Epidemiology of pediatric abuse- and neglect-associated emergency department visits in the United States: an analysis of the 2019-2022 National Hospital Ambulatory Medical Care Survey.

CONTEXT: Child maltreatment impacts an estimated 7.4 per 1,000 children in the United States. Given the severity of some abusive injuries, children may present to emergency departments (EDs). As such, EDs are primary screening locations for early identification of maltreatment. OBJECTIVES: Our objective was to assess rates of maltreatment among pediatric patients presenting to the ED. METHODS: We performed a cross-sectional analysis of pediatric ED visits utilizing data from the Center for Disease Control's (CDC) National Hospital Ambulatory Medical Care Survey (NHAMCS, 2019-2022). Child mal

Prehospital ECG with ST-depression and T-wave inversion are associated with new onset heart failure in individuals transported by ambulance for suspected acute coronary syndrome.

BACKGROUND: Prehospital electrocardiogram(s) (ECG) can improve early detection of acute coronary syndrome (ST-segment elevation myocardial infarction [STEMI], non-STEMI, and unstable angina) and inform prehospital activation of cardiac catheterization lab; thus, reducing total ischemic time and improving patient outcomes. Less is known, however, about the association of prehospital ECG ischemic findings and long term adverse clinical events. With this in mind, this study was designed to examine the: 1) frequency of prehospital ECGs for acute myocardial ischemia (ST-elevation, ST-depression, an

An ancestral 10-bp repeat expansion in VWA1 causes recessive hereditary motor neuropathy.

The extracellular matrix comprises a network of macromolecules such as collagens, proteoglycans and glycoproteins. VWA1 (von Willebrand factor A domain containing 1) encodes a component of the extracellular matrix that interacts with perlecan/collagen VI, appears to be involved in stabilizing extracellular matrix structures, and demonstrates high expression levels in tibial nerve. Vwa1-deficient mice manifest with abnormal peripheral nerve structure/function; however, VWA1 variants have not previously been associated with human disease. By interrogating the genome sequences of 74 180 individua

Selection of an Appropriate Tool for Assessing Geriatric Frailty in Clinical Practice: A Narrative Review.

BACKGROUND: Frailty is a clinically significant geriatric syndrome characterized by reduced physiological reserve and increased vulnerability to stressors, resulting in a higher risk of adverse health outcomes such as functional decline, hospitalization, institutionalization, and mortality. Its early identification is essential for risk stratification, care planning, and individualized intervention. As no universal biomarker of frailty exists, its assessment relies on standardized clinical assessment instruments. However, the available tools are not interchangeable, and their optimal use depen

Prospective Five-Point Serial Assessment of Radiological and Functional Outcome Dissociation After Locking Plate Fixation of Proximal Tibial Fractures.

INTRODUCTION: Orthopedic surgeons have traditionally assessed the success of tibial plateau fixation by evaluating radiological restoration of articular congruity, as observed on the initial postoperative X-ray. However, patient-centered outcomes such as pain, ambulation, knee stability, and return to work progress more slowly and in an unpredictable manner. This study investigated whether optimal radiological results on postoperative day 1 correspond to comparable functional recovery at six months, utilizing five serial assessments of both radiological and patient-focused functional outcomes

Handgrip Strength and Dehydroepiandrosterone Sulfate in a Frailty Unit: A Retrospective Study.

INTRODUCTION: Frailty is characterized by vulnerability and decline in physical, mental, and social activity, significantly contributing to adverse health outcomes. Frailty encompasses nutritional status, muscle strength, inflammation, and hormones. Dehydroepiandrosterone sulfate (DHEAS) is one of the hormones hypothesized to play a role in frailty. Handgrip strength (HGS) correlates with overall muscle strength. The fatigue, resistance, ambulation, illnesses, and loss of weight (FRAIL) scale can be used to readily screen frailty. Identifying markers associated with frailty can facilitate its
