Selective, sustained, and shift in attention in patients with diagnoses of schizophrenia.

Attentional deficits are a prominent aspect of cognitive dysfunction in schizophrenia. The present study was designed to investigate attention deficit in a group of patients with diagnosis of schizophrenia. According to the segmental set theory suggested by Hogarty and Flesher, three aspects of attention problems, selective, sustained, and shift in attention, were studied. The 30 patients hospitalized on three psychiatric wards at Shiraz and Isfahan and 30 normal healthy subjects matched for age, sex, and years of education were administered a computerized Continuous Performance Test, Stroop C

Experimentally induced stress in rheumatoid arthritis of recent onset: effects on peripheral blood lymphocytes.

OBJECTIVE: To examine the effects of experimentally-induced stress on the mobilization of peripheral blood lymphocytes (PBL) in patients with rheumatoid arthritis (RA) of recent onset. METHODS: Twenty-two (16 F, 6 M) patients (mean age 57.6 yrs.) and 23 (15 F, 8 M) healthy subjects (mean age 54.7 yrs.) were subjected to experimental stressors. The numbers of T-cells, B-cells, and NK-cells were determined before and after the completion of tasks inducing physical and mental effort. RESULTS: The change in PBL in response to stress was about equal for patients and healthy subjects (p > 0.75 in

Respiratory distress in newborn: treated with ventilation in a level II nursery.

Fifty consecutive neonates with respiratory distress persisting beyond 6 h of age were studied during a 18 month period (total deliveries 2000/y). Twenty two neonates were managed with oxygen hood with increasing oxygen concentration, 28 with continuous positive airway pressure (CPAP) ventilation using a nasal cannula. Of these babies on CPAP, 10 were shifted to intermittent positive pressure ventilation (IPPV) on a pressure limited, time cycled ventilator (Neovent, Vickers). Babies were monitored with continuous hemoglobin oxygen saturation (SaO2), hourly blood pressure and vital charting. Ra

AI-Assisted Carotid Ultrasound for Nonexpert Use in Primary Care Cardiovascular Prevention: User-Centered Iterative Development Study.

BACKGROUND: Carotid ultrasound is traditionally confined to specialist settings because examination quality and diagnostic reliability are highly dependent on the operator's technical skill and experience. Recent advances in AI-assisted ultrasound may enable task shifting to primary care staff and support the integration of personalized visual risk communication into routine preventive care. Visualization of subclinical atherosclerosis has been shown to strengthen cardiovascular risk communication and support preventive engagement. However, limited evidence exists on how such technology can be…

Phone-Based Intervention Under Nurse Guidance for Control of Hypertension After Stroke: A Randomized Multicenter Phase 3 Trial in Ghana.

BACKGROUND: Addressing the rising burden of stroke in low-income countries will require pragmatic and scalable interventions targeting major … care (P=0.12). CONCLUSIONS: Leveraging mobile health intervention with minimal sophistication and task shifting to nurses on top of usual care safely improved blood pressure control among stroke survivors in a low-resource setting. Further study is warranted to confirm these findings and understand outcome drivers. REGISTRATION: URL: https://www.clinicaltria …

Simultaneous 5-HT1BR agonist/5-HT6R antagonist action as a potential treatment of Parkinson's disease and its comorbidities.

Parkinson's disease (PD) treatment focuses mainly on the augmentation of dopamine transmission, but to alleviate adverse motor effects accompanying L-DOPA use, additional treatments with serotonergic (5-HT) medications may be considered. We propose a novel concept based on the simultaneous activation of 5-HT1BR and 5-HT6R blockade as a putative therapeutic option for PD treatment. We have developed PZKKN-94, a dual human 5-HT1BR agonist (EC50 = 39 nM) and human 5-HT6R antagonist (KB = 7.7 nM), with selectivity over 43 targets, favorable drug-like properties, and brain penetration. Importantly,

Effectiveness of a stepped-care programme of WHO psychological interventions in migrant populations resettled in Italy: Study protocol for the RESPOND randomized controlled trial.

INTRODUCTION: Migrant populations, including workers, undocumented migrants, asylum seekers, refugees, internationally displaced persons, and other populations on the move, are exposed to a variety of stressors and potentially traumatic events before, during, and after the migration process. In recent years, the COVID-19 pandemic has represented an additional stressor, especially for migrants on the move. As a consequence, migration may increase vulnerability of individuals toward a worsening of subjective wellbeing, quality of life, and mental health, which, in turn, may increase the risk of

Motor Imagery to Facilitate Sensorimotor Re-Learning (MOTIFS) after traumatic knee injury: study protocol for an adaptive randomized controlled trial.

BACKGROUND: Treatment following traumatic knee injury includes neuromuscular training, with or without surgical reconstruction. The aim of rehabilitation is to restore muscle function and address psychological factors to allow a return to activity. Attention is often on rehabilitation of knee function, but deficiencies often persist. Specific interventions addressing psychological factors are sparing with varying degrees of success. We have developed a novel training program, MOTor Imagery to Facilitate Sensorimotor Re-Learning (MOTIFS), which integrates simultaneous psychological training int

Client and provider preferences for HIV care: Implications for implementing differentiated service delivery in Thailand.

INTRODUCTION: Differentiated service delivery (DSD) for antiretroviral therapy (ART) maintenance embodies the client-centred approach to tai … identified as one promising DSD model in Thailand. However, low preference for decentralization and task shifting may prove challenging to implement other models, especially since many providers were unsure about DSD benefits. This calls for local implementation studies to prove feasibility and governmental and social support to legitimize and normalize DSD in …

Cascading network failure across the Alzheimer's disease spectrum.

