New developments in clinical aspects of lymphatic disease.

The lymphatic system is fundamentally important to cardiovascular disease, infection and immunity, cancer, and probably obesity--the four major challenges in healthcare in the 21st century. This Review will consider the manner in which new knowledge of lymphatic genes and molecular mechanisms has demonstrated that lymphatic dysfunction should no longer be considered a passive bystander in disease but rather an active player in many pathological processes and, therefore, a genuine target for future therapeutic developments. The specific roles of the lymphatic system in edema, genetic aspects of

Inflammatory BCG adenitis associated with immune reconstitution following allogeneic haematopoietic stem cell transplant in infancy.

We describe four cases of a localized, granulomatous reaction to BCG including ipsilateral painful, suppurative lymphadenopathy associated with donor immune reconstitution following allogeneic haematopoietic stem cell transplant performed in infancy and preceded by uneventful, routine BCG immunisation. The management of the inflammatory disease in these cases with surgery, antimycobacterial chemotherapy and steroids, is discussed.

Increased echogenicity of renal cortex: a transient feature in acutely ill children.

OBJECTIVE: The purpose of our study was to determine the frequency of hyperechogenicity of renal parenchyma in children with acute abdominal … Clinical diagnoses were idiopathic acute abdominal pain (n = 74), appendicitis (n = 83), mesenteric lymphadenitis (n = 15), ileocecitis (n = 7), gastroenteritis (n = 7), Crohn's disease (n = 1), intussusception (n = 1), and pneumonia (n = 1). No concurrent renal disease was diagnosed. CONCLUSION: Increased echogenicity of renal parenchyma in children with acut …

Primary localized amyloidosis manifested as supraclavicular and mediastinal lymphadenopathy.

Thoracic involvement of amyloidosis is relatively rare, but mediastinal lymphadenopathy in the absence of pulmonary parenchymal involvement is extremely rare. The case presented here is of a previously healthy elderly woman who developed a palpable mass in the right supraclavicular area. The chest CT scan showed extensive, contiguous and homogeneous low attenuated lymphadenopathy with stippled calcification in the right supraclavicular area and mediastinum. Amyloidosis was confirmed histopathologically on a biopsy specimen from a right supraclavicular lymph node. Because there were no other si

[Kikuchi-Fujimoto's disease: 3 cases report].

OBJECTIVE: Kikuchi-Fujimoto's is a rare disease, characterized by the diversity of clinical symptoms, with predominance of enlarged cervical lymph nodes and fever. We report three cases with different presentations. MATERIAL AND METHOD: The first one was a classic form, frequently described in literature. The second was characterized by concomitant association with systemic lupus erythematosus. Histologic findings of Kikuchi's disease were the presence of nuclear debris, absence of haemathoxilin bodies and neutrophils polynuclears. The presentation of the third case was acute meningitis with

A rare case of Crohn's disease in head and neck surgery.

Oral manifestations of Crohn's disease may present to the Otolaryngologist in the form of ulceration, glossitis and odynophagia, but rarer examples of presentation are also recognized. In this case a 24-year-old lady presented with cervical lymphadenopathy, the subsequent investigation of which resulted in the identification of the disease both in this node and in the tonsils. It is noted that these lesions may precede the classical intestinal manifestations and so the Otolaryngologist could aid in the initial diagnosis of the disease. The pathogenic possibilities of how Crohn's disease might

The etiology and clinical characteristics of mesenteric adenitis in Korean adults.

This study is aimed at investigating the etiology and clinical characteristics of mesenteric adenitis in Korean adults, prospectively. Clinical manifestations of fifteen patients who presented with the acute onset of right lower quadrant pain and sonographically enlarged mesenteric lymph nodes and normal appendix were evaluated. For etiologic diagnosis, stool culture, serologic test for Epstein-Barr virus, and Widal test were performed. Colonoscopy with mucosal biopsies and microbial tissue cultures were performed in 12 of 15 patients. Of fifteen patients 6 were male and the average age was 29

Schistosomiasis associated with a mediastinal mass: case report and review of the literature.

A 13-year-old girl presented with fever, night sweat, weight loss, abdominal pain, haematuria and hepatosplenomegaly. Urinalysis revealed many Schistosoma haematobium ova, but rectal snip examination was negative for schistosomal ova. X-ray and CT scan of the chest revealed enlargement of the anterior, superior, mediastinal and left suprahilar lymph node with an adjacent left pulmonary parenchymal opacity and small peripheral lesions on the right side. A bone marrow aspiration and biopsy was normal. The patient was treated with Praziquantel for her urinary schistosomiasis. Because of her clini

Primary lymphocutaneous Nocardia brasiliensis infection.

We report a rare case of primary lymphocutaneous Nocardia brasiliensis infection occurring in an elderly diabetic man maintained on long-term therapy with prednisone. The mode of inoculation was that of a puncture wound caused by a contaminated needle that occurred one month prior to the development of the clinically apparent infection. The course of this patient's infection was protracted and required two surgical procedures and several months of antibiotic therapy. A review of 15 previously reported cases is presented as well as a discussion of appropriate medical therapy.

[The problem of joint sarcoidosis].

Joint symptoms in sarcoidosis are early manifestations of this systemic disease; patients with such symptoms, however, are seldom examined by a rheumatologist. The authors investigated 72 patients in whom a definite diagnosis of sarcoidosis had been made at the 1. University Clinic for Tuberculosis and Respiratory Diseases. Joint symptoms were found in 75% of the patients. In those with erythema nodosum they were found in 94%. Acute onset of the disease was found in more than 50% of the patients with erythema nodosum. Objective joint abnormalities were noted in 28% of the patients without eryt
