Anti-Compensatory Saccades Changes After Visuo-Vestibular Physical Therapy in People With Acute Unilateral Vestibulopathy: A Prospective Observational Study.

BACKGROUND AND PURPOSE: Acute unilateral vestibulopathy (AUVP) impairs the vestibulo-ocular reflex (VOR), leading to gaze instability and significant functional disability. Anti-compensatory saccades (AcS) assessed using the Suppression Head Impulse Paradigm (SHIMP) reflect vestibulo-saccadic interaction and may represent a marker of central compensation. The aim of the study was to analyze changes in AcS following structured visuo-vestibular physical therapy (VVPT). METHODS: Ninety-two patients with AUVP (mean age 59 years, 57 women and 35 men) completed an 8-week VVPT program (one session p

Increased Labyrinthine T1 Postgadolinium Signal Intensity Is Associated with the Degree of Ipsilateral Sensorineural Hearing Loss in Patients with Sporadic Vestibular Schwannoma.

BACKGROUND AND PURPOSE: Vestibular schwannomas are benign, generally slow-growing tumors, commonly presenting with hearing loss. Alterations in the labyrinthine signal are seen in patients with vestibular schwannoma; however, the association between imaging abnormalities and hearing function remains poorly defined. The purpose of this study was to determine whether labyrinthine signal intensity is associated with hearing in patients with sporadic vestibular schwannoma. MATERIALS AND METHODS: This was an institutional review board-approved retrospective review of patients from a prospectively

Otolith dysfunction during vertiginous attacks in Ménière's disease.

OBJECTIVE: Vertiginous attacks of Ménière's disease (MD) are characterized by various types of vertigo, namely rotatory vertigo, dizziness and drop attack. When a patient complains of dizziness without spontaneous nystagmus, otolith dysfunction cannot be ruled out. The purpose of this study was to evaluate otolith dysfunction during vertiginous attacks of MD or delayed endolymphatic hydrops. MATERIAL AND METHODS: Vestibular function tests were carried out daily for several days in 11 patients (MD, n = 9; delayed endolymphatic hydrops, n = 2) during vertiginous attacks. Otolith function was ev…

[Assessment of cochlear function with distortion products of otoacoustic emissions in acoustic neuroma].

Unilateral hearing loss (HL) caused by an acoustic neuroma (AN) is regarded to be neural ("non-cochlear"). By using otoacoustic emissions it is possible to prove concurrent sensory (cochlear) HL. We prefer distortion-product otoacoustic emissions (DPOAE) because these are more frequency-specific and stable. From April 1994 to May 1996, we examined preoperatively 24 patients with radiologically proven AN. Pure-tone audiometry, DP-grams, brainstem-evoked response audiometry (BERA) and vestibular studies were performed in all cases, while special auditory tests (Fowler, Carhart) and short increme

Resting state brain network segregation is associated with walking speed and working memory in older adults.

Older adults exhibit larger individual differences in walking ability and cognitive function than young adults. Characterizing intrinsic brain connectivity differences in older adults across a wide walking performance spectrum may provide insight into the mechanisms of functional decline in some older adults and resilience in others. Thus, the objectives of this study were to: (1) determine whether young adults and high- and low-functioning older adults show group differences in brain network segregation, and (2) determine whether network segregation is associated with working memory and walki

Pre- and post-season visio-vestibular function in healthy adolescent athletes.

OBJECTIVE: To evaluate pre - to post-season differences in individual subtests of the Visio-Vestibular Examination (VVE) in healthy middle and high school athletes. METHODS: This prospective cohort study recruited participants from a private suburban United States secondary school. Participants completed a demographic questionnaire prior to the start of their season. A proxy for head impact exposure was estimated by incorporating previously published head impact frequencies by team and sport. The VVE was completed pre - and post-season and consisted of 9 subtests: smooth pursuit, horizontal/v

Sensory control of balance: a comparison of children with fetal alcohol spectrum disorders to children with typical development.

BACKGROUND: Inefficient central processing and integration of visual, vestibular, and somatosensory information may contribute to poor balance and diminished postural control in children with fetal alcohol spectrum disorders (FASD). OBJECTIVES: This pilot study examined sensorimotor performance and the sensory control of balance using a battery of clinical tests in combination with an experimental laboratory assessment that quantifies sensory subsystem use (i.e., sensory weighting) among a systematically diagnosed sample of children with FASD and children with typical development. METHODS: U

Aging attenuates the vestibulosympathetic reflex in humans.

