Inner ear decompression sickness following altitude chamber operation.

Decompression sickness (DCS) is a known hazard of altitude chamber operation. The musculoskeletal, dermal, neurological and pulmonary manifestations of DCS are well recognized, but inner ear injury has not been reported. We present the unusual case of a medical corpsman suffering from vestibular DCS after an altitude chamber exposure to 25,000 ft. The patient had a good clinical response to hyperbaric treatment, but there was laboratory evidence of mild residual vestibular damage with full compensation. This case suggests that aviation medical personnel should be more aware of the possible occ

Click-evoked vestibular activation in the Tullio phenomenon.

Click-evoked vestibulocollic reflexes were studied in a patient with a unilateral Tullio phenomenon (sound induced vestibular symptoms) and the findings were compared with those of a group of normal subjects. Compared with normal subjects, the reflexes elicited from her symptomatic side were large and had an abnormally low threshold, but retained a normal waveform. The click-evoked responses in this patient show changes consistent with her symptomatology and are indicative of a pathological increase in the normal vestibular sensitivity to sound.

Vestibular function and sensory interaction in space flight.

The vestibular system and vestibulo-visual interaction were examined in 11 astronauts by the electrooculographic (EOG) method during short- and long-term flights on days 2, 3, 5, 9, 22, 50, 164, and 169 (experiments OPTOKINES and LABYRINTH). In space (flight days 2 and 3), they showed enhanced spontaneous vertical nystagmus, and disorders of tracking of vertical and diagonal movements of the stimulus which improved after active head movements. Early increasing of the reactivity of the cupulo-endolymphatic system (flight days 2-3) was replaced after 5 days of flight with a reduction of the vest

Familial progressive vestibulocochlear dysfunction.

A kindred is presented in which several members had complaints of head movement-dependent oscillopsia due to acquired vestibular areflexia in combination with progressive hearing loss. History was noncontributory for other neurologic or otologic diseases (including infectious diseases) or use of neuro-ototoxic drugs. The pedigree suggests autosomal dominant inheritance.

Menière's disease in children.

Fourteen children (aged 14 years or younger) with typical Ménière's triad with cochlear sensorineural hearing loss, tinnitus, and intermittent vertigo attacks lasting from minutes to hours were investigated in four different neuro-otologic centers. Nine children, labeled as having "idiopathic Ménière's disease," developed the auditory and vestibular symptoms without any detectable causative factor. Five children, labeled as suffering from "secondary Ménière's syndrome," had histories of an initial hearing loss following mumps, hemophilus influenza meningitis, temporal bone fracture, or congeni

Flunarizine treatment in dizzy patients with vestibular hyperreactivity and hyperventilation.

Flunarizine (10 mg/day) was given in an open pilot study to 14 patients with the primary complaint of dizziness, who showed vestibular hyperreactivity in the velocity step (VS) test and were proven to have a hyperventilation syndrome (HVS) without any other disease. The VS test was repeated after 4-6 weeks of treatment. Twelve of these HVS patients reported alleviation of dizziness. Objective evaluation of the results of treatment was attempted by defining suitable response parameters and comparing their values to those obtained in a control group of 14 healthy volunteers tested twice with the

Evaluation of audiovestibular status in leprosy.

One hundred twenty five cases of lepromatous leprosy and 25 cases of tuberculoid leprosy were investigated for audiovestibular status. Impaired hearing was detected in 52 percent and vestibular hypofuction in 7.2 per cent of lepromatous cases. Conductive deafness was due to eustachian tube catarrh secondary to atrophic rhinitis associated with the disease. The perceptive deafness and vestibular hypofuction were due to end organ lesion probably due to E.N.L. reaction. The vestibulococlear nerve involvement was considered to be unlikely. In tuberculoid leprosy derangement in hearing was not obse

Postural vertigo and positional nystagmus.

Oscillation of the eyes of a patient when the head is placed in a certain position is objective evidence to support a complaint of postural vertigo-dizziness when the head is tilted forward or upward or turned to one side or the other. Since positional nystagmus may be difficult to evoke and may be elicited at one time and not at another, it is important to make repeated tests, lest a causative lesion be overlooked. Vertigo in such cases may be caused by pathologic change in the eighth peripheral nerve or in the central vestibular pathways. Sometimes no organic disease is observable even thoug

Recognizing Decompensated Meniere's Disease Using High Frequency Rotational Test.

OBJECTIVES: The purpose of this study is to investigate the effectiveness of the high-frequency rotational test for discrimination of patients with decompensated from those with compensated Meniere's disease. MATERIALS AND METHODS: Patients with unilateral Meniere's disease were divided into two groups (compensated and decompensated), based on the presence of clinically significant positional nystagmus as a determinant of the compensation status. All patients and subjects underwent pure tone audiometry, video nystagmography, and the vestibular autorotation test (VAT). The gain, phase, and asy

Mapping affected territory of anterior/posterior inferior cerebellar artery infarction using a vestibular test battery.

CONCLUSION: Although the affected territory in the posterior/anterior inferior cerebellar artery (PICA/AICA) infarction could not be meticulously demonstrated by magnetic resonance imaging (MRI), it could be picked up by the results of a vestibular test battery comprising caloric, ocular vestibular evoked myogenic potential (oVEMP), and cervical VEMP (cVEMP) tests. OBJECTIVES: This study applied audiometry and caloric, oVEMP, and cVEMP tests to map affected territory in patients with PICA/AICA infarction. METHODS: Fourteen patients, including 11 with PICA infarction and 3 with AICA infarctio

Lateral semicircular canal plugging in severe Ménière's disease: a clinical prospective study about 28 patients.

OBJECTIVE: To evaluate the safety and efficiency of the plugging of the lateral semicircular canal to control vertigo in severe Ménière's disease (MD). STUDY DESIGN: Prospective follow-up study. SETTING: University hospital. PATIENTS: Twenty-eight patients with MD with refractory vertigo and severe disability (functional scale 5 or 6). INTERVENTIONS: Lateral canal plugging was performed in pathological ear for each patient. MAIN OUTCOME MEASURES: The evaluation of therapy followed the guidelines for diagnosis and evaluation of therapy in MD (1995). Hearing, frequency of vertigo, and funct

Subjective visual vertical in acute attacks of Ménière's disease.

OBJECTIVES: To investigate whether or not and how often there is otolith dysfunction in an acute attack of Ménière's disease. PATIENTS: Twenty-two definite cases of unilateral Ménière's disease diagnosed in accordance with the 1995 criteria of the American Academy of Otolaryngology-Head and Neck Surgery for Ménière's disease. INTERVENTION: Subjective visual vertical (SVV) test was performed before, at, and after acute attacks on 22 patients with unilateral Ménière's disease who showed normal tilts of SVV before acute attacks. MAIN OUTCOME MEASURE: Abnormal tilts of SVV. RESULTS: Of 22 case

Changes in caloric nystagmus induced by vasodilating drugs.

Systemic blood flow, blood flow to the peripheral skin, middle ear, and temporal bone, and that to the inner ear of normal volunteers was manipulated selectively through the use of vasodilator drugs. The effect of selective vasodilation was monitored via changes in the temporal course of the slow-phase velocity of nystagmus evoked by bithermal caloric irrigation. Drug influenced eye velocities were compared with control values measured before drug administration. Eye velocities measured after niacin (a peripheral vasodilator) administration generally were greater than control values, whereas e
