Pediatric hydrocephalus and imaging evaluation of ventriculomegaly in the emergency setting.

Hydrocephalus is a frequently encountered neurological disorder of cerebrospinal fluid (CSF) physiology or dynamics, resulting in the abnormal enlargement of CSF spaces and often presenting with signs and symptoms of raised intracranial pressure. The etiology of hydrocephalus is numerous, regardless of the model used to understand its pathophysiology. Differentiating ventricular enlargement due to hydrocephalus or parenchymal atrophy can be difficult in pediatric patients, but certain imaging signs could help differentiate the two entities. The management of hydrocephalus aims to restore norma

Upward transtentorial herniation, hydrocephalus, and cerebellar edema in hypertensive encephalopathy.

BACKGROUND: Edema of the cerebellum with secondary obstructive hydrocephalus is a rare presentation of hypertensive encephalopathy. The authors report an unusual case of isolated posterior fossa swelling with upward transtentorial herniation and hydrocephalus causing neurologic deterioration. These patients are often initially evaluated by a neurologist because of the acute neurologic symptoms. Prompt diagnosis with aggressive blood pressure control may obviate the need for emergent cerebrospinal fluid (CSF) diversion. REVIEW SUMMARY: This is a case report of a 26-year-old man who presented t

Rhythmic oscillations with a wavelength of 0.5-2 min in transcranial Doppler recordings.

We have studied intracranial pressure (ICP) B-waves and their association with rhythmic changes in blood flow velocity (B-wave equivalents) by transcranial Doppler sonography (TCD) monitoring. In overnight TCD recordings in 10 normal young adults, these rhythmic changes in blood flow velocity were higher and more frequent during REM sleep and sleep stage 1 than during other sleep stages. B-wave equivalents also had a longer wavelength during REM sleep. Their relative frequency in these normal subjects over one night ranged from 35 to 73%. Peripheral resistance (assessed by the Pourcelot index)

A quantitative model of the cerebral windkessel and its relevance to disorders of intracranial dynamics.

OBJECTIVE: Traditional models of intracranial dynamics fail to capture several important features of the intracranial pressure (ICP) pulse. Experiments show that, at a local amplitude minimum, the ICP pulse normally precedes the arterial blood pressure (ABP) pulse, and the cranium is a band-stop filter centered at the heart rate for the ICP pulse with respect to the ABP pulse, which is the cerebral windkessel mechanism. These observations are inconsistent with existing pressure-volume models. METHODS: To explore these issues, the authors modeled the ABP and ICP pulses by using a simple electr

Lethal septicemia caused by calcified cavo-tricuspid thrombus. Case report on a patient with atrial shunt.

With the introduction of one way calibrated shunt systems, made of silicon rubber, an efficient treatment of hydrocephalus seemed to be possible. The mortality rate decreased rapidly for example for non-communicating hydrocephalus from 80 to 20%. However, other complications appeared, such as shunt infections, shunt malfunctions and even severe thromboembolic complications and septic shock. This report presents one patient with shunt-treated normal pressure hydrocephalus who developed pulmonary emboli, mural calcified atrial thrombus with involvement of the tricuspid valve and shunt infection

Transmantle Pressure Computed from MR Imaging Measurements of Aqueduct Flow and Dimensions.

BACKGROUND AND PURPOSE: Measuring transmantle pressure, the instantaneous pressure difference between the lateral ventricles and the cranial subarachnoid space, by intracranial pressure sensors has limitations. The aim of this study was to compute transmantle pressure noninvasively with a novel nondimensional fluid mechanics model in volunteers and to identify differences related to age and aqueductal dimensions. MATERIALS AND METHODS: Brain MR images including cardiac-gated 2D phase-contrast MR imaging and fast-spoiled gradient recalled imaging were obtained in 77 volunteers ranging in age f

Palliative lumboperitoneal shunt for leptomeningeal metastasis-related hydrocephalus: A case series.

BACKGROUND: Leptomeningeal metastasis-related hydrocephalus causes distress to patients with end-stage cancer through headache and other symptoms by elevating intracranial pressure, thus reducing quality of life. Ventriculoperitoneal shunt has been used as a treatment option in palliative care. We review four cases of patients who underwent lumboperitoneal shunt for leptomeningeal metastasis-related hydrocephalus. CASES: All patients suffered from severe headache and nausea. The primary lesion was histologically diagnosed as lung adenocarcinoma in each case. The duration from diagnosis to ons

A venous mechanism of ventriculomegaly shared between traumatic brain injury and normal ageing.

