[Role of surgical treatment in thoracic empyema with bronchopleural and/or thoracic empyema].

Sixty-nine patients with thoracic empyema treated surgically were experienced from May, 1978 through December, 1990. Thirty-nine cases had bronchopleural and/or thoracic fistula. Thirty-two patients were associated with pulmonary tuberculosis, of whom fourteen had tuberculous empyema and eighteen were sequelae of pulmonary tuberculosis or tuberculous pleurisy. The remainder were postoperative, postpneumonic, and posttraumatic empyemas. Of fourteen patients who developed postoperative bronchopleural fistula, there were ten patients who had lobectomy or pneumonectomy for lung cancers. Omental pe

Fibrinolytic and coagulation mechanisms in stages of inflammation: a study of BCG-induced pleural exudate in guinea pig.

Pleural fluid from an early, active phase of BCG-induced pleurisy was compared with fluid from late, healing phase, characterized by fibrinous adhesions. Exudates were tested for proteolytic activity on chromogenic peptide substrates designed for plasmin, tissue plasminogen activator, factor Xa, thrombin and plasma kallikrein. Considerable activity of active-phase pleural fluid was found on all of these substrates, and significantly lower values in the healing phase. Most exudates from both stages had very low fibrinogen concentration. Fibrinopeptide A, fibrinolytic products and antiplasmin we

Pericardial calcification in childhood.

Calcific constrictive pericarditis (CCP) in a three-year-old child with symptoms of cardiac compression was confirmed by cardiac catheterization and angiography. Histologic examination of the pericardial tissue removed at operation revealed a tuberculous etiology. Though unusual in the pediatric age group, constrictive pericarditis (CP) may occur in children, most often as a complication of tuberculosis. Pericardial calcification may also develop in children with CP, though this too is rare. The diagnosis of CCP can be established by cardiac catheterization and angiography. Pericardiectomy is

Recurrent pneumoperitoneum with pneumatosis cystoides intestinalis in tuberculous pleurisy: Case report.

RATIONALE: Pneumatosis cystoides intestinalis (PCI) is a rare condition for which autopsy studies have reported a prevalence of 0.03% in the general population. Although PCI can be associated with pneumoperitoneum, many cases are benign and do not reflect viscus perforation. Coexistence with tuberculous pleurisy and recurrent pneumoperitoneum is rare and may be underrecognized. PATIENT CONCERNS: In this report, we present the case of a 38-year-old Tibetan male who experienced tuberculous pleurisy accompanied by recurrent pneumoperitoneum with PCI. Despite the presence of free air, he remained

EFFECTIVENESS OF EARLY VIDEO-ASSISTED MINI-ACCESS SURGERY IN TREATMENT OF COMPLICATED FORMS OF TUBERCULOUS PLEURISY.

The efficacy and safety of mini-thoracotomy with video tracking in the treatment of complicated forms of tuberculous pleurisy was studied. The study included 78 patients who were treated in the department of extrapulmonary surgery of the Aktobe Regional TB Dispensary in the period from 2016 to 2019. A mini-thoracotomy with video-assistant, and a control (41 patients) group - standard thoracotomy was performed in patients of the main (37 patients) group. Early surgical interventions according to the VATS method in the treatment of complicated forms of tuberculous pleurisy reduced the patient's

Utility of adenosine deaminase (ADA), PCR & thoracoscopy in differentiating tuberculous & non-tuberculous pleural effusion complicating chronic kidney disease.

BACKGROUND & OBJECTIVES: Pleural effusion is a common occurrence in patients with late-stage chronic kidney disease (CKD). In developing countries, many effusions remain undiagnosed after pleural fluid analysis (PFA) and patients are empirically treated with antitubercular therapy. The aim of this study was to evaluate the role of adenosine deaminase (ADA), nucleic acid amplification tests (NAAT) and medical thoracoscopy in distinguishing tubercular and non-tubercular aetiologies in exudative pleural effusions complicating CKD. METHODS: Consecutive stage 4 and 5 CKD patients with pleural effu

Tuberculous oesophagopleural fistula.

Hydropneumothorax complicating to oesophagopleural and broncho-oesophageal fistula is an unusual presentation and is reported in literature as separate clinical entities. Here we present a case of hydropneumothorax with both oesophagopleural and broncho-oesophageal fistula of tuberculous aetiology occurring simultaneously in the same patient. Oesophagopleural fistula was suspected clinically by colour of pleural fluid. Subsequent barium swallow demonstrated leak of contrast material from oesophagus into pleural cavity. Later on, computed tomography of chest with oral contrast confirmed communi

Neutralization of TNFalpha alters inflammation in guinea pig tuberculous pleuritis.

Previously, treatment with anti-gpTNFalpha antibody enhanced TNFalpha mRNA expression in pulmonary granulomas microdissected from non-vaccinated guinea pigs, and modified splenic granuloma architecture. In this study, pleural fluid, cells, and granulomatous tissues were collected 3, 5, and 8 days post-pleurisy induction in guinea pigs treated with anti-gpTNFalpha or normal serum control. Neutralizing TNFalpha reduced the percentage of macrophages in the pleural exudate while increasing the proportions of neutrophils and lymphocytes. Cell-associated mycobacterial loads were increased in guinea

Diagnostic value of pleural fluid adenosine deaminase activity in tuberculous pleurisy.

BACKGROUND: Diagnosis of tuberculous pleuritis is difficult because of its nonspecific clinical presentation and insufficient efficiency of traditional diagnostic methods. We investigated the use of adenosine deaminase (ADA) activity in tuberculous pleuritis diagnosis. METHODS: We optimized a kinetic assay and retrospectively analysed 210 patients with exudative pleural effusions. Using the ROC curve, we determined the optimal cutoff for TB pleurisy. RESULTS: One hundred forty-seven exudative samples were nontuberculous (non-TB) and 63 were tuberculous (TB). There was statistically significa

Tuberculous pleural effusion: experience with six months of treatment with isoniazid and rifampicin.

BACKGROUND: Tuberculous pleurisy is associated with small numbers of bacteria. Due to the low rate of primary resistance to antituberculous drugs a two-drug regimen was used to treat the condition. METHODS: Patients received isoniazid 5 mg/kg and rifampicin 10 mg/kg daily for six months. Clinical, radiological, and haematological assessments were performed during treatment and patients were followed up for a median period of 41 (range 6-96) months. RESULTS: One hundred and thirty patients were studied with a mean age of 27 (range 11-53) years. Seven were withdrawn due to parenchymal disease

Lipoprotein analysis in a chyliform pleural effusion: implications for pathogenesis and diagnosis.

A chyliform effusion is an uncommon high lipid pleural effusion that does not result from a leakage of the thoracic duct. Characteristically, it emerges from chronic pleurisy and contains high levels of cholesterol. The origin of this cholesterol is unknown, but it is often attributed to the degeneration of red and white blood cells. In this study we have carried out detailed lipoprotein analyses in a chyliform effusion, a chronic tuberculous effusion and three inflammatory effusions of recent onset, in an attempt to elucidate the process of cholesterol accumulation and possible lipoprotein al

[Respiratory manifestations in aspergillosis].

Aspergillus is a genus of cosmopolitan fungi with a selective pulmonary tropism. Their pathogenic role is due either to spreading in pre-existing pulmonary cavities, or to their allergizing capacity. Cavitary sequellae of tuberculosis and suppuration, particularly frequent and important in tropical environment, are elective localization for Aspergillus colonization. Surgical treatment is nowadays the only efficient one. Allergic manifestations are a more complex problem of therapy, exclusion of allergen being difficult to get in tropical environment.
