Diagnosis of ALG12-CDG by exome sequencing in a case of severe skeletal dysplasia.

Congenital Disorder of Glycosylation type Ig (ALG12-CDG) is part of a group of autosomal recessive conditions caused by deficiency of proteins involved in the assembly of dolichol-oligosaccharides used for protein N-glycosylation. In ALG12-CDG, the enzyme affected is encoded by the ALG12 gene. Affected individuals present clinically with neurodevelopmental delay, growth retardation, immune deficiency, male genital hypoplasia, and cardiomyopathy. A total of six individuals have been reported in the literature. Here, we present an infant with rhizomelic short stature, talipes equinovarus, platys

A shortened Knee Injury and Osteoarthritis Outcome Score (KOOS) is sufficient for measuring change in a cohort of patellofemoral instability patients.

INTRODUCTION: Various knee-related patient-reported outcome measures (PROMs) have been used for patients who undergo surgical treatment for patellofemoral instability. There has been limited evaluation of their suitability and no agreement about an optimal set of measures. OBJECTIVES: To evaluate the Knee injury and Osteoarthritis Outcome Score (KOOS) and published shorter versions to determine their suitability for patients with patellofemoral instability. METHODS: The study cohort consisted of patients who underwent surgical stabilization with medial patellofemoral ligament reconstruction

MRI of the knee with arthroscopic correlation.

Major advances in MRI and arthroscopy have allowed for enhanced diagnosis and subsequent management of ligamentous and soft tissue injuries of the knee. Recognition of the appearance of acute ACL and PCL injuries on MRI can enhance arthroscopic reconstruction. PCL injuries are often more subtle and can present with indirect signs. T2-weighted MRI imaging can examine which structures have been damaged in the posterolateral corner which may manifest arthroscopically as a drive-through sign. Characterization of PLC, meniscus, MCL injuries and OCD lesions on MRI have remarkable correlation with ar

Vertical dislocation of the patella: report of 2 cases.

Vertical dislocation of the patella is rare and usually occurs as a result of blunt trauma. The patella rotates around a vertical axis. It can be diagnosed by its typical clinical features. This article presents a case of 2 patients with vertical dislocation who presented with high-impact injuries and had classic signs of a vertically dislocated patella. Unlike lateral patella dislocation, reduction under sedation is usually not possible. This is mainly due to increased tension within the quadriceps mechanism, which makes reduction difficult. The other factor that makes reduction difficult is

Use of carbon fibers in the reconstruction of knee ligaments.

Carbon fibers were used to reconstruct the anterior cruciate ligament and other knee ligaments in a consecutive series of 26 (mostly acute) patients who had suffered serious occupation-related injuries. After an average follow-up time of 52.4 months (one patient lost, one patient refused follow-up examination), the anterior drawer at 30 degrees was less than 5 mm (side-to-side difference) in 14 patients, 5-10 mm in six patients, and greater than 10 mm in four patients. Of 24 evaluatable patients, 22 returned to work, including 20 patients who work underground. Chronic pain, recurrent effusion,

Arthroscopic resection of the shelf (mediopatellar plica) in the knee under local anesthesia in outpatient clinic.

Forty-two knees with symptomatic mediopatellar plicae (shelves) were managed using the operative method under local anesthesia. Of these, thirty-one knees had isolated mediopatellar plicae, and eleven had other associated intra-articular pathologic condition, such as meniscal tear, lateral patellar tracking, osteochondritis dissecans, and minor osteoarthritic changes. The clinical results were excellent or good in thirty-seven (88%) patients, fair in five patients after an average follow-up of twenty-five months. No case showed deterioration after surgery. Because there are no definitive crite

Posterior cruciate ligament reconstruction using a septum-preserving technique.

OBJECTIVE: Description of a reproducible surgical technique for single-bundle anterolateral reconstruction of the posterior cruciate ligament (PCL) based on a septum-sparing approach. This technique is less traumatic than the trans-septum approach. The article illustrates surgical steps to simplify the technical aspects of the procedure. INDICATIONS: A complete grade III symptomatic tear of the PCL associated with instability and often discomfort (deceleration, stairs) or subsequent gonalgia arising from the medial compartment or patellofemoral joint. Injury of the peripheral joint stabilizer

Measurement of TT-TG can change with sequential MRIs due to variations in tibiofemoral rotation in patellofemoral instability patients.