Recently, age-related timing dissociation between the superficial and deep venous systems has been observed; this was particularly pronounced in patients with normal pressure hydrocephalus, suggesting a common mechanism of ventriculomegaly. Establishing the relationship between venous drainage and ventricular enlargement would be clinically relevant and could provide insight into the mechanisms underlying brain ageing. To investigate a possible link between venous drainage and ventriculomegaly in both normal ageing and pathological conditions, we compared 225 healthy subjects (137 males and 88

CSF formation rate-a potential glymphatic flow parameter in hydrocephalus?

BACKGROUND: Studies indicate that brain clearance via the glymphatic system is impaired in idiopathic normal pressure hydrocephalus (INPH). This has been suggested to result from reduced cerebrospinal fluid (CSF) turnover, which could be caused by a reduced CSF formation rate. The aim of this study was to determine the formation rate of CSF in a cohort of patients investigated for INPH and compare this to a historical control cohort. METHODS: CSF formation rate was estimated in 135 (75 ± 6 years old, 64/71 men/women) patients undergoing investigation for INPH. A semiautomatic CSF infusion inv

Traumatic brain edema induced by ventricular puncture. A study by magnetic resonance imaging.

Magnetic resonance imaging demonstrates after ventricular catheterization a focal brain hypersignal corresponding to a parenchymal edema along the drain track. In the course of our daily clinical activity, this hypersignal extension seemed more pronounced when catheterizing the frontal area than the junctional parieto-temporo-occipital parenchyma (or trigonal area). In order to confirm this impression, we prospectively studied ten consecutive patients with normal pressure hydrocephalus in whom both of these brain regions were successively catheterized first by a frontal puncture for intracrani

Papilledema secondary to vestibular schwannoma: An atypical case without intracranial hypertension.

In most cases, vestibular schwannomas with papilledema are associated with intracranial hypertension secondary to hydrocephalus (obstructive or communicating). We describe the atypical case of a 39-years-old man who presented with bilateral papilledema revealing a vestibular schwannoma, but without hydrocephalus and with normal intracranial pressure. Ophtalmologic signs were completely resolved after tumor removal. The pathophysiological mechanism generally described to explain bilateral papilledema in such cases is tumor-induced hyperproteinorachia. However, in the absence of hydrocephalus or

Navigating the ventricles: Novel insights into the pathogenesis of hydrocephalus.

Congenital hydrocephalus occurs in one in 500-1000 babies born in the United States and acquired hydrocephalus may occur as the consequence of stroke, intraventricular and subarachnoid hemorrhage, traumatic brain injuries, brain tumors, craniectomy or may be idiopathic, as in the case of normal pressure hydrocephalus. Irrespective of its prevalence and significant impact on quality of life, neurosurgeons still rely on invasive cerebrospinal fluid shunt systems for the treatment of hydrocephalus that are exceptionally prone to failure and/or infection. Further understanding of this process at a

Cerebral blood volume alterations during fractional pneumoencephalography.

Simultaneous and continuous measurements of the cerebral blood volume (CBV), cerebrospinal fluid (CSF) and blood pressure were carried out in six patients during fractional pneumoencephalography in order to examine intracranial volumetric interactions. Three patients (Group A) showed normal encephalographic findings, and in three patients (Group B) communicating hydrocephalus with convexity block was found encephalographically. In all patients the injection of air was followed by an immediate increase of CSF pressure and blood pressure and a concomitant decrease of CBV. The initial CSF pressur

Lumboperitoneal shunt placement under local anesthesia: a technical note from an experience with more than 200 cases.

The purpose of this technical note was to report that lumboperitoneal shunt (LPS) placement under local anesthesia can be a promising alternative for treating idiopathic normal pressure hydrocephalus (iNPH), particularly in elderly patients who might be reluctant to undergo surgery under general anesthesia. The authors present their experience with 202 cases of LPS placement performed for iNPH under local anesthesia between March 2022 and September 2024. The median operative time was 35 minutes, and the median operating room time was 71 minutes. Procedure-related complications were observed in

Pleural effusions appearing in the rehabilitation ward after ventriculoperitoneal shunts: a report of two adult cases and a review of the literature.

This study presents two adult patients who experienced pleural effusion during hospitalization for stroke rehabilitation therapy after ventriculoperitoneal shunt placement for normal pressure hydrocephalus associated with aneurysmal subarachnoid haemorrhage. The pleural effusion appeared without migration of the catheter into the thoracic cavity. Because of respiratory insufficiency, which prevented progress in their rehabilitation programme, thoracentesis was repeated for recurrent pleural effusions, the composition of which differed significantly from that of cerebrospinal fluid. Both cases
