The immunological pathogenesis of IgG4-related disease categorized by clinical characteristics.

IgG4-related disease (IgG4-RD) is an immune disorder characterized by organ enlargement and fibrosis leading to functional impairment. Key i … dominant immune cell subsets involved in its pathogenesis. Specifically, the clinical phenotype of retroperitoneal fibrosis/aortitis is characterized by CX3CR1 + CTLs as the dominant key immune cell subset, while Mikulicz disease with systemic involvement is dominated by Tfh2. In addition to classification based on organ distribution, IgG4-RD can also be categorized into …

Renal denervation ameliorates the risk of ventricular fibrillation in overweight and heart failure.

AIMS: Both obesity and heart failure (HF) are associated with sudden cardiac death. The current study aimed to investigate the effects of overweight and HF on the substrate for ventricular fibrillation (VF), and whether renal denervation (RDN) can protect the heart from sympathetic activation and cardiac remodelling in HF rabbits fed with high-fat diet (HFD). METHODS AND RESULTS: Twenty-four rabbits randomized into control group fed with regular diet (Control), HFD, HFD-HF, and HFD-HF-RDN groups. Rapid ventricular pacing of 400 b.p.m. for 4 weeks was applied in HFD-HF and HFD-HF-RDN. Surgical

Blunt-tip coaxial introducer: a revisited tool for difficult CT-guided biopsy in the chest and abdomen.

OBJECTIVE: We describe a coaxial introducer provided with an additional blunt-tip stylet that allows safe access to difficult-to-reach lymph nodes in the chest, abdomen, and pelvis under CT control. CONCLUSION: Once the thoracic, abdominal, or pelvic wall is crossed by the introducer fitted with the sharp-tip stylet, the blunt-tip stylet replaces the sharp stylet for further progression in the fat toward the target. The soft-tip stylet carries a smaller risk of inadvertent perforation displacing vital structures.

IgG4-related periaortitis vs idiopathic periaortitis: is there a role for atherosclerotic plaque in the pathogenesis of IgG4-related periaortitis?

OBJECTIVE: The diagnosis of (isolated) IgG4-related periaortitis is often based on elevated serum IgG4 levels since in tissues such as the aorta, biopsies cannot be easily performed. However, the role for serum IgG4 as a biomarker for IgG4-related periaortitis is indistinct. The main purpose of our study was to identify clinical differences between periaortitis with elevated vs normal serum IgG4 levels. METHODS: A retrospective study was performed on 68 non-infectious periaortitis patients. We compared demographic, clinical, biochemical and radiological data in patients with elevated serum Ig

Non-aneurysmal inflammation of the aorta in combination with aortic valve disease.

A 61-year-old man presented with aortic valve stenosis, coronary artery disease in combination with rheumatic arthritis, peri-aortic inflammation and fibrosis with aortitis. In the pathological literature this has been termed chronic periaortitis, while in the surgical literature, non-aneurysmal dilatation of the aorta associated with atherosclerosis and inflammation has been described as aortitis. The name chronic periarteritis refers to the associated involvement of coronary arteries. Although different terminologies have been used, it is suggested that all of them most likely represent the

Erdheim-Chester disease versus multifocal fibrosis and Ormond's disease: a diagnostic dilemma.

AIM: The aim of the study was to evaluate the effectiveness of different imaging techniques with respect to diagnosis and differential diagnosis between Erdheim-Chester disease (ECD) and multifocal fibrosis (MF)/Ormond's disease (OD). METHOD: Three cases of ECD were included, two of which were misdiagnosed as MF/OD. Findings in different imaging techniques [plain radiography, skeletal scintigraphy, computed tomography (CT) and magnetic resonance imaging (MRI)] of the lower extremities, chest MRI, craniofacial MRI, abdominal CT and MRI) were compared and ranked with regard to diagnostic effica

Characteristics and prognosis of IgG4-related periaortitis/periarteritis: A systematic literature review.

OBJECTIVE: Immunoglobulin G4 (IgG4)-related disease is a systemic chronic fibroinflammatory disease that can affect almost every organ of the body. IgG4-related periaortitis/periarteritis is a newly recognized subset of IgG4-related disease, and its characteristics and prognosis remain unclear. We investigated the clinical characteristics and prognosis of IgG4-related periaortitis/periarteritis. METHODS: We performed a systematic literature review of IgG4-related periaortitis/periarteritis. Additionally, we have summarized the characteristics and prognosis of IgG4-related coronary arteritis.

