Effects of massive doses of ergocalciferol plus cholesterol on pregnant rats and their offspring.

Ergocalciferol (320,000 or 480,000 IU/kg) plus cholesterol (60 mg/kg) in olive oil solution was administered daily on 1, 2, or 4 consecutive days to pregnant rats from 9,10, 14, or 18 of gestation. The control animals received only olive oil. Disseminated lesions of metastic calcinosis were found in various tissues, in the coronary arteries and myocardium, in the media of the abnormal aorta, in the lung and pleura, in the gastoinstestinal tract, and in the kidney. This is in contrast to the atherosclerosis described in nonpregnant rats fed a similiar diet. A significant decline in maternal wei

Retrospective Cohort Study on the Optimal Timing of Orogastric Tube/Nasogastric Tube Insertion in Infants With Pyloric Stenosis.

BACKGROUND: Hypertrophic pyloric stenosis in infants can cause a buildup of gastric contents. Orogastric tubes (OGTs) or nasogastric tubes (NGTs) are often placed in patients with pyloric stenosis before surgical management to prevent aspiration. However, exacerbation of gastric losses may lead to electrolyte abnormalities that can delay surgery, and placement has been associated with increased risk of postoperative emesis. Currently, there are no evidence-based guidelines regarding OGT/NGT placement in these patients. This study examines whether OGT/NGT placement before arrival in the operati

False-positive pH aspirates after nasogastric tube insertion in head and neck tumour.

Nasogastric (NG) feeding tubes are commonly inserted to supplement enteral nutrition in certain patient groups, including those with head and neck cancers where swallowing may be compromised. An NHS National Patient Safety Alert was released in 2011 detailing ongoing cases of significant morbidity and mortality attached to the incorrect placement of NG feeding tubes in hospital inpatients. Since 2005, there were 21 deaths and 79 cases of harm nationally due to feeding into the lung through misplaced tubes. pH testing remains the first-line method of placement confirmation, with chest x-ray use

Safe practices for enteral nutrition in critically ill patients.

Promoting patient safety in the enterally fed critically ill patient is dependent on nursing surveillance and recognition of potential areas of patient harm and medical errors. Identifying areas for potential human error, administrative and organizational conditions that are conducive to error, and the patient's own tolerance to EN need to be recognized by the critical care nurse if the risk for EN complications is to be minimized.

The buried bumper syndrome: the usefulness of retrieval PEG tubes in its management.

Percutaneous endoscopic gastrostomy is a safe and easy method and carries a low mortality and complication rate. The buried bumper syndrome is a rare and late complication of percutaneous endoscopic gastrostomy tube placement. An 80-year-old man with bilateral basal ganglia bleeding was unable to swallow safely and required tube feeding. A Flexiflo Inverta percutaneous endoscopic gastrostomy tube was successfully inserted by pull technique. One year later, he was readmitted to our clinic because of nonfunctioning tube and peristomal cellulites. Endoscopy demonstrated dimpling of the gastric mu

Dietary trans-resveratrol bioavailability and effect on CCl4-induced liver lipid peroxidation.

BACKGROUND AND AIM: Several in vitro studies have demonstrated the ability of pure trans-resveratrol (t-Res) to act as an anti-oxidant, but the scientific literature is lacking in in vivo studies dealing with dietary t-Res bioavailability in oxidative stress models. Our aim was to investigate the bioavailability of t-Res from dietary sources and its effect on an animal model of carbon tetrachloride (CCl4)-induced liver lipid peroxidation. METHODS: Ten rats were intragastrically administered for 14 days with a grape-stalk extract determining a daily t-Res dosage of 3 mg/kg. The control group (

Is nasogastric tube drainage required after reconstructive surgery for neurogenic bladder dysfunction?

OBJECTIVES: To determine whether the routine use of nasogastric tubes (NGTs) after bowel surgery for neurogenic bladder dysfunction improves outcomes. METHODS: We retrospectively evaluated 54 consecutive patients (30 women, 24 men) with neurogenic bladder who underwent bladder reconstruction or replacement with bowel segments by one surgeon from December 2000 to August 2005. The first 32 [NGT(+)] had NGTs placed during the procedure, whereas the subsequent 22 [NGT(-)] did not. We compared short-term postoperative outcomes between groups. RESULTS: Patient age ranged from 17 to 74 years (mean,

Transpyloric feeding in gastroesophageal-reflux-associated apnea in premature infants.