BACKGROUND: The vestibular system contributes to sympathetic activation by engagement of the otolith organs. However, there is a significant loss of vestibular function with aging. Therefore, the purpose of the present study was to determine if young and older individuals differ in their cardiovascular and sympathetic responses to otolithic stimulation (ie, head-down rotation, HDR). We hypothesized that responses to otolithic stimulation would be attenuated in older adults because of morphological and physiological alterations that occur in the vestibular system with aging. A Prospective Study on the Vestibular Toxicity of Gentamicin in a Clinical Setting.

OBJECTIVE: Gentamicin is a widely used aminoglycoside with ototoxicity as a known adverse effect. Because of the difficulty in clinical recognition, the prevalence of gentamicin ototoxicity in practice is thought to be higher than reported. This study aimed to prospectively assess the effect of gentamicin on vestibular function and whether ototoxicity is underrecognized. STUDY DESIGN: Single-center, prospective, nonblinded trial. SETTING: Inpatient tertiary hospital setting followed by vestibular outpatient clinic review. PATIENTS: Forty-eight patients undergoing a urologic procedure were r

Comparison of additive and subtractive digital workflow for the accuracy of removable occlusal stabilization-splints - an in-vitro study.

OBJECTIVES: Patients with stage IV periodontitis often require additional rehabilitation of the compromised dentition due to anatomical and functional consequences of tooth loss. Removable occlusal splints can be helpful in these advanced cases. This in-vitro study compared the accuracy and internal fit of removable occlusal splints fabricated by 3D-printing and milling from a standardized STL-file. MATERIALS AND METHODS: A standard-tessellation-language (STL)-file was created from a scanned master-model of the upper jaw and used for production of forty identical splints. Twenty splints were

Skull Vibration Induced afternystagmus: A new clinical indicator of superior canal dehiscence.

Vibrations applied to the cranium induce, in 55%-92% of patients with CT-verified superior canal dehiscence (SCD), a perstimulatory nystagmus most often ipsilaterally beating when the vertex location is stimulated at 100Hz. The skull vibration-induced nystagmus (SVIN) test is a bone-conducted (BC) Tullio phenomenon in patients with a 3rd mobile window syndrome (TMWS). Here, we describe the technical cues and procedures to obtain an afternystagmus regularly observed in 55% of a series of 43 patients with SCD chosen as examples and stimulated at 100Hz and higher frequencies by bone-conducted vib

Prediction of Balance Compensation After Vestibular Schwannoma Surgery.

Background Balance compensation after vestibular schwannoma (VS) surgery is under the influence of specific preoperative patient and tumor characteristics. Objective To prospectively identify potential prognostic factors for balance recovery, we compared the respective influence of these preoperative characteristics on balance compensation after VS surgery. Methods In 50 patients scheduled for VS surgical ablation, we measured postural control before surgery (BS), 8 (AS8) days after, and 90 (AS90) days after surgery. Based on factors found previously in the literature, we evaluated age, body m

Vestibular evoked myogenic potentials in benign paroxysmal positional vertigo and Meniere's disease.

The objective was to investigate vestibular evoked myogenic potentials (VEMP) in benign paroxysmal positional vertigo (BPPV) and Meniere's disease, and to determine if this type of testing is valuable for assessing the vestibular system. A prospective controlled clinical study was designed in a tertiary referral center setting. The 62 participants included 17 healthy controls and 45 other subjects selected from patients who presented with the complaint of vertigo (25 diagnosed with BPPV and 20 diagnosed with Meniere's disease). The main outcome measures of VEMP were recorded in all subjects an

Narrow internal auditory canal syndrome: parasaggital reconstruction.

Narrow internal auditory canal (IAC) syndrome is a malformation of the temporal bone, that is defined as an IAC diameter of only 1-2 mm on high-resolution computed tomographic scans (HRCT). This syndrome is known to be caused by the absence (aplasia or hypoplasia) of the vestibulocochlear nerve. We present a case of unilateral narrow IAC syndrome which was diagnosed by HRCT. The aplasia of the vestibulocochlear nerve was confirmed using parasigittal reconstruction magnetic resonance image (MRI). The IAC was composed of two separate canals, one of which contained a facial nerve and the other wa