Complex biological systems are organized across various spatiotemporal scales with particular scientific disciplines dedicated to the study of each scale (e.g. genetics, molecular biology and cognitive neuroscience). When considering disease pathophysiology, one must contemplate the scale at which the disease process is being observed and how these processes impact other levels of organization. Historically Alzheimer's disease has been viewed as a disease of abnormally aggregated proteins by pathologists and molecular biologists and a disease of clinical symptoms by neurologists and psychologi

Metabolic-associated steatotic liver disease in children and adolescents: a scoping review and narrative synthesis of epidemiology, risk factors, and screening approaches with emerging implications for sub-Saharan Africa.

BACKGROUND: Metabolic-Associated Steatotic Liver Disease (MASLD) is recognized as one of the most common chronic liver diseases in children globally, rising in tandem with the childhood obesity pandemic. Although high-income countries focus on advanced phenotyping, Sub-Saharan Africa (SSA) faces a distinct "two-speed" epidemic characterized by rapid urbanization and a unique "double burden" of malnutrition and obesity. This review examines the global and regional epidemiology of paediatric MASLD, contrasting established Western practices with the unique genetic, environmental, and diagnostic c

Cancer workforce-a global crisis: a Lancet Oncology Commission.

The shortage of well-trained personnel to deliver cancer care and conduct research remains a major obstacle to reducing disparities in cancer survival between high-income countries and low-income and middle-income countries (LMICs). This Commission set out to provide actionable guidance for strengthening the global cancer workforce with an emphasis on reducing these disparities. To better understand the extent of the rising global cancer burden and the resulting workforce needs, we modelled the current and future global landscape of 17 common cancers and 18 workforce personnel types. Among the

Psychometric properties of MERIT: A concise tool for community health workers to screen priority mental disorders.

BACKGROUND: Task shifting is an accepted way to address the treatment gap for mental health disorders. Community Health Workers (CHWs) can play vital roles through screening, referral, and providing basic counseling. To address the dearth of a concise screening instrument for CHWs, the "Mental HEalth ScReenIng Tool for Community Health Workers in India [MERIT]" was designed and developed. In this paper, we have examined the psychometric properties of MERIT. METHODS: MERIT consists of nine stems (consisting of 11 questions) covering the following five domains: (a) Substance Abuse (alcohol, tob…

Home-based digital counselling by frontline community workers for anxiety and depression symptoms in rural Sindh, Pakistan: the mPareshan intervention.

BACKGROUND: There is limited evidence that interventions for improving mental well-being can be integrated sustainably into primary health care in Pakistan. We piloted 'mPareshan digital intervention' locally, adapted from WHO mhGAP and delivered by trained and supervised women lay workers to learn if it was feasible and possibly effective in reducing anxiety and depression prior to proposing implementation on a larger scale. METHODS: Using Generalized Anxiety Disorder-7 (GAD-7) and Patient Health Questionnaire-9 (PHQ-9), a baseline household survey was conducted by independent data collector

The acceptability, feasibility and impact of a lay health counsellor delivered health promoting schools programme in India: a case study evaluation.

BACKGROUND: Studies in resource-limited settings have shown that there are constraints to the use of teachers, peers or health professionals to deliver school health promotion interventions. School health programmes delivered by trained lay health counsellors could offer a cost-effective alternative. This paper presents a case study of a multi-component school health promotion intervention in India that was delivered by lay school health counsellors, who possessed neither formal educational nor health provider qualifications. METHODS: The intervention was based on the WHO's Health Promoting S

[Reflections on maternal techniques and the rearing of infants and young children in the Ivory Coast].

The core hypothesis of the authors is that certain particularities of the traditional bringing up of infants and young children in Côte-d'Ivoire modify its affective experiencing and therefore its libidinal organization. Ivorian breast feeding is extremely permissive, subjected to the sole wish of the child, traditionally until the age of 2. It maintains a belated mother-infant fusion in the context of intense oral satisfaction. The end of this idyllic fusion state generally is brought about brutally. When it is contemporary to the ending of carrying the child on the back, this milk weaning co

Glomus cell heterogeneity underpins distinct carotid body chemoreflex pathways: implications for hypertension.

AIMS: The carotid body (CB) is a multimodal chemosensory organ, yet how it encodes differential external stimuli to drive distinct peripheral chemoreflex responses remains unclear. This study aimed to define differences in intrinsic cellular mechanisms underlying CB signalling and to investigate how these processes are altered in pre-hypertension. METHODS AND RESULTS: We used potassium cyanide (KCN) as a hypoxia mimetic to activate the CB.K+ channels implicated in hypoxia sensing were pharmacologically inhibited, with TWIK-related acid-sensitive K+ (TASK) channels targeted using Ba2+ or ML365

Investigating the Impact of Automation on the Health Care Workforce Through Autonomous Telemedicine in the Cataract Pathway: Protocol for a Multicenter Study.

BACKGROUND: While digital health innovations are increasingly being adopted by health care organizations, implementation is often carried out without considering the impacts on frontline staff who will be using the technology and who will be affected by its introduction. The enthusiasm surrounding the use of artificial intelligence (AI)-enabled digital solutions in health care is tempered by uncertainty around how it will change the working lives and practices of health care professionals. Digital enablement can be viewed as facilitating enhanced effectiveness and efficiency by improving servi

Effects of acute ischemic stroke on binaural perception.

Stroke-induced lesions at different locations in the brain can affect various aspects of binaural hearing, including spatial perception. Previous studies found impairments in binaural hearing, especially in patients with temporal lobe tumors or lesions, but also resulting from lesions all along the auditory pathway from brainstem nuclei up to the auditory cortex. Currently, structural magnetic resonance imaging (MRI) is used in the clinical treatment routine of stroke patients. In combination with structural imaging, an analysis of binaural hearing enables a better understanding of hearing-rel
