Bidirectional Glenn for residual outflow obstruction in Tetralogy of Fallot.

BACKGROUND: Residual right ventricular outflow obstruction during Tetralogy of Fallot repair necessitates peri-operative revision often requiring trans-annular patch with its negative sequels. Bidirectional Glenn shunt in this setting reduces trans-pulmonary gradient to avoid revision. METHODS: Bidirectional Glenn shunt was added during Tetralogy repair in patients with significant residual obstruction. A total of 53 patients between January, 2011 and June, 2018 were included. Final follow-up was conducted in July, 2018. RESULTS: Mean age at operation was 5.63±3.1 years. Right to left ventri

Accuracy of computed tomography in detection of great vessel stenosis or hypoplasia before superior bidirectional cavopulmonary connection: Comparison with cardiac catheterization and surgical findings.

BACKGROUND: Cardiac catheterization is the gold-standard modality for investigation of cardiovascular morphology before bidirectional cavopulmonary connection, but requires general anaesthesia and is associated with procedural risk. AIMS: To assess the diagnostic accuracy and safety of computed tomography in diagnosing great vessel stenosis/hypoplasia compared with cardiac catheterization and surgical findings. METHODS: Twenty-seven patients (10 after Norwood stage I) underwent computed tomography before surgery between January 2010 and June 2016; 16 of these patients also underwent cardiac

Echo-Doppler assessment of ventricular filling pressures in adult Fontan patients.

BACKGROUND: Diastolic dysfunction is felt to be part of the natural history of patients with prior Fontan operation. Despite that, data on noninvasive assessment of diastolic function and ventricular filling pressures in Fontan patients are limited. METHODS: We included 45 adult Fontan patients who underwent right heart catheterization with pulmonary artery wedge pressure (PAWP) measurement and transthoracic echo-Doppler assessment within 7 days. Offline measurement of systemic atrioventricular valve pulsed-wave (PW) and medial/lateral atrioventricular annular tissue Doppler velocities was pe

A composite noninvasive index correlates with liver fibrosis scores in post-Fontan patients: Preliminary findings.

OBJECTIVE: We hypothesized that clinic-based, hepatic-ultrasound, elastography measurements, either alone or in combination with other noninvasive variables, might correlate with liver-biopsy fibrosis scores in patients post-Fontan. METHODS: Between March 2012 and February 2017, we identified patients post-Fontan that underwent elective cardiac catheterization and simultaneous transvenous hepatic biopsy. From this group, we selected patients that met inclusion criteria for liver-ultrasound, shear-wave elastography. Utilizing the results of elastography, laboratory testing, and time post-Fonta

Fontan-associated protein-losing enteropathy and heart transplant: A Pediatric Heart Transplant Study analysis.

BACKGROUND: Post-Fontan protein-losing enteropathy (PLE) is associated with significant morbidity and mortality. Although heart transplantation (HTx) can be curative, PLE may increase the risk of morbidity before and after HTx. This study analyzed the influence of PLE influence on waiting list and post-HTx outcomes in a pediatric cohort. METHODS: Fontan patients listed for HTx and enrolled in the Pediatric Heart Transplant Study from 1999 to 2012 were stratified by a diagnosis of PLE, and the association of PLE with waiting list and post-HTx mortality, rejection, and infection was analyzed.

Iron deficiency and hematological changes in adult patients after Fontan operation.

BACKGROUND: Growing evidence indicates that iron-deficiency anemia is common in patients with congenital heart diseases. The aim of this study was to characterize hematologic changes and iron metabolism in adult Fontan patients. We also searched for the associations between these parameters and physical performance in the study group. METHODS AND RESULTS: Thirty-two white Fontan patients with a mean age of 25 ± 4.5 years and 30 healthy control subjects matched for age and sex were studied. Complete blood count together with iron-related parameters was determined in plasma of peripheral venous

Protein-losing enteropathy after the Fontan operation: associations and predictors of clinical outcome.

OBJECTIVE: We sought to define what clinical parameters were related to the ultimate outcome in Fontan patients who had developed protein-losing enteropathy (PLE). BACKGROUND: PLE is a serious complication of the Fontan operation. Several preoperative and perioperative findings are associated with later PLE. However, there is limited information regarding postoperative abnormalities contributing to or influencing outcome in PLE. METHODS: We evaluated 44 consecutive Fontan patients with PLE. A matched control group of Fontan patients without PLE was used for comparison (matched-pair analyses)

The beneficial effects of total cavopulmonary conversion and arrhythmia surgery for the failed Fontan.

