Validation of the Clenched Fist View in Detecting Scapholunate Ligamentous Injury.

BACKGROUND: The purpose of this study was to determine the predictive value of the clenched fist stress views in identifying scapholunate ligament injuries. METHODS: An institutional review board-approved retrospective chart review was conducted of adult patients who underwent diagnostic arthroscopy with a ligamentous wrist injury from 2015 to 2020. Standard posteroanterior, lateral, and clenched fist stress radiographs were reviewed and scapholunate ligament gaps recorded. RESULTS: A total of 124 patients were included, of which 88 had normal standard radiographs and clenched fist radiograp

Current Techniques in Scapholunate Ligament Reconstruction.

Scapholunate ligament injuries are common and can lead to a predictable pattern of arthritis (scaphoid lunate advanced collapse wrist) if unrecognized or untreated. This article describes the relevant anatomy, biomechanics, and classification system, and provides an up-to-date literature-based review of treatment options, including acute repair and various reconstruction techniques. It also helps guide surgeons in making decisions regarding a systematic treatment algorithm for these injuries.

[Scaphoid fractures - Guidelines for diagnosis and treatment].

This article suggests algorithms for diagnosis and treatment of scaphoid fractures. A clinical suspected scaphoid fracture without signs of fracture on conventional radiographs should have a supplementary MRI done within 5-7 days. Displaced fractures and all proximal fractures should be classified by CT. Fracture union should be evaluated by CT. 90 procent of non- or minimally displaced waist fractures are healed after 6 weeks of conservative treatment. Non- or minimally displaced fractures with signs of instability can be treated conservatively, but require prolonged immobilisation. Fractures

The epidemiology of scaphoid fractures in Sweden: a nationwide registry study.

The epidemiology of scaphoid fractures has been described in limited populations, and incidence reports have been inconsistent. We investigated the nationwide incidence of scaphoid fractures by evaluating data on 34,377 patients in the Swedish National Patient Register for the years 2006-2015 regarding diagnosis, age, sex and treatment. The data were validated in 300 random patients, and incidence rates were adjusted accordingly. Forty-one per cent of the initially diagnosed fractures were false positives. The adjusted true fracture incidence rate was 22 per 100,000 person-years. During the de

Results of percutaneous fixation and distal radius core decompression in scaphoid waist non-unions treated without grafting.

UNLABELLED: Scaphoid non-union management is still a challenge in clinical practice for orthopaedic surgeons. Though several treatment methods have been described, there is an ongoing debate about optimum management. Based on new concepts about avascular conditions, promising results were reported with metaphyseal decompression of the distal radius by increasing the vascularization of the radial column of the carpus. We aimed to evaluate the clinical, radiological, and functional outcomes of distal radius core decompression and fixation with palmar percutaneous cannulated compression screws wi

Advanced imaging of the scapholunate ligamentous complex.

The scapholunate joint is one of the most involved in wrist injuries. Its stability depends on primary and secondary stabilisers forming together the scapholunate complex. This ligamentous complex is often evaluated by wrist arthroscopy. To avoid surgery as diagnostic procedure, optimization of MR imaging parameters as use of three-dimensional (3D) sequences with very thin slices and high spatial resolution, is needed to detect lesions of the intrinsic and extrinsic ligaments of the scapholunate complex. The paper reviews the literature on imaging of radial-sided carpal ligaments with advanced

Scapholunate interosseous ligament disruption in professional basketball players: treatment by direct repair and dorsal ligamentoplasty.

In the authors' experience scapholunate interosseous ligament (SLIL) disruption with resultant scapholunate dissociation (SLD) is the most frequent disabling carpal injury among professional basketball players. Prompt diagnosis, precision surgical repair, and intensive sport-specific rehabilitation are requisites for optimal recovery. This article reports the techniques and results of a consistent surgical protocol comprising accurate carpal reduction, direct SLIL repair, and dorsal intercarpal ligament augmentation for 25 professional basketball players with disabling SLD. Interobserver reliability of computed tomography to diagnose scaphoid waist fracture union.

