reduce their efficacy and cost-effectiveness. Recently, two systems have been developed that mitigate many of these limitations, the Spring Distraction System (SDS) and the One Way Self-Expanding Rod (OWSER). The purpose of the multicenter BiPOWR trial was to compare 1-year efficacy and -safety of both strategies in the treatment of neuromuscular or syndromic EOS. METHODS: Non-ambulant, neuromuscular/syndromic EOS patients were included in three academic hospitals. They were rando

PROcalcitonin and NEWS2 evaluation for Timely identification of sepsis and Optimal use of antibiotics in the emergency department (PRONTO): protocol for a multicentre, open-label, randomised controlled trial.

INTRODUCTION: Sepsis is a common, potentially life-threatening complication of infection. The optimal treatment for sepsis includes prompt antibiotics and intravenous fluids, facilitated by its early and accurate recognition. Currently, clinicians identify and assess severity of suspected sepsis using validated clinical scoring systems. In England, the National Early Warning Score 2 (NEWS2) has been mandated across all National Health Service (NHS) trusts and ambulance organisations. Like many clinical scoring systems, NEWS2 should not be used without clinical judgement to determine either the

Defining the clinical, molecular and imaging spectrum of adaptor protein complex 4-associated hereditary spastic paraplegia.

Bi-allelic loss-of-function variants in genes that encode subunits of the adaptor protein complex 4 (AP-4) lead to prototypical yet poorly understood forms of childhood-onset and complex hereditary spastic paraplegia: SPG47 (AP4B1), SPG50 (AP4M1), SPG51 (AP4E1) and SPG52 (AP4S1). Here, we report a detailed cross-sectional analysis of clinical, imaging and molecular data of 156 patients from 101 families. Enrolled patients were of diverse ethnic backgrounds and covered a wide age range (1.0-49.3 years). While the mean age at symptom onset was 0.8 ± 0.6 years [standard deviation (SD), range 0.2-

Mixed-methods feasibility study of a community-based model to improve equity and efficiency in dementia research participation: protocol for ACCESS D.

INTRODUCTION: Despite national efforts to improve research inclusion, people from underserved communities remain underrepresented in dementia trials. Barriers occur at the point of initial engagement and also within the participation pathway itself, as the structure and burden of early screening procedures can discourage continuation. ACCESS D (Advancing Community Collaboration and Engagement Strategies in Dementia) aims to address these challenges by testing a community-based model that combines co-produced engagement events, low-burden research participation, and real-time support from the S

The Novel Use of a Custom Three-Dimensional-Printed Polycaprolactone-Tricalcium Phosphate Synthetic Bone Scaffold (Osteopore®) in the Management of a Critical-Sized Bone Defect for a Patient with an Open Intra-Articular Calcaneal Fracture - A Case Report.

INTRODUCTION: Intra-articular calcaneal fractures are generally treated with open reduction and internal fixation in order to restore calcaneal anatomy as well as subtalar and calcaneocuboid joint congruency. Bone loss is common due to the impaction of cancellous bone beneath the posterior facet as a result of axial loading. Critical-sized bone defects are commonly addressed with the Masquelet "induced membrane technique" incorporating autogenous or allogenous bone graft. Synthetic bone scaffolds are readily available and customizable through three-dimensional (3D) printing. This case report d

Nonoperative Management of a Spontaneous Perforation of Neobladder Secondary to Blackout From Solitary Binge Drinking: A Case Report and Current Literature Review.

The orthotopic ileal neobladder is becoming a popular technique of urinary diversion after radical cystectomy (RC) for localized muscle-invasive bladder cancer (MIBC), allowing patient continence, with a more desirable body image and good quality of life. Minimally invasive robot-assisted RC and neobladder have the potential to minimize physical and psychological trauma and are increasingly being adopted for patients with MIBC worldwide. Spontaneous perforation of orthotopic neobladder is uncommon;however, it represents serious complications. Solitary binge drinking can be dangerous in a patie

GPi DBS treatment outcome in children with monogenic dystonia: a case series and review of the literature.

INTRODUCTION: Dystonia is the third most common pediatric movement disorder and is often difficult to treat. Deep brain stimulation (DBS) of the internal pallidum (GPi) has been demonstrated as a safe and effective treatment for genetic dystonia in adolescents and adults. The results of DBS in children are limited to individual cases or case series, although it has been proven to be an effective procedure in carefully selected pediatric cohorts. The aim of our study was to present the treatment outcome for 7- to 9-year-old pediatric patients with disabling monogenic isolated generalized DYT-TH

Development and validation of a prognostic model predicting symptomatic hemorrhagic transformation in acute ischemic stroke at scale in the OHDSI network.

BACKGROUND AND PURPOSE: Hemorrhagic transformation (HT) after cerebral infarction is a complex and multifactorial phenomenon in the acute stage of ischemic stroke, and often results in a poor prognosis. Thus, identifying risk factors and making an early prediction of HT in acute cerebral infarction contributes not only to the selections of therapeutic regimen but also, more importantly, to the improvement of prognosis of acute cerebral infarction. The purpose of this study was to develop and validate a model to predict a patient's risk of HT within 30 days of initial ischemic stroke. METHODS:
