Should asymptomatic patients with congenital lung malformations undergo surgery? A decision analysis.

BACKGROUND: Congenital lung malformations (CLMs) are being detected more frequently during pregnancy. There is controversy regarding the optimal treatment for an asymptomatic child with prenatally diagnosed CLMs. Due to the paucity of information from clinical trials, we developed decision analytic models to compare two treatment strategies for such patients-elective surgical resection versus expectant management. METHODS: We built decision analytic models stratified by lesion size. We used data from English language literature identified through PubMed searches along with estimates from expe

Immersion in dual-language programs does not impede children's native language processing.

AIMS AND OBJECTIVES: The benefits of dual-language immersion (DLI) versus English-only classrooms for minority-language speakers' acquisition of English have been well documented. However, less is known about the effect(s) of DLI on majority-language speakers' native English skills. Prior studies largely used accuracy-focused measures to index children's language skills; it is possible that processing-based tasks are more sensitive to the effects of DLI experience. METHODOLOGY: Thirty-three monolingual native English-speaking children attending English-only classrooms and thirty-three English

AI-driven discovery of GPNMB CAR T cells as a multi-cancer therapy.

Chimeric antigen receptor (CAR) T cells have demonstrated curative potential in hematologic cancers and increasing efficacy in solid tumors and non-malignant diseases. However, target identification remains a major bottleneck. We developed an artificial intelligence (AI)-driven approach for CAR T cell target discovery by integrating single-cell RNA sequencing datasets from human skin cancer and healthy tissue. Candidates were refined using public datasets to optimize for tumor composition, tissue specificity, and clinical feasibility. Large language models were applied to prioritize and nomina…

Stress detection through prompt engineering with a general-purpose LLM.

Advancements in large language models (LLMs) have opened new avenues for mental health monitoring through social media analysis. In this study, we present an iterative prompt engineering framework that significantly enhances the performance of the general-purpose LLM, GPT-4, for stress detection in social media posts, leveraging psychologist-informed hints. This approach achieved a substantial 17% accuracy improvement from 72% to 89% for the January 2025 version of GPT-4, alongside an 80% reduction in false positives compared to baseline zero-shot prompting. Our method not only surpassed domai…

When helpfulness backfires: LLMs and the risk of false medical information due to sycophantic behavior.

Large language models (LLMs) exhibit a vulnerability arising from being trained to be helpful: a tendency to comply with illogical requests that would generate false information, even when they have the knowledge to identify the request as illogical. This study investigated this vulnerability in the medical domain, evaluating five frontier LLMs using prompts that misrepresent equivalent drug relationships. We tested baseline sycophancy, the impact of prompts allowing rejection and emphasizing factual recall, and the effects of fine-tuning on a dataset of illogical requests, including out-of-di…

Readability of Chatbot Responses in Prostate Cancer and Urological Care: Objective Metrics Versus Patient Perceptions.

Large language models (LLMs) are increasingly explored as chatbots for patient education, including applications in urooncology. Since only 12% of adults have proficient health literacy and most patient information materials exceed recommended reading levels, improving readability is crucial. Although LLMs could potentially increase the readability of medical information, evidence is mixed, underscoring the need to assess chatbot outputs in clinical settings. Therefore, this study evaluates the measured and perceived readability of chatbot responses in speech-based interactions with urological…

FGMDTI: Fine-grained Multimodal Learning for Drug-Target Interaction Prediction via Bidirectional Cross-Attention.

Drug-target interaction (DTI) prediction is critical for drug discovery and repositioning. Existing methods often rely on coarse-grained representations and fail to capture the reciprocal nature of molecular recognition. We propose FGMDTI, a fine-grained multimodal framework that models bidirectional interactions between drugs and proteins represented as functional units, each encoded through a dual-view feature representation that integrates structural features from pre-trained foundation models and semantic features from large language model (LLM)-based text embeddings. Results on five bench

PatternFormer: Learning Multiple Solution Patterns in Reaction--Diffusion Systems.

Many nonlinear models across physics, chemistry, and biology exhibit multiple solutions for the same parameters, and capturing this entire solution set is essential for understanding pattern-forming systems. Yet existing learned surrogates are fundamentally single-valued: neural operators map each parameter to a single output, and physics-informed neural networks converge to one branch. We develop PatternFormer (PF), a large language model-based framework for learning the multiple solutions of nonlinear partial differential equations. By transforming unordered coexisting solutions into canonic

Retrieval-augmented generation in medicine: A scoping review of technical implementations, clinical applications, and ethical considerations.

The rapid growth of medical knowledge and the increasing complexity of clinical practice pose challenges. In this context, large language models (LLMs) demonstrate value; however, inherent limitations remain. Retrieval-augmented generation (RAG) shows potential to enhance their clinical applicability. This study reviews RAG applications in medicine. We find that research primarily relies on publicly available data, with limited use of private data. For retrieval, approaches commonly rely on English-centric embedding models, while LLMs are mostly generic, with limited use of medical-specific LL…

Evaluating AI-Based Automated Essay Scoring Through Signal Detection Theory: Beyond Aggregate Agreement Metrics.

The rapid adoption of Large Language Models (LLMs) in educational assessment has reshaped scoring practices, yet evaluation remains tethered to aggregate reliability metrics like Quadratic Weighted Kappa, which obscure discrimination and rater effects. This study applies Signal Detection Theory to evaluate eight state-of-the-art LLMs (including Claude 3.5 Haiku, DeepSeek-V3, Gemini 3 Flash, GPT-4o, and Grok 4.1) against expert human raters across 1,726 essays. From Answers to Agents: What Must Change Before Generative and Agentic AI Become Clinical Infrastructure in Hematology.

Generative artificial intelligence in hematology is entering a new phase. The dominant question, whether large language models are accurate enough for clinical decision support, is being overtaken by a harder one, as systems shift from answering questions to acting: extracting structured cases, routing them, classifying variants, and grounding recommendations in guidelines and case memory. In 2026, a hematology agent achieved roughly 83% concordance with tumor-board decisions in a prospective silent trial, with hallucinations in 0.3%, suggesting that for well-structured tasks the binding const…

Speech clocks decode dementia phenotypes, social exposome, and biological aging.

Biological aging clocks offer estimations of aging and dementia, yet scalability is limited. We introduce a large-scale, cross-national speech clock derived from 2928 individuals across five Latin American countries, spanning healthy controls (HCs), mild cognitive impairment (MCI), Alzheimer's disease (AD), and non-language/language-dominant frontotemporal dementia (nldFTD/ldFTD).
