Five cases of delayed nonexpulsive suprachoroidal hemorrhage after trabeculectomy in aphakic eyes are reported. Four cases were done under general anesthesia and in three there was straining and bucking at extubation. The most common presentation was sudden severe ocular pain one day postoperatively, associated with marked decrease in vision and low intraocular pressure. The prognosis was related to the extent of the hemorrhage; where suprachoroidal hemorrhage was extensive, surgical drainage appeared to help. Two patients with visual loss after cataract extraction were found to have prominent pallid optic disc edema, peripapillary hemorrhage, and cystoid maculopathy. We retrospectively studied 61 consecutive cases of penetrating keratoplasty performed for aphakic bullous keratopathy or pseudophakic bullous keratopathy. After keratoplasty, the mean visual acuities of both groups of patients improved significantly from preoperative levels (P less than .0001). 65 aphakic patients (89 eyes) wore extended wear contact lenses for an average period of 22.8 months. 74% of the patients developed one or more readily detectable ocular changes during the course of the study. We studied the incidence of rhegmatogenous retinal detachment in Rochester, Minnesota, for the nine-year period from 1970 through 1978. The age-adjusted rate for idiopathic rhegmatogenous retinal detachment was 10.1 per 100,000 population per year (45 cases). A survey of physicians in several retinal centers in this country provided information on more than 500 patients who underwent photocoagulation treatment primarily for retinal, vascular, and macular diseases, and in some instances, for retinal breaks. In the last four years we have treated with the mobile argon-laser beam 25 cases of aphakic pupillary block glaucoma (p. b. g.). An attempt is made to depolarize current positions regarding intraocular implant lens surgery by presenting a reasonable list of advantages and disadvantages of the procedure. A current position on this controversial subject is presented and is based on currently available knowledge. A birth of interest in lens implantation occurred especially in the United states in 1974. This coincides with the beginning of four major American lens manufacturers.