PURPOSE: There are various anatomic risk factors for patellofemoral instability (PFI) that help guide surgical treatment, including the tibial tubercle to trochlear groove (TT-TG) distance. However, no study has analysed the temporal changes in TT-TG prior to surgical intervention. This study sought to understand the variations in TT-TG over time for pediatric patients suffering from PFI prior to surgical intervention. The authors hypothesised that the TT-TG would substantially change between time points. METHODS: Patients undergoing medial patellofemoral ligament (MPFL) reconstruction betwee

Editorial Commentary: Magnetic Resonance Imaging Is Helpful in Predicting High-Grade Knee Rotatory Instability: But When in Doubt, Always Examine the Patient.

Unrecognized rotatory instability as evidenced by a high-grade pivot shift is well known to compromise anterior cruciate ligament (ACL) reconstruction results. By measuring which patients have anterior tibial subluxation of the lateral compartment ≥ 6 mm on a preoperative MRI, surgeons may be better able to counsel patients on postoperative expectations, as well as prepare for additional procedures to treat high-grade rotatory instability. Additionally, as there is an increased incidence of lateral meniscus tears in high-grade rotatory ACL lesions, surgeons should be vigilant and prepared to r

Revision lateralization osteotomy of the tibial tubercle has excellent outcomes in patients suffering from previous medial tibial tubercle overcorrection.

INTRODUCTION: Tibial tubercle osteotomy (TTO) is a common procedure used to treat patients with patellofemoral instability (PFI) and osteoarthritis (PFOA). Medial patellar maltracking due to previous excessive medialization of the tibial tubercle has rarely been reported. Therefore, the goal of this study was to assess patient-reported outcome measures (PROMs) after revision osteotomy with lateralization of the tibial tubercle (RL-TTO) to correct medial patellofemoral maltracking. MATERIALS AND METHODS: Between 2017 and 2021, a series of 11 patients (male/female 1/10; age 35.8 ± 10.5 years) w

Return to Play After Medial Patellofemoral Ligament Reconstruction: A Systematic Review.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is being performed more frequently in athletes experiencing recurrent patellar instability. PURPOSE/HYPOTHESIS: The purpose was to systematically review the evidence in the orthopaedic sports medicine literature to determine both the rate and timing of return to play after MPFL reconstruction and the rate of further patellar instability. Our hypothesis was that there would be a high rate of return to play after MPFL reconstruction. STUDY DESIGN: Systematic review. METHODS: A systematic literature search was performed based on

Modern Treatment of Recurrent Patellofemoral Instability - Combined Medial Patellofemoral Ligament Reconstruction and Tibial Tubercle Transfer.

PURPOSE OF THE STUDY The treatment of recurrent patellofemoral instability has undergone a paradigm change during the last 15 years. Instead of non-anatomical operations, the current concept favors the reconstruction of injured structures and the normalization of the biomechanical environment. Our aim was to briefly summarize the etiology, diagnostic regimen, and therapeutic decision making of this varied patient group and to review our own patients who underwent combined medial patellofemoral ligament reconstrucion and tibial tubercle transfer. MATERIAL AND METHODS Between 2015-2017 we perfor

Trochleoplasty in dysplastic knee trochlea.

In patients complaining of recurrent patellar dislocations or persistent retropatellar pain due to a dysplastic femoral trochlea, we perform a Henri Dejour trochleoplasty. In this technique the femoral trochlea is deepened by removing the subchondral trochlear bone followed by incision, impaction and fixation of the cartilage flange along the trochlear groove. Between 1996 and 1999, 13 procedures were performed in 12 patients. Strictly lateral X-rays showed dysplasia of the trochlea, as defined by the "crossing sign", whether or not in combination with patella alta. Patients were assessed usin

Recurrence after patellar dislocation. Redislocation in 37/75 patients followed for 6-24 years.

75 patients had closed treatment for a primary unilateral acute patellar dislocation. All patients were studied clinically, radiographically and for isokinetic thigh muscle performance a median of 11 (6-24) years later. During the follow-up, 37 patients had experienced recurrent dislocations. The functional outcomes were similar in patients with or without recurrence. Radiographically unstable patellar morphology, spontaneous reduction of the dislocation and low volume of hemarthrosis predisposed to recurrence.
