Online adaptive radiotherapy in pelvic and thoracic cancers - comparing toxicities, clinical outcomes and technical parameters between conventional image-guided radiotherapy and online adaptive radiotherapy - the study protocol for the prospective, registry-based cohort study (PRoART).

BACKGROUND: Conventional image-guided radiotherapy (IGRT) typically relies on a computed tomography (CT)-based treatment planning process (planning CT, pCT) performed prior to the start of treatment. During this process, a patient-specific treatment plan is generated, which is then delivered to the patient with a linear accelerator on a daily basis, usually with image guidance to compensate for variations in patient setup. However, daily interfractional anatomical variations in target position, shape, and volume, as well as in surrounding organs at risk (OARs), can only be addressed indirectly

Spectrum of Esophageal Motility Disorders on High-Resolution Manometry in Patients With Non-obstructive Dysphagia.

Background and objectives Esophageal motility disorders (EMDs) are a major cause of non‑obstructive dysphagia. However, regional data from eastern India are limited. This study aims to describe the spectrum of EMDs in patients with non‑obstructive dysphagia using high‑resolution manometry (HRM) at a tertiary care center in eastern India, and to compare clinical symptoms, and endoscopic and barium findings in patients with achalasia versus non‑achalasia. Methods This cross‑sectional observational study was conducted between August 2023 and March 2025 at a tertiary care center in eastern India.

Functional determinants of uterine contractility in endometriosis and adenomyosis: a systematic review and meta-analysis.

IMPORTANCE: Evidence suggests that aberrant uterine contractility in nonpregnant women with endometriosis and adenomyosis contributes to symptoms and potentially heralds their pathogenesis. However, uterine peristalsis remains understudied, inconsistently measured, and poorly understood. OBJECTIVE: To summarize evidence on uterine contractility across the menstrual cycle phases in women with endometriosis and adenomyosis. DATA SOURCES: PubMed/MEDLINE, Embase, and Scopus databases searched up to May 2, 2024. STUDY SELECTION AND SYNTHESIS: Observational studies compared quantitative measures

Is esophageal manometry essential for the diagnosis of achalasia? Identifying patients with achalasia by the esophageal clearance method.

BACKGROUND AND AIM: Achalasia is a disease characterized by inadequate relaxation of the lower esophageal sphincter (LES) and impaired peristalsis, for which esophageal motor function testing is essential in making a definitive diagnosis. However, the diffusion rate of esophageal pressure testing in Japan is low. We, therefore, examined whether achalasia could be identified by esophageal clearance testing with low-density barium (Timed Barium Esophagogram: TBE). MATERIALS AND METHODS: 126 cases (62 males, median age of 46 years), excluding those who had not undergone TBE during their initial

Patients with ineffective esophageal motility benefit from laparoscopic antireflux surgery.

BACKGROUND: Gastroesophageal reflux disease (GERD) is a common chronic disorder of the gastrointestinal tract, affecting more than 50% of Americans. The development of GERD may be associated with ineffective esophageal motility (IEM). The impact of esophageal motility on outcomes post laparoscopic antireflux surgery (LARS), including quality of life (QOL), remains to be defined. The purpose of this study is to analyze and compare QOL outcomes following LARS among patients with and without ineffective esophageal motility (IEM). METHODS: This is a single-institution, retrospective review of a p

Activation of intestinal spinal afferent endings by changes in intra-mesenteric arterial pressure.

KEY POINTS: A major class of mechano-nociceptors to the intestine have mechanotransduction sites on extramural and intramural arteries and arterioles ('vascular afferents'). These sensory neurons can be activated by compression or axial stretch of vessels. Using isolated preparations we showed that increasing intra-arterial pressure, within the physiological range, activated mechano-nociceptors on vessels in intact mesenteric arcades, but not in isolated arteries. This suggests that distortion of the branching vascular tree is the mechanical adequate stimulus for these sensory neurons, rather

Effects of heat stress on the antimicrobial drug resistance of Escherichia coli of the intestinal flora of swine.

The effects of heat stress on the antimicrobial drug resistance of Escherichia coli of the intestinal tract of swine were studied in animals from a farm that had not been supplementing antimicrobials in feed for the past 10 years. In one study, 10 finisher hogs were heat stressed (34 degrees C) for 24 h. Antimicrobial resistance levels after stress were significantly higher (P < 0.05) when compared with pre-stress levels for amikacin, ampicillin, cephalothin, neomycin and tetracycline from faecal samples. This high level of resistance persisted to slaughter that occurred at 10 days post-stress

Water-soluble contrast media in obstructed in ischemic small intestine. A clinical and experimental study.

In this study the attention was focused on the possible application of the new low-osmolar water-soluble contrast media in already existing routines for radiologic diagnostic work-up and management of the abdominal emergencies of simple intestinal obstruction and ischemia: Iohexol was a good, or better, alternative to sodium diatrizoate regarding taste acceptance and patient reactions: Seventy-five per cent of patients characterized the taste of iohexol as good or neutral, while 52% gave sodium diatrizoate similar scores. The scores were also consistently in favor of iohexol as compared with s

Pneumatosis Cystoides Intestinalis in Muscular Dystrophy and Congenital Myopathies: A Report of Five Cases.