Postoperative Fontan patients can develop hemodynamic abnormalities and refractory atrial arrhythmias resulting in significant morbidity and mortality. We present our experience with total cavopulmonary artery conversion and arrhythmia surgery. Between 1994 and 2001, 41 patients underwent total cavopulmonary artery conversion and arrhythmia surgery. Significant hemodynamic lesions were repaired concomitantly: aortic aneurysm (n=1), atrioventricular valve insufficiency (n=8), and pulmonary artery stenosis (n=9). Thirty-five patients were in New York Heart Association class III or IV. Mean age a

Potential goals for the dimensions of the pulmonary arteries and aorta with stenting after the Fontan operation.

The purpose of this study was to clarify desired stent sizes for stenotic lesions in the post-Fontan circulation. Using angiograms from 22 patients before and at late follow-up (> or = 15 years) after the Fontan operation, we measured the maximum diameters of the proximal pulmonary arteries (PA) and the descending aorta. The diameters of the PA ipsilateral to the inferior vena cava, contralateral to the inferior vena cava, and descending aorta after the Fontan were 10.6-22.6 (15.8 +/- 3.3), 8.0-19.1 (12.9 +/- 3.1), and 12.1-18.9 (15.8 +/- 2.0) mm, respectively, while the percent of normal pred

Does single ventricle physiology affect survival of children requiring extracorporeal membrane oxygenation support following cardiac surgery?

BACKGROUND: Improved survival with postoperative extracorporeal membrane oxygenation (ECMO) has expanded its application to children with single ventricle (SV) anomalies. We examine current-era outcomes of postoperative ECMO with special focus on patients with SV. METHODS: Demographic, anatomic, surgical, and support details of 100 consecutive children requiring postoperative ECMO (2007-2012) were included into multivariable regression models to identify factors affecting survival. RESULTS: Median age was 73 days (4 days-16.2 years), 31 patients had SV physiology. The ECMO indication was fai

Double-barrel Damus-Kaye-Stansel operation is better than end-to-side Damus-Kaye-Stansel operation for preserving the pulmonary valve function: the importance of preserving the shape of the pulmonary sinus.

OBJECTIVE: The Damus-Kaye-Stansel operation sometimes results in deteriorating semilunar valve insufficiency. We verified the semilunar valve function after the Damus-Kaye-Stansel operation and compared the end-to-side Damus-Kaye-Stansel with the double-barrel Damus-Kaye-Stansel. METHODS: Forty-seven patients who underwent the Damus-Kaye-Stansel operation between June 1993 and August 2008 were retrospectively reviewed. Any patient who underwent a Norwood-type operation was excluded. The median age at operation was 19 months (range, 0-276 months). Forty-five patients were Fontan candidates. Th

Magnetic Resonance Imaging-Based Psoas Muscle Volume as an Additional Risk Factor for Fontan Failure: A Prospective Cohort Study.

BACKGROUND: Sarcopenia is prevalent in patients with Fontan circulation and may contribute to functional decline and adverse outcomes. It is commonly assessed using the cross-sectional area of the psoas muscle (PMA), although volumetric assessment of psoas muscle volume (PMV) may better capture true muscle mass. The ability of PMA versus PMV to differentiate patients with Fontan circulation from healthy controls and to provide incremental prognostic information remains incompletely defined. METHODS: In this prospective study, abdominopelvic magnetic resonance imaging was used to quantify PMA

Lung function and exercise capacity in single ventricle patients post Fontan palliation and potential role of diaphragm ultrasound imaging.

AIMS: Patients with Fontan circulation have reduced functional capacity, due to a reduced cardiac output, but also to an abnormal lung function and respiratory muscle weakness. This study aimed to assess lung function and exercise capacity in a cohort of patients with Fontan circulation and evaluate the prognostic impact of restrictive lung function (RLF). In addition, diaphragmatic ultrasound (DUS) investigated diaphragmatic function in a subgroup of patients. METHODS: Patients with Fontan palliation who underwent lung function evaluation were included. A subgroup of patients additionally un

Risk Factors for Adverse Outcomes in a Fontan Population.

Improved survival following Fontan surgery is tempered by late Fontan failure. Heterogeneous multi-organ complications contribute to eventual Fontan failure varying over time. The prevalence of Fontan-related complications and their association with adverse outcomes were evaluated. A Fontan cohort of 668 pediatric (n = 490) and adult (n = 178) patients from two healthcare systems were linked to 2010-2019 healthcare encounters and death certificates. Bivariate analyses examined covariates by the primary composite outcome of heart transplant, hepatocellular carcinoma (HCC) or death. Multivariate
