Chlorhexidine is a widely used and effective antiseptic agent. Although skin contact is usually well tolerated, it may cause both immediate and delayed hypersensitivity reactions. We report a case of immediate hypersensitivity to chlorhexidine causing both skin and respiratory symptoms following occupational exposure to chlorhexidine in a health-care worker.

BACKGROUND: Paraneoplastic dermatoses may be the first manifestation of a malignancy. Rapid recognition is therefore important.
CASE DESCRIPTION: A 72-year-old woman who had been treated in the past for metastasised neuroendocrine carcinoma and had undergone curative treatment for stage-1c endometrial carcinoma developed three paraneoplastic dermatoses over a 2-year period: acanthosis nigricans, tripe palms and acquired hypertrichosis lanuginosa. Half a year later, cutaneous metastases derived from the endometrial carcinoma treated 12 years prior were discovered.

BACKGROUND: Although they know the long-term complications of smoking, many smokers require additional motivation for entering a smoking cessation programme. At the same time, smoking is considered a possible promoting factor for various skin diseases.
OBJECTIVE: The present study assesses the impact of smoking on occupational hand dermatitis in a high-risk population.
MATERIAL AND METHODS: 1,355 metal workers, 97.3% of them shift workers, took part in a standardized interview and a dermatological examination of their hands. A subgroup of 1,020 males was willing to be followed-up for 1 year.

Adams-Oliver syndrome is a rare congenital anomaly characterized by aplasia cutis congenita (ACC) and variable degrees of terminal transverse limb defects. We report on a neonatal case with the sporadic form of the disease with minimal expression, illustrating the wide spectrum of clinical expression in Adams-Oliver syndrome. We also review the literature and highlight the different pathogenetic hypotheses of this syndrome.

A child owning pet rats developed an eruptive fever with blisters, polyarthritis, and spectacular desquamation of the hands. Streptobacillus moniliformis was identified after culture of the child's blister fluid and was detected in rat samples by molecular methods. Asparagus (Asparagus officinalis),a member of the Liliaceae family, is a popular vegetable, most commonly eaten in May. A 55-year-old cook presented with seasonal (always in May) recurrent dyshidrosiform hand eczema which prevented him from working. He also reported developing dyspnea and dysphagia after consuming asparagus. Diagnostic allergy testing revealed positive reactions against asparagus in prick-to-prick and epicutaneous patch tests.
