Threatened limb from stingray injury.

We present the case of a 43-year-old female who suffered a stingray injury to her left ankle. The sting caused occlusion of the dorsalis pedis artery, causing dry gangrene of the medial forefoot. A below knee amputation was recommended but she was transferred for a second opinion. A Prostaglandin E1 infusion was commenced, resulting in alleviation of pain and improvement in perfusion. Amputation of great and second toes was performed, with the head of the first metatarsal preserved and covered via a cross-over skin flap raised from the contralateral leg. Achilles tendon lengthening was then pe

[The psychogenic clubfoot. A rare posttraumatic condition (author's transl)].

At the beginning of this century and during World War I psychogenic contractures were very common; later on they became rare.--The author reports and demonstrates 3 cases of psychogenic supination contractures of the foot (functional clubfeet) following a slight trauma of the ankle, the treatment of which was carried out in 1946, 1974 and 1976. All of these patients were women. The deformity of the foot was caused by a permanent contraction of the tibialis-muscles without an underlying organic disease. The therapy consisted of an arthrodesis of the midtarsal joints along with a transfer of the

Complex, atypical clubfoot: follow-up after up to 16 years reveals a high risk of relapse but good functional and radiological outcomes.

INTRODUCTION: The treatment of complex atypical clubfoot poses many challenges. In this paper, we report on the course of complex clubfoot, … ection was performed in n = 3 feet in addition to Achilles tendon lengthening and tibialis anterior tendon transfer. CONCLUSION: Good primary correction of complex clubfoot using the modified Ponseti technique results in a high recurrence rate in the medium term. Relapse treatment without peritalar arthrolysis procedures produces good functional results even tho …

A Comparison of Split Peroneus Brevis Tendon and Semitendinosus Allograft Tendon for Lateral Ankle Ligament Reconstruction.

Lateral ankle instability is a debilitating condition that is often unresponsive to conservative therapy. Many techniques for operative repair have been proposed, most commonly performed as the Broström or modified Broström procedure. In patients with failed primary repair, hereditary collagen disorders, strenuous work activity, obesity, or ligamentous laxity, the Broström repair is less likely to be successful, and anatomic or nonanatomic reconstruction should be considered. The purpose of this study was to compare the functional outcomes and patient satisfaction between anatomic and nonanato

Complex iatrogenic clubfoot: is it a real entity?

In our study, we aimed to demonstrate whether a complex iatrogenic clubfoot really exists; identify the causative mechanisms; and determine … ex clubfeet. Relapsed clubfeet were treated by Achilles tenotomy or lengthening and anterior tibial tendon transfer. We believe that faulty manipulation and a poor casting technique may convert a typical clubfoot into a complex iatrogenic deformity. Risk factors include severe clubfoot, short and stubby foot, and unmolded casts slipping down.

Decision-making in massive rotator cuff tear.

Treatment of massive rotator cuff tears has developed over many years ranging from conservative treatment to open and arthroscopic repair, muscle transfers and reversed arthroplasty. The evolution of more advanced techniques in arthroscopic repair has changed the treatment approach and improved the prognosis for functional outcome despite low healing rates. Due to this rapid development, our evidence-based knowledge today is mainly founded in Level 3 and Level 4 studies. Based on the literature, the current knowledge on treatment of symptomatic massive rotator cuff tears is proposed in an algo

Surgical management of insertional calcific achilles tendinosis with a central tendon splitting approach.

BACKGROUND: Insertional calcific Achilles tendinosis is a painful, frequently disabling condition. Numerous operative approaches for this problem have been described. This study evaluated the outcome of a central tendon splitting approach. METHODS: Twenty-two patients were evaluated after a central tendon splitting approach for persistent insertional calcific Achilles tendinosis. Followup averaged 34 (11 to 64) months. Suture anchors were routinely used to augment the tendon insertion after debridement. An American Orthopaedic Foot and Ankle Society (AOFAS) hindfoot score, shoewear comfort, a

Surgical Options for Massive Rotator Cuff Tears with Preserved Full Range of Motion in Middle-Aged Patients.

Rotator cuff tears (RCTs) are a primary cause of shoulder pain and dysfunction. The progression of these tears, coupled with the intricate biomechanics of the shoulder joint, makes the repair and management of massive rotator cuff tears (MRCTs) particularly challenging for surgeons. For younger patients who are ineligible for arthroplasty, surgical approaches for MRCTs-such as biologic augmentation, interpositional grafts, and techniques like superior capsular reconstruction and biceps rerouting to counteract humeral upward migration-are gaining prominence. Additionally, tendon transfers, supr…

Tissue Engineering: Step Ahead in Maxillofacial Reconstruction.

Within the precedent decade, a new field of "tissue engineering" or "tissue regeneration" emerge that offers an innovative and exhilarating substitute for maxillofacial reconstruction. It offers a new option to supplement existing treatment regimens for reconstruction/regeneration of the oral and craniofacial complex, which includes the teeth, periodontium, bones, soft tissues (oral mucosa, conjunctiva, skin), salivary glands, and the temporomandibular joint (bone and cartilage), as well as blood vessels, muscles, tendons, and nerves. Tissue engineering is based on harvesting the stem cells wh

An anterior-lateral thigh perforator flap on a recipient brachial-radial vein graft for complex wound reconstruction: a case report.

A case of challenging microsurgical reconstruction of a difficult defect in a radiated upper limb is reported. A difficult wound, with tendon and bone exposition, developed on the dorsum of the forearm in a 76-year-old patient; she had been radiated since almost 50 years and her left hand had also been revascularized twice with venous grafts between the humeral artery and the superficial palmar arch. After failure of a local flap, an anterior-lateral thigh perforator flap was successfully transferred with end-to-side anastomoses on the arterialized venous graft. Up to date follow-up shows a go

Outcome of surgical treatment for forearm pronation deformities in children with obstetric brachial plexus injuries.

Seven children were operated on for pronation contractures of the forearm due to obstetric brachial plexus injuries. All underwent extensive preoperative evaluations to determine the extent of injury, secondary deformities, and capacity to perform a few basic tasks. Sequential video studies were used to document these findings. Operative procedures performed included various combinations of tendon/muscle lengthenings and/or transfers. Postoperative evaluations focused on function rather than gains in active range of motion and the patient/parental assessment of the benefit of the procedure by

Results of modified Robert Jones operation for clawed hallux.

The results of modified Robert Jones operation for clawed hallux carried out on 36 feet were reviewed by clinical and radiological investigations. Twenty-nine feet (80 per cent) achieved good results while 3 feet (8-5 per cent) obtained fair results and in 4 feet (11 per cent) the results were poor. Better results were obtained in the poliomyelitis cases than in the other groups. Correction of deformity appears to be achieved by the transferred extensor hallucis longus elevating the first metatarsal neck and by the flexor hallucis longus flexing the rigid toe obtained by interphalangeal arthro

Functional recovery strategies in progressive supranuclear palsy with cerebellar predominance.

This report details a case of a patient in his 60s who exhibited ataxic gait, frequent falls, limb incoordination, tremors and rigidity, leading to a diagnosis of progressive supranuclear palsy with cerebellar predominance (PSP-C). Clinical assessment indicated slurred speech, increased muscle tone, deep tendon reflexes that were mildly reduced asymmetrically on the left side and reduced strength, while sensations remained intact. PSP-C is an uncommon variant of PSP often misidentified as multiple system atrophy-cerebellar type (MSA-C) because of shared characteristics. The patient participate
