Antiphospholipid syndrome leading to myocardial infarction in a Fontan patient 17 years after Norwood palliation.

Appropriate regimens for anticoagulation in the single ventricle population remain controversial and are usually based on individual practitioners' preferences. We present the case of a patient with a history of hypoplastic left heart syndrome and Norwood palliation who presented at 17 years of age with an acute myocardial infarction and a new diagnosis of antiphospholipid syndrome.

[Interventional occlusion of interatrial communication after modified Fontan operation].

UNLABELLED: After modified Fontan operations various communications between the systemic and pulmonary venous returns may cause persistent or increasing postoperative cyanosis. Interventional closure of these right-to-left shunts may be necessary to eliminate hypoxemia and to reduce the risk of paradoxical embolic complications. PATIENTS AND METHODS: Eighteen patients with a mean age of 5.6 +/- 4.1 (2.5-17.5) years underwent interventional closure of a right-to-left shunt 17.4 +/- 15.8 (3-60) months after a modified Fontan operation. After test balloon occlusion fenestrations were closed in 1

Extracardiac Fontan operation for complex cardiac anomalies: seven years' experience.

METHODS: Between 1988 and 1995, 60 patients with complex cardiac anomalies underwent a total extracardiac cavopulmonary connection, a combination of a bidirectional cavopulmonary anastomosis with an extracardiac conduit interposition between the inferior vena cava and pulmonary arteries, except in one patient in whom direct anastomosis was possible. In 40 patients the total extracardiac cavopulmonary connection followed preliminary bidirectional cavopulmonary anastomosis, associated with a modified Damus-Kaye-Stansel anastomosis in 16. The conduits were constructed of Dacron fabric (n = 34), h

Use of miniature biplane transesophageal echocardiography during pediatric atrial catheter interventional procedures.

We reported the use of a new miniature biplane TEE probe during pediatric cardiac interventional catheterization procedures. Use of this imaging modality provided significant advantages during dilation of obstructed venous pathways and closure of interatrial defects. Procedural characteristics and specific congenital heart lesion-related advantages were discussed.

Pediatric biplane transesophageal echocardiography: preliminary experience.

Recent development of prototype pediatric biplane transducers has extended the use of biplane TEE imaging to the neonate. One such TEE probe with 64 imaging elements in each pallet was used perioperatively in 46 infants and children (weight 2.9 to 32 kg) undergoing surgery for complex congenital heart disease. The success rate for passing the transducer, complications of the procedure, and the number of cases in which the vertical plane provided information that was not available from the horizontal plane were determined. Probe insertion was successful in all patients. Imaging was discontinued

Congestion Enriches Intra-hepatic Macrophages Through Reverse Zonation of CXCL9 in Liver Sinusoidal Endothelial Cells.

BACKGROUND & AIMS: Congestion alters the microenvironment of the liver sinusoid along the portal-central axis. We studied spatial changes in immune cells in the sinusoid that contribute to congestive fibrosis and portal hypertension (PHTN). METHODS: To visualize the distribution of immune cells in congestive hepatopathy (CH), we performed imaging mass cytometry (IMC) on liver tissue from patients with CH, Fontan-associated liver disease (FALD), and controls. We performed partial ligation of the inferior vena cava (pIVCL) to simulate CH in mice and isolated primary liver cells for single-cell

Dextrocardia in Adults with Congenital Heart Disease.

BACKGROUND/OBJECTIVES: Dextrocardia is rare in the general population, and may be associated with significant additional cardiac malformations. We aimed to identify the prevalence and patterns of additional cardiac defects, as well as the associated long-term morbidity and mortality, in adult patients with dextrocardia, in a specialised Adult Congenital Heart Disease (ACHD) service. METHODS: A retrospective study of patients with dextrocardia managed by our tertiary ACHD service, since January 2000, was performed. Medical records were reviewed and the National Death Index was consulted to con

Current treatment for Wolff-Parkinson-White syndrome: results and surgical implications.