PURPOSE: To determine the interobserver agreement and diagnostic performance characteristics of computed tomography (CT) for determining union of scaphoid waist fractures. METHODS: A total of 59 orthopedic and trauma surgeons rated for union a set of 30 sagittal CT scans of 30 scaphoid waist fractures. Of these fractures, 20 were treated nonoperatively, were imaged between 6 and 10 weeks after injury, and were known to have eventually achieved union. Ten were operatively confirmed to be ununited. We rated each scan as united or ununited using a Web-based rating application. We assessed intero

The incidence of scaphoid fracture in a military population.

BACKGROUND: The rate of occurrence of scaphoid injury is not well known. The incidence of scaphoid fracture has been described mostly in small, injured cohort populations, which may underestimate its frequency. We studied the epidemiology of the scaphoid fracture using a large database in a military population. PATIENTS AND METHODS: The Defense Medical Epidemiology Database (DMED), a comprehensive database which tracks medical care for all four military services of the United States, was queried for the first occurrence of scaphoid fractures using International Classification of Diseases (ICD

[Arthroscopically controlled minimal invasive screw fixation of scaphoid fractures. A pilot study].

We could not find any systematic studies about the value of arthroscopy in scaphoid fractures in the literature, and therefore undertook a pilot study of 20 scaphoid fractures which were treated arthroscopically by the Herbert-Whipple screw and were minimally invasive. We found very good radiological and clinical results in 17 patients and good results in 3 patients evaluated by the Wozasek and Moser score. For reduction of dislocated fractures we used distraction with 5 kg in the wine waiters position. In four cases we used additional K wires as joysticks. Arthroscopy is less valuable in redu

Kinematic tracking of the small bones of the wrist in sequential 3DCT and dynamic 4DCT volume images using open-source Hierarchical 3D Registration, a module within SlicerAutoscoperM.

Quantitative kinematic tracking of in vivo skeletal structures efficiently and with high temporal resolution is important to a wide range of questions in biomechanical research. As such, software tools are needed for efficient, semi-automated registration of bones imaged with either 3DCT or 4DCT. This study presents Hierarchical 3D Registration (3DH), an open-source approach to skeletal tracking of three-dimensional image volume sets. The 3DH approach and software are presented using sequential 3DCT datasets of participants performing various thumb tasks and dynamic 4DCT datasets of simulated

Prevalence of the Os Supranaviculare: A Systematic Review with Meta-Analysis.

Background/Objectives: The os supranaviculare (OSSN), also known as os talonaviculare dorsale, astragalo-scaphoid ossicle, or Pirie's bone, is a small extra bone that was first described in 1921 by A.H. Pirie and is located at the top front part of the navicular bone or talonavicular joint. The knowledge regarding the epidemiology of the OSSN is scarcely established, as its prevalence remains unknown and varies significantly among multiple studies. This meta-analysis aims to clarify and systematically summarize all available data on the characteristics and prevalence of the OSSN. Methods: Four

Non-traumatic scapholunate dissociation in a 10-year-old female: A case report.

INTRODUCTION: Severe or minor repetitive trauma, inflammation, infection, tumors, and congenital ligamentous laxity have been etiologically implicated in scapholunate dissociation (SLD). While a few cases of patients with asymptomatic SLD have been reported in the literature, despite radiographically demonstrated widened scapholunate angles and rotatory subluxation of the scaphoid bone, these patients experienced only mild or no pain and no dorsal intercalated segment instability deformity. Here, we report the case of a monolateral non-traumatic SLD in a young 10-year-old girl that led to an i…

Magnetic Resonance Imaging-Detected Features of Inflammation and Erosions in Symptom-Free Persons From the General Population.

OBJECTIVE: The use of magnetic resonance imaging (MRI)-detected inflammation and joint damage in the diagnosis of rheumatoid arthritis is recommended by a European League Against Rheumatism imaging task force. This recommendation is based on the sensitivity of MRI and not on specificity. Knowledge of the prevalence of MRI-detected features in symptom-free persons, however, is pivotal when considering MRI for diagnostic purposes. METHODS: From November 2013 to December 2014, 196 symptom-free persons of different ages were recruited from the general population. Inclusion criteria were no histor
