Percutaneous implantation of cerebral microdialysis catheters by twist-drill craniostomy in neurocritical patients: description of the technique and results of a feasibility study in 97 patients.

Cerebral microdialysis is increasingly used to monitor several types of neurocritical patients. This study presents the technique used in our unit for percutaneous implantation of cerebral microdialysis catheters using a small twist-drill craniostomy that can be performed in the intensive care unit (ICU). We also present the results of this technique in 89 head-injured patients and in eight patients with a malignant middle cerebral artery (MCA) infarction. One hundred and twenty-two cerebral microdialysis catheters were implanted in the 97 patients included in this study. One cerebral microdia

Spread of malignant lymphoid cells into rat central nervous system with intact and disrupted blood-brain barrier.

The pathways of spread of malignant lymphoid cells into the central nervous system (CNS) were studied using a T lymphoblastic leukaemia/lymphoma model of inbred PVG rats. The effects of intraperitoneal, intracarotid, intravenous, intrathecal and intracerebral routes of transplantation were analysed, and the significance of the blood-brain barrier (BBB) in preventing neoplastic cell invasion was studied by disrupting the BBB with focal cold injury. Extraneurally transplanted cells appeared first in the dura and subarachnoid space. From the latter they spread further into the perivascular space

The NERD model: reflex circuit dysfunction as a systems-level driver of persistent post-concussion symptoms.

Persistent post-concussive symptoms are often attributed to diffuse cortical dysfunction, yet this perspective may overlook key systems-level mechanisms. We propose a conceptual framework in which dysfunction arises from the dynamic interplay among five functional nodes: the sensory interface, reflex-brain stem hub, cerebellar module, basal ganglia-thalamic modulator, and cerebral cortex. Grounded in clinical observation and systems-level dynamic modeling, this framework treats the brain as a time-evolving control network that processes inputs, integrates them across hierarchical nodes, and ge

Beyond the scan: quick MRI's emergence in pediatric emergency care.

PURPOSE OF REVIEW: Computed tomography (CT) remains the dominant neuroimaging modality in the pediatric emergency department despite growing awareness of the risks associated with cumulative ionizing radiation exposure. Advances in rapid MRI have renewed interest in quick brain MRI (qbMRI) as a feasible, radiation-free alternative. This manuscript explores emerging strategies to reduce unnecessary CT utilization, expand MRI accessibility, and synthesize growing evidence supporting the use of qbMRI across a range of pediatric emergency indications. RECENT FINDINGS: Recent literature demonstrat

Combination Therapy of Estrogen and Progesterone Attenuates Behavioral Impairments and Autophagy via circLrp1b/miR-27a-3p/Dram2 Pathway After Traumatic Brain Injury.

In the present investigation, the influences of progesterone (P4) and 17β-estradiol (estradiol or E2), alone and in combination, on spatial and non-spatial cognition and anxiety-like behaviors were assessed following traumatic brain injury (TBI). We also evaluated the impacts of these sex hormones on the miR-27a-3p and CircLrp1b gene expression and proteins involved in the autophagy pathway in the hippocampus of rats. Male rats were randomly divided into 12 groups (six groups to measure brain water content and six groups for molecular and behavioral studies): sham, TBI, TBI/vehicle, TBI/P4, TB

Impact of Root Venting on Brain Injury in Minithoracotomy Aortic Valve Replacement: Prospective Evaluation With Preoperative and Postoperative Brain MRI in 252 Patients.

BACKGROUND: The predominant method for preventing cerebral infarction secondary to air embolism in cardiac surgery is the use of root vents to remove air. However, in the case of minimally invasive aortic valve replacement (MIAVR) surgery, the risk of air embolism is lower compared with other types of minimally invasive cardiac surgery, and some surgeons routinely forgo root venting. This study sought to quantify how much difference root venting makes in stroke and asymptomatic brain injury (ABI) incidence in MIAVR. METHODS: The study enrolled patients undergoing elective MIAVR from January 2

Propofol anesthesia improves stroke outcomes over isoflurane anesthesia-a longitudinal multiparametric MRI study in a rodent model of transient middle cerebral artery occlusion.