[Periaortitis following endovascular aortic repair].

BACKGROUND: Infection is the most obvious cause of fever following implantation of an endovascular prosthesis; however, fever and inflammation around the arterial wall can also be caused by a sterile inflammatory reaction: periaortitis. CASE STUDY: An 81-year-old man was referred because of fever and back pain. He had undergone an endovascular aortic repair (EVAR) 1 year earlier; an endovascular prosthesis had been placed during this procedure, to repair an abdominal aortic aneurysm. A positive emission tomography (PET)-CT scan revealed uptake of 18F-fluorodeoxyglucose around the endovascular

Chronic periaortitis with antiphospholipid syndrome.

Chronic periaortitis (CP) is a rare disease, the common denominator of which is a retroperitoneal periaortic fibroinflammatory tissue that frequently obstructs neighboring structures. CP has already been reported in association with various autoimmune disorders, such as Hashimoto's thyroiditis, systemic lupus erythematosus and Sjögren's syndrome. The association with autoimmune disorders suggests that CP is a manifestation of a systemic autoimmune process. We describe the first case of antiphospholipid syndrome in a patient with CP.

[Magnetic resonance urography: possibilities and prospects].

A new technique of urinary tracts visualization--magnetoresonance urography (MRU)--for a year (2000) was applied in examination of 25 patients aged 17 to 63 years with ureteral concrements (n = 11), ureteropelvic stenosis (n = 10), ureteral stenosis (n = 2), urinary bladder tumor (n = 1), Ormond's disease (n = 1). MRU provides the same information about the obstruction and dilatation of the urinary tracts as excretory urography (EU). In cases of EU contraindication (allergy, renal failure) or lack of contrast substance excretion, MRU allows to avoid additional invasive diagnostic procedures. I

Unusual causes of mesenteric ischemia.

Although complications of generalized atherosclerosis most commonly cause intestinal ischemia, a number of unusual causes may be responsible. These unusual causes can be grouped into six major categories: (1) mechanical, (2) drugs, (3) hematologic, (4) endocrine, (5) vasculopathies, and (6) miscellaneous. Morbidity and mortality rates remain high because these rare diseases frequently go unrecognized until patients suffer adverse outcomes. A high index of suspicion may decrease the delay in diagnosis of mesenteric ischemia caused by these disorders.

Inflammatory aneurysms of the abdominal aorta: a five years experience.

During a five-year period, seven out of 85 patients undergoing surgery for abdominal aortic aneurysm, were identified as having the inflammatory variant. The clinical data, management and follow-up are reported. Computed tomography is the preferred examination for the disclosure of the nature of the aneurysm. Aneurysmectomy with graft replacement is advocated as the treatment of choice and requires a specific technique. Inflammatory abdominal aortic aneurysms are a clinical distinct entity but are considered as being a variant of the atherosclerotic abdominal aneurysm. The etiology has as yet

Urological complications in chronic inflammatory diseases of the bowel.

In advanced stages, the three most common inflammatory bowel diseases - colonic diverticulitis, Crohn's disease and ulcerative colitis - cause, in some 10% of cases, secondary urological pathology involving either bladder or ureter. Colovesical fistula is found more frequently in diverticulitis and less often in Crohn's disease, which penetrates predominantly from the ileum into the bladder. On the other hand, if uretic stenosis develops it will be caused on the right side by Crohn's disease and on the left by ulcerative colitis. Vesico-intestinal fistulae will close without sequelae after res

[Renal autotransplantation or iliac transposition of the kidney after section of its vascular pedicle. Apropos of 2 personal cases. Review of 100 published cases].

Two cases of auto-transplantation of the kidney are presented. The first was carried out without prior planning during a ureterolysis procedure for retro-peritoneal fibrosis with a single kidney, by virtue of per-operative thrombosis of the renal artery. The second was used in the treatment of hypertension due to bilateral atheromatous renal artery stenosis with a lesion at the main bifurcation of the artery. In both these cases in which the surgical indication was related to the artery, the ureter was left intact. After 2 and 1 years respectively, the results are good. The important points of