OBJECTIVE: The aetiological role of gastroesophageal reflux in apnea of prematurity is controversial. We hypothesized that transpyloric feeds, which decreases reflux and aspiration, will not be associated with decrease in reflux-related apnea. STUDY DESIGN: The shows retrospective chart review of 41 premature babies on transpyloric feeds. Fifteen infants meeting the inclusion criteria of apnea of prematurity and clinical evidence of gastroesophageal reflux were included. Primary data points were number of apneas before and after transpyloric feeds. t-statistics was used for analysis. RESULTS

Early osmoregulatory stimulation of neurohypophyseal hormone secretion and thirst after gastric NaCl loads.

Cerebral osmoreceptors mediate thirst and neurohypophyseal secretion stimulated by increases in the effective osmolality of plasma (P(osmol)). The present experiments determined whether an intragastric load of hypertonic saline (ig HS; 0.5 M NaCl, 4 ml) would potentiate these responses before induced increases in P(osmol) in the general circulation could be detected by cerebral osmoreceptors. Adult rats deprived of water overnight and then given intragastric HS consumed much more water in 15-30 min than rats given either pretreatment alone, even though systemic P(osmol) had not yet increased s

[Incidence of salivary fistulas in relation to timing of oral nutrition after laryngectomy].

BACKGROUND AND OBJECTIVE: A pharyngocutaneous fistula is the most common complication after total laryngectomy. In Germany, a traditional recommendation is to use a nasogastric tube for feeding for 10-14 days postoperatively because many surgeons believe that oral feeding after surgery contributes to fistula development. However, there is no international agreement about when to begin oral feeding after total laryngectomy. Some authors begin oral feeding between the 1st and 4th postoperative day without any nasogastric tube, while others using a nasogastric tube delay oral feedings until 7-14

Acute methanol intoxication generates free radicals in rats: an ESR spin trapping investigation.

Electron spin resonance (ESR) spectroscopy has been used to investigate free radical generation in rats with acute methanol poisoning. The spin trapping technique was used where a spin trapping agent, alpha-(4-pyridyl 1-oxide)-N-tert-butylnitrone (POBN), reacted with the corresponding alcohol-derived or alcohol-dependent radical to form radical adducts. One radical adduct was detected in both bile and urine samples 2 h after acute methanol poisoning in male Sprague Dawley rats. The hyperfine coupling constants for the radical adduct from [(13)C]-labeled methanol detected in the bile were a(N)

[Gastrostomy instead of gastric intubation for stomach decompression after large urologic operations].

In 31 patients who had undergone extensive intraabdominal urological surgery, a gastrostomy was performed for gastric decompression instead of introduction of a nasogastric tube. There were no complications during the procedure. All patients tolerated the gastrostomy well. Two were fed through the gastrotomy. We conclude, that gastrostomy is a safe method of gastric decompression.

Electromagnetic Versus Blind Guidance of a Postpyloric Feeding Tube in Critically Ill Children.

BACKGROUND AND OBJECTIVES: Postpyloric feeding tube placement is a time-consuming procedure associated with multiple attempts and radiation exposure. Our objective with this study is to compare the time, attempts, and radiation exposure using the electromagnetic versus blind method to place a postpyloric feeding tube in critically ill children. Our hypothesis is that using electromagnetic guidance decreases the procedure time, number of x-rays, and number of attempts, compared to the blind method. METHODS: Eleven pediatric nurses participated in a randomized controlled intention-to-treat stud

Preoperative Educational Intervention Decreases Unplanned Gastrostomy-Related Health Care Utilization.

Apprehension in taking independent care of children with medical devices may lead to unnecessary visits to the ED and/or acute clinic (AC). To address these concerns, our institution implemented a gastrostomy tube (GT) class in 2011 for caretakers. We hypothesized that inappropriate GT-related ED/AC visits would be lower in preoperatively educated caregivers. We performed a retrospective cohort study of all patients aged 0 to 18 who received GT (surgical or percutaneous) at our institution between 2006 and 2015 (n = 1340). Class attendance (trained vs untrained) and unscheduled GT-related ED/A

Prospective evaluation of clinical outcomes and quality of life after gastric tube interposition as esophageal reconstruction in children.

Few studies on gastric tube interposition for esophageal reconstruction in children have assessed the long-term outcomes and quality of life (QoL). The aim of this study is to evaluate the long-term outcomes and QoL after a gastric tube interposition by reviewing our experiences with esophageal reconstruction.Twenty-six patients were included who underwent gastric tube interposition from 1996 to 2011 at our institution. We reviewed the medical records and conducted telephone surveys, prospectively performed esophagography, endoscopy, 24-hour pH monitoring, and esophageal manometry. The median