General anesthesia is routinely used in endovascular thrombectomy procedures, for which volatile gas and/or intravenous propofol are recommended. Emerging evidence suggests propofol may have superior effects on disability and/or mortality rates, but a mode-of-action underlying these class-specific effects remains unknown. Here, a moderate isoflurane or propofol dosage on experimental stroke outcomes was retrospectively compared using serial multiparametric MRI and behavioral testing. Adult male rats (N = 26) were subjected to 90-min filament-induced transient middle cerebral artery occlusion.

Correlation between early computed tomography findings and neurological outcome in pediatric traumatic brain injury patients.

Traumatic brain injury (TBI) is a leading cause of morbidity and mortality in children. Head computed tomography (CT) is frequently utilized for evaluating trauma-related characteristics, selecting treatment options, and monitoring complications in the early stages. This study assessed the relationship between cranial CT findings and early and late neurological outcomes in pediatric TBI patients admitted to the pediatric intensive care unit (PICU). The study included children aged 1 month to 18 years who were admitted to the PICU due to TBI between 2014 and 2020. Sociodemographic data, clinica

Interhemispheric Transcingulate Sulcus Approach to Deep-Seated Medial Frontal and Parietal Lesions-Fiber Dissection Study With Illustrative Cases.

BACKGROUND: Surgery for lesions located in the medial frontal and parietal lobes can be quite challenging for neurosurgeons because of morbidities that may arise from damage to critical midline structures or intact neural tissue that need to be crossed to reach the lesion. In our anatomic studies, the cingulate sulcus was observed as an alternative access route for lesions located in medial frontal and parietal lobes. OBJECTIVE: To explain the microsurgical anatomy of the medial hemisphere and cingulate sulcus and to demonstrate the interhemispheric transcingulate sulcus approach (ITCSA) with

Altered white matter microstructural organization in posttraumatic stress disorder across 3047 adults: results from the PGC-ENIGMA PTSD consortium.

A growing number of studies have examined alterations in white matter organization in people with posttraumatic stress disorder (PTSD) using diffusion MRI (dMRI), but the results have been mixed which may be partially due to relatively small sample sizes among studies. Altered structural connectivity may be both a neurobiological vulnerability for, and a result of, PTSD. In an effort to find reliable effects, we present a multi-cohort analysis of dMRI metrics across 3047 individuals from 28 cohorts currently participating in the PGC-ENIGMA PTSD working group (a joint partnership between the Ps

Magnetic resonance susceptibility weighted imaging in neurosurgery: current applications and future perspectives.

Susceptibility weighted imaging (SWI) is a relatively new imaging technique. Its high sensitivity to hemorrhagic components and ability to depict microvasculature by means of susceptibility effects within the veins allow for the accurate detection, grading, and monitoring of brain tumors. This imaging modality can also detect changes in blood flow to monitor stroke recovery and reveal specific subtypes of vascular malformations. In addition, small punctate lesions can be demonstrated with SWI, suggesting diffuse axonal injury, and the location of these lesions can help predict neurological out

Differences in brain edema and intracranial pressure following traumatic brain injury across the estrous cycle: involvement of female sex steroid hormones.

It has been shown that sex steroid hormones have profound neuroprotective effects in experimental traumatic brain injury (TBI). Because the endogenous hormone levels are proven to differ with estrous cycle stage, we evaluated whether estrous cycle stage affects various outcomes following diffuse TBI. TBI was induced by Marmarou's method in normal cycling and in ovariectomized rats with physiologically relevant restoration of hormonal levels by hormone capsule implantation. Intracranial pressure (ICP) and cerebral perfusion pressure (CPP) were measured before and different times after TBI and b

Stroke patterns in neonatal group B streptococcal meningitis.

