[Trochleoplasty: Surgical technique : Video article].

A 29-year-old physically active patient presented with recurrent right-sided patellar dislocation. Clinical and radiological investigation showed patellar instability with stable cruciate and collateral ligaments, excess internal rotation of the right femur, as well as trochlear dysplasia. Treatment consisted of trochleoplasty in combination with medial patellofemoral ligament reconstruction.

Comparison of fixation methods after anteromedialization osteotomy of the tibial tubercle for patellar instability.

PURPOSE: The aim of this study was to evaluate the biomechanical strength of two 4.5-mm screws and three 3.5-mm screws for fixation of the tibial tubercle after anteromedialization osteotomy. METHODS: Anteromedialization of the tibial tubercle was performed on 5 pairs of fresh-frozen cadaveric lower extremities. One leg from each pair was randomized to fixation with two 4.5-mm screws and the contralateral leg to fixation with three 3.5-mm screws. Each specimen was loaded cyclically to simulate an active straight-leg raise and then to failure while displacement of the tubercle fragment was rec

Patellar instability with increased knee flexion due to lateral femoral condyle distal dysplasia: a report of two cases.

We present two case reports with clinical and radiological assessment of the anatomical features at the distal lateral end of trochlea and femur in patients with patellar instability in higher knee flexion and how these findings could possibly be involved in the genesis of this rare type of patellar instability. Both patients underwent several (nine and seven) surgical procedures, but the patellar instability could not be successfully eliminated. Our hypothesis was that a short and flattened lateral distal condyle/trochlea may cause lateral patellar instability in higher flexion. We found cons

Tibial plateau fractures. Management and expected results.

The objective of treatment of tibial plateau fractures is precise reconstruction of the articular surfaces, stable fragment fixation allowing early motion, and repair of all concomitant lesions. A classification scheme is employed to include the high incidence of concomitant lesions in specific fracture types. The term "complex trauma" is used for extensive injuries involving multiple structural elements of the knee joint. Exact grading of the soft-tissue injury is crucial for the treatment plan. The authors suggest a four-grade classification system of closed and open soft-tissue injury. Pref

Tibial tuberosity fracture in adolescents--report of a case and review of the literature.

Avulsion fracture of the tibial tuberosity usually caused by sudden violent contraction of the quadriceps muscles is uncommon and predominantly occurs in adolescents. Illustrative is the case history of a 16-year old boy with an Ogden I-B avulsion fracture successfully treated by open reduction and internal screw fixation. In these cases, anatomic reduction should be the primary goal. In specific cases this can be achieved by closed reduction, however, usually open reduction and fixation is indicated.

Effect of experimental patellar luxation on the knee joint in the rat.

In 16 male Wistar albino rats the patella was fixed to the lateral aspect of the lateral condylus of the femur. Animals with luxated knee joint and controls were sacrificed 7-9 months after surgery. Operated knee joints were processed for histology and subjected to morphometric analysis in comparison to the controls.- It was found that (1) the artificial disturbance of the femoro-patellar junction resulted in a severe destruction of articular cartilages of the tibio-femoral and femoro-patellar junction; (2) the destructions progressed with time; (3) there was no substantial difference between

[A case of central core disease. Light microscopic and histochemical studies (author's transl)].

A sporadic case of central core disease in a 5 1/2-year-old girl is reported. Clinically, a retarded motor development existed, furthermore, a muscle weakness and hypotonia of the extremities and trunk, contractures of the hip- and knee-joint,and luxation of both hip-joints. Biopsy specimens are taken from both Mm. gastrocnemii. Muscle fibres show, by morphologic examination, 95 per cent cores, which are characteristic for this myopathy. A further abnormality is seen inthe histochemical preparations for phosphorylase, succinate dehydrogenase, NAD diaphorase tetrazolium reductase, myofibrillar

The Safety of Postoperative Selective Cyclooxygenase-2 Inhibitors in Chronically Anticoagulated Patients Undergoing Primary Total Hip Arthroplasty.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs), particularly selective cyclooxygenase-2 (COX-2) inhibitors, are foundational to multimodal pain management following primary total-hip arthroplasty (THA) but concerns regarding bleeding often limit their use in patients receiving chronic anticoagulation. While data support COX-2 inhibitor safety following total knee arthroplasty in this population, THA-specific evidence remains limited. METHODS: Utilizing a national database, patients undergoing primary THA on chronic anticoagulation between 2015 and 2025 were identified and stratifie

Flexion Dislocation After Limb Lengthening: Correction With Distal Femoral Osteotomy, Quadriceps Release, and Physeal-Sparing Medial Patellofemoral Ligament (MPFL) Reconstruction.

BACKGROUND: Patellar instability is a common clinical condition in skeletally immature individuals. Surgical treatment is considered when risk of recurrence is high. INDICATIONS: Distal femoral osteotomy is indicated in the setting of obligate flexion dislocation where femoral valgus contributes to a shortened lateral column, with concurrent quadriceps procedures considered for chronic contracture and medial patellofemoral ligament (MPFL) reconstruction for added stabilization. TECHNIQUE DESCRIPTION: This procedure is performed in a stepwise manner as some components may not be necessary bas

Decreased National Rate of below the Knee Amputation in Patients with Popliteal Artery Injury.

BACKGROUND: Popliteal arterial injury (PAI) is the second most common infrainguinal arterial injury after femoral artery injury with an incidence < 0.2%. A 2003 analysis of the National Trauma Data Bank (NTDB) reported a below the knee amputation (BKA) rate of 7.1% in patients with PAI as well as higher risk in those with an associated fracture or nerve injury. Given advances in vascular surgical techniques, improved multidisciplinary care, and expeditious diagnosis with computed tomography angiography, we hypothesized that the national rate of BKA in patients with PAI has decreased and sought

Chronic Musculoskeletal Pain in a Case of Larsen Syndrome: Role of Myofascial Trigger Points.

BACKGROUND: Larsen syndrome is an extremely rare congenital disorder characterized by craniofacial anomalies and multiple skeletal deformities such as short stature, kyphoscoliosis of the spine, cervical spine instability, clubfeet, and joint dislocations especially of the knee and hip joints. Musculoskeletal pain in a case of Larsen syndrome signals red flags as these patients have various congenital anomalies. However, myofascial trigger points could be a cause of chronic musculoskeletal pain in these patients. Myofascial trigger points are easy to diagnose and treat. CASE REPORT: We are pr
