PURPOSE: This study describes a case series of 15 patients with radial head fractures who underwent radial head excision using an arthroscopic technique.
METHODS: Over a four year period, 15 patients (average age 49.6 years) who had sustained a radial head or radial neck fracture underwent an arthroscopic excision of the radial head. Four patients had an unreconstructable comminuted fracture (early group; EG) and 11 patients had pain and loss of motion with an associated non- or malunion (late group; LG).
RESULTS: The mean time to surgery following injury was three weeks (one to five) for th

Forearm shaft fractures are the most common fractures of the upper extremity in young adults. By definition, these fractures are diaphyseal fractures; however, due to the complex functional unity formed by the forearm shaft during motion both bone forearm fractures are treated as intra-articular fractures [1, 3]. This is why the gold standard of treatment in adults is osteosynthesis. The aim of the surgical intervention is the anatomical reduction with exact reconstruction of length, shaft axis and rotation. This particularly concerns Monteggia and Galeazzi fractures where the adjacent joint n

This case involves a 58-year-old man who sustained a severe left forearm injury from a motor vehicle accident. Imaging revealed comminuted fractures and arterial occlusions in the left forearm. As the injury was crush type and exploration revealed vessels with avulsion, the decision was made to perform amputation due to nonsalvageability. The patient underwent irrigation, debridement, and amputation, followed by coverage via a full-thickness skin graft from the amputated hand. Postoperatively, the patient received antibiotics and wound care, resulting in successful graft integration and healin

INTRODUCTION: Bridge plating of distal femur fractures with lateral locking plates is susceptible to varus collapse, fixation failure, and nonunion. While medial and lateral dual plating has been described in clinical series, the biomechanical effects of dual plating of distal femur fractures have yet to be clearly defined. The purpose of this study was to compare dual plating to lateral locked bridge plating alone in a cadaveric distal femur gap osteotomy model.
MATERIALS AND METHODS: Gap osteotomies were created in eight matched pairs of cadaveric female distal femurs (average age: 64 yrs (

We describe a low-cost (instrument cost) technique for joint distraction using 2 Kirschner wires and a toothed lamina spreader in lieu of a Hintermann distractor. The described technique allows for temporary intra-articular distraction and visualization and preservation of the articular surface with extra-articular instrumentation. The technique can also allow for closed reduction and percutaneous treatment in cases of soft tissue compromise. Additionally, the technique uses common orthopedic surgical instruments, leading to a minimal learning curve for novice surgeons. We have found this dist

The literature pertaining to pediatric skull fracture is primarily clinically based and thus motivated by the need for effective assessment of both fracture characteristics (type, frequency, location, and mechanics) and context (severity of injury, associated soft tissue damage, and prognosis). From a strictly descriptive standpoint, these schemas employ overlapping levels of detail that confound the nonclinical description of fractures in the forensic context. For this reason, application of these schemas in the forensic anthropological interpretation of skull fractures is inappropriate. We a

UNLABELLED: The literature regarding reconstruction of foot bone defects is limited. The purpose of this study is to present a case report with an extensive bone defect of the first metatarsal bone which was treated with the use of the induced membrane technique. A 53-year-old man, with comminuted foot grade IIIb open fracture was treated with the Masquelet procedure. At 14 months follow-up, clinical and radiological assessment of the foot revealed osseous healing and no signs of infection, osteolysis or hardware failure. At 18 months follow-up, the patient had no pain and returned to his usua

An 80-year-old man with pancreatic adenocarcinoma and depression was found with his head impaled on a bolt that had been screwed into a hole that had been drilled in the floor of a shed at his home address. Once the bolt was in place the decedent had winched a heavy weight above it, using a pulley that he had attached to the metal roof frame, and the front fork of a bicycle frame. The latter had been bolted to a nearby work bench as a winching device. After the weight had been positioned, he had placed his head over the bolt and cut the rope with a kitchen knife. The impact of the falling weig

This study investigated the optimal cast immobilization position for conservative treatment of Colles' fracture using finite element analysis (FEA). Ten upper limbs from cadavers were scanned with computed tomography in five palmar-dorsiflexion positions (V30, V15, DV0, D15, and D30) and five radioulnar deviation positions (U20, U10, RU0, R10, and R20). Colles' fracture models with comminuted dorsal cortex and intact volar cortical continuity were developed and subjected to FEA. Each position was assessed for fragment displacement, principal strain at the fracture site, and equivalent stress p

INTRODUCTION: Coronal shear fractures of the distal humerus are uncommon injuries representing 6% of distal humeral fractures. There is no univocal consensus about the correct management of this type of fracture. A national survey was conducted to gain more insight into the current classification, diagnosis and treatment of coronal shear fractures in Italy.
MATERIALS AND METHODS: A postal survey was sent to all AO Italian members including residency orthopaedic surgeons. The survey consisted of general questions about personal experience in the management of these fractures: types of classifi

This paper presents the outcomes of a conservative approach to a bilateral condylar fracture in an edentulous patient. Serial changes of the inclination angle were measured in an edentulous mandibular fracture treated with a monoblock Gunning splint. After being hit by a motor vehicle, a 67-year-old man fell onto his face. He had been edentulous for 7 years and did not use dentures. His mouth-opening was 28 mm between the edentulous maxilla and the mandible. X-rays and computed tomography revealed a comminuted mandibular angle fracture and bilateral condylar fractures. Stone casts were made fro

Fractures of talar body are uncommon injuries often associated with fractures of other long bones and in polytraumatized patients. The integrity of the talus is essential for the normal function of the ankle, subtalar, and midtarsal joints. The relative infrequency of this injury limits the number of studies available to guide treatment. They occur as a result of high-velocity trauma and are therefore associated with considerable soft tissue damage. Axial compression with supination or pronation is the common mechanism of injury. Great care is necessary for diagnosing and treating these injuri

The management of radial nerve palsy associated with fractures of the shaft of the humerus has been disputed for several decades. This study has systematically reviewed the published evidence and developed an algorithm to guide management. We searched web-based databases for studies published in the past 40 years and identified further pages through manual searches of the bibliography in papers identified electronically. Of 391 papers identified initially, encompassing a total of 1045 patients with radial nerve palsy, 35 papers met all our criteria for eligibility. Meticulous extraction of the