From July 1986 to January 1991, 123 patients with Wolff-Parkinson-White syndrome underwent operation for ablation of aberrant conduction pathways. There were 85 male and 38 female patients ranging in age from 11 months to 68 years. Associated anomalies included Ebstein's anomaly, sudden death syndrome, coronary artery disease, cardiomyopathy, abdominal aortic aneurysm, neurofibromatosis, other arrhythmias, or other complex congenital heart disease. Forty-one patients had multiple accessory pathways. Operative results showed a 7% initial failure rate, which dropped to 3% after reoperation. One

A simplified approach for assessment of intracardiac baffles and extracardiac conduits in congenital heart surgery with two- and three-dimensional magnetic resonance imaging.

BACKGROUND: Intracardiac baffles and extracardiac conduits have been used in the reconstructive surgery of a broad spectrum of congenital cardiac malformations. Periodic evaluation of these structures may not lend itself readily to echocardiographic and angiographic imaging. The purpose of the study was to describe the experience of our institution with the use of magnetic resonance imaging (MRI) in evaluating conduits and baffles and to describe the simplified approach we developed to image these structures, which allows for grouping individual lesions into broad categories. METHODS AND RESU

Validation of Congestive Hepatic Fibrosis Score in Pediatric and Adult Fontan-Associated Liver Disease.

Fontan-associated liver disease is a unique form of congestive hepatopathy occurring after Fontan palliation of single functional ventricle congenital heart disease. Although congestive hepatic fibrosis post-Fontan has been scored with various histologic systems, none have been validated in this population. The Congestive Hepatic Fibrosis Score (CHFS) was developed to assess liver disease in congestive hepatopathy secondary to chronic right heart failure and is a promising tool for staging congestive hepatic fibrosis post-Fontan. We sought to validate the CHFS in this setting and to examine cl

Benefits of Chronic Atrial Pacing in Fontan Physiology with Sinus Node Dysfunction and Preserved Ejection Fraction: Novel Therapeutic Approach for Failing Fontan.

Increased heart rate (HR) can reduce central venous pressure (CVP) in patients with preserved ventricular systolic function. We aimed to investigate whether this theory works in failing Fontan physiology. Pacemaker implantation (PMI) was performed in six Fontan patients (age: 6-40 years) with sinus node dysfunction (SND) without junctional rhythm (JR). Baseline median (interquartile [IQR]) values of CVP, ventricular end-diastolic pressure (EDP), and ejection fraction (EF) were 12 (11-15) mmHg, 13 (10-16) mmHg, and 57% (53%-64%), respectively. Before PMI, the relationships among HR, cardiac ind

Low Ventricular Stiffness Is Associated With Suboptimal Outcomes in Patients With a Single Right Ventricle After the Fontan Operation: A Novel Phenotype.

BACKGROUND: Despite a rigorous screening process, including cardiac catheterization, a subset of patients with a single right ventricle (SRV) demonstrates suboptimal short-term outcomes after the Fontan operation. The goal of this study was to perform a comprehensive assessment of diastolic function in pre-Fontan patients with an SRV using invasive reference-standard measures and determine their associations with post-Fontan outcomes. METHODS AND RESULTS: Children aged 2 to 6 years with SRV physiology undergoing pre-Fontan heart catheterization were recruited prospectively. Patients were divi

Impact of 4D-Flow CMR Parameters on Functional Evaluation of Fontan Circulation.

We sought to evaluate the potential clinical role of 4D-flow cardiac magnetic resonance (CMR)-derived energetics and flow parameters in a cohort of patients' post-Fontan palliation. In patients with Fontan circulation who underwent 4D-Flow CMR, streamlines distribution was evaluated, as well a 4D-flow CMR-derived energetics parameters as kinetic energy (KE) and energy loss (EL) normalized by volume. EL/KE index as a marker of flow efficiency was also calculated. Cardiopulmonary exercise test (CPET) was also performed in a subgroup of patients. The population study included 55 patients (mean ag