Pneumatosis cystoides intestinalis (PCI) is a rare disease wherein air accumulates in the intestinal subserosa and submucosa, causing multiple gaseous cysts within the gastrointestinal wall. While PCI has various known risk factors, reports identifying muscular diseases as a factor are scarce. The aim of this study is to elucidate the clinical characteristics of PCI in muscle disease. We present a case series of five cases, including two cases of Duchenne muscular dystrophy (DMD) and three cases of rare congenital myopathies. All cases are of male patients, with poor intestinal peristalsis and

Differential sensitivity of gastric and small intestinal muscles to inducible knockdown of anoctamin 1 and the effects on gastrointestinal motility.

KEY POINTS: Electrical pacemaking in gastrointestinal muscles is generated by specialized interstitial cells of Cajal that produce the patterns of contractions required for peristalsis and segmentation in the gut. The calcium-activated chloride conductance anoctamin-1 (Ano1) has been shown to be responsible for the generation of pacemaker activity in GI muscles, but this conclusion is established from studies of juvenile animals in which effects of reduced Ano1 on gastric emptying and motor patterns could not be evaluated. Knocking down Ano1 expression using Cre/LoxP technology caused dramatic

Simplifying the evaluation of postprandial antral motor function in patients with suspected gastroparesis.

OBJECTIVES: Antral hypomotility is associated with symptoms of gastric stasis[fnc,1. AIMS: To quantitate antral motor function in patients with suspected gastroparesis due to idiopathic or secondary hypomotility; and to determine whether there are simpler indices to assess antral motility. METHODS: Standard eight-lumen antroduodenal manometry was performed in 67 patients for 3-h fasting and 2-h postprandial measurements. Antral motility 1 cm proximal to the pylorus was quantitated for a 2-h fed period as an index: MI = ln[(number contractions x (amplitudes) + 1]. Fifteen healthy volunteers s

Xiao Cheng Qi Decoction, an Ancient Chinese Herbal Mixture, Relieves Loperamide-Induced Slow-Transit Constipation in Mice: An Action Mediated by Gut Microbiota.

Xiao Cheng Qi (XCQ) decoction, an ancient Chinese herbal mixture, has been used in treating slow-transit constipation (STC) for years. The underlying action mechanism in relieving the clinical symptoms is unclear. Several lines of evidence point to a strong link between constipation and gut microbiota. Short-chain fatty acids (SCFAs) and microbial metabolites have been shown to affect 5-HT synthesis by activating the GPR43 receptor localized on intestinal enterochromaffin cells, since 5-HT receptors are known to influence colonic peristalsis. The objective of this study was to evaluate the eff

Slow transit constipation and lower urinary tract dysfunction.

INTRODUCTION: Many theories have been proposed for the coexistence of constipation and lower urinary tract dysfunction (LUTD), such as bladder compression from a distended rectum and stimulation of sacral reflexes from a full rectum. In these cases, successful treatment of constipation should result in resolution of bladder symptoms. Some children have refractory constipation and others respond well to treatment, but once treatment is discontinued most children relapse back into their constipation. This may indicate the existence of a defect in colon motility, with a persistent peristalsis pro

[Postpyloric feeding tubes for surgical intensive care patients. Pilot series to evaluate two methods for bedside placement].

Bedside placement of postpyloric feeding tubes in surgical intensive care patients: a pilot series to evaluate two methods. Early enteral feeding is thought to be a key factor in maintaining the integrity of the gastrointestinal tract mucosal barrier associated with less bacterial translocation and decreased stimulation of the systemic inflammatory response and subsequent improved outcome in intensive care patients. Thus enteral feeding by nasogastric tubes is the preferred route of nutritional support for most surgical intensive care patients. However, intensive care patients with delayed gas

Progress with novel pharmacological strategies for gastro-oesophageal reflux disease.

Gastro-oesophageal reflux disease (GORD) is a chronic disorder characterised by an increased exposure of the oesophagus to intragastric contents. Currently, GORD symptoms are maintained under control with antisecretory agents, mainly gastric proton pump inhibitors (PPIs). Although impaired oesophageal motility may partly underlie the pathophysiology of GORD, the use of prokinetic agents has been found to be unsatisfactory. To date, novel pharmacological approaches for GORD are mainly related to the control of transient lower oesophageal sphincter (LOS) relaxations (TLOSRs). The majority of pat

Pharmacology of 0-chlorobenzylidene malononitrile (CS).

1. The effects of o-chlorobenzylidene malononitrile (CS) have been studied on several isolated organs and tissues, anaesthetized animals and cat encéphale isolé preparations.2. On the isolated guinea-pig ileum an initial dose of CS produced a small, non-maintained contraction. Subsequent doses had reduced effects. There was no effect on peristalsis when the substance was given intraluminally.3. No significant effects of CS were detected on the rat phrenic nerve-diaphragm preparation, the isolated perfused rabbit heart or on the contractor response of the indirectly stimulated cat tibialis musc

An analysis of possible nervous mechanisms involved in the peristaltic reflex.

1. The effects of drugs on peristalsis and on the contractions of the two muscle coats of the isolated guinea-pig ileum in response to co-axial electrical stimulation have been studied.2. Co-axial stimulation (0.1 msec pulses) never produces simultaneous contraction of both muscle coats. When one muscle contracts, the other either relaxes or remains quiescent.3. The circular muscle contraction has two components. The first is reflex in origin and is brought about either by distension of the gut with increasing intraluminal filling or by the contraction of the longitudinal muscle in response to