Neonatal group B streptococcus meningitis causes neurologic morbidity and mortality. Cerebrovascular involvement is a common, poorly studied, and potentially modifiable pathologic process. We hypothesized that imaging patterns of focal brain infarction are recognizable in neonatal group B streptococcal meningitis. A consecutive case series included term neonates with the following: (1) bacterial meningitis, (2) acute group B streptococcal infection (positive cerebrospinal fluid/blood culture), (3) brain magnetic resonance imaging within 14 days, and (4) acute intraparenchymal focal infarctions

Kinetics of endothelin-induced inhibition and glucose permeability of astrocyte gap junctions.

Gap junctions contribute to important functions of communicating glial cells in brain physiology and pathology. Endothelins (ETs), a vasoactive family of peptides present in the brain, have been described as potent inhibitors of astrocyte gap junctional communication. Through dye-coupling studies we demonstrate here that this inhibition occurs rapidly and then successively reverses and returns to control levels after 90 min of continuous ET1 or ET3 exposure. In addition, long-term exposure of cells to ET3, which acts mainly on ETB receptors, also desensitized the acute action of ET1, which was

Neurosonographic abnormalities in neonates with hypoxic ischemic encephalopathy.

Pattern of neurosonographic (NSG) abnormalities in 150 term newborn infants with hypoxic ischemic encephalopathy (HIE) was studied. Sonographic abnormalities, presumably indicating cerebral edema and or ischemia, were observed in 86% (n = 129) cases. Obliteration of the ventricles occurred as the sole abnormality in 30 (20%) cases. Eighty (53%) patients had diffusely increased echogenicity of the brain parenchyma (DPE) in addition to the compression of the ventricles, sulci and the interhemispheric fissure. Focal parenchymal echodense (FPE) lesions occurred in nine (6%) neonates with HIE. Ten

Cortical control of inhibition of return: evidence from patients with inferior parietal damage and visual neglect.

Inhibition of return (IOR) refers to slower reaction times to targets presented at previously stimulated or inspected locations. This phenomenon biases orienting towards novel locations and is functional to an effective exploration of the environment. Patients with right brain damage and left visual neglect explore their environment asymmetrically, with strong difficulties to orient attention to left-sided objects. We show for the first time a dissociation between manual and saccadic IOR in neglect. Our patients demonstrated facilitation, instead of inhibition, for repeated right-sided targets

Assessment of extremely premature lambs supported by the Extrauterine Environment for Neonatal Development (EXTEND).

BACKGROUND: Our team has previously reported physiologic support by the EXTra-uterine Environment for Neonatal Development (EXTEND) of 105 to 117 days gestational age (GA) lambs for up to 28 days with normal organ maturation. However, the fetal lamb brain matures more rapidly, requiring the study of 90-105 day GA fetal lambs to assess more neurodevelopmentally equivalent lambs to the 23-25 week GA extreme premature infant. METHODS: Extremely preterm lambs (90-95 days of GA) were delivered by C-section and supported by EXTEND. Estimated circuit flows were maintained at around 325 ml/kg/min. Af

[A contribution to infection with yellow fever virus 17D in chick embryos (author's transl)].

During the production of yellow fever virus 17 D vaccine from chick embryos, few embryos die in the time between injection of the eggs and harvest of the embryos. In order to answer the question whether the death of the embryos is due to the infection or to the injury caused by the injection, it is necessary to examine the embryos macro- and microscopically for pathological changes. 8 and 9 days old chick embryos were inoculated into the amniotic cavity with different concentrations of yellow fever virus 17 D (table 1). The embryos were removed from the eggs between the 2nd and 7th day after i

Traumatic Brain Injury in Patients With Mandibular Fractures.

BACKGROUND: Traumatic brain injury (TBI) associated with facial fractures is a major public health concern worldwide. The rate of TBI in patients with mandibular fractures ranges from 21.3% to 39.6%. However, the risk factors for TBI in patients with mandibular fractures remain unknown. Our study evaluates these risk factors. METHODS: We retrospectively reviewed patients who presented with traumatic mandibular fractures in 2018 and 2019. Excluded were patients with no documentation of Glasgow Coma Scale. Our primary outcomes were: (1) prevalence of concomitant TBI on presentation defined as h
