BACKGROUND: Amoebic colitis is the most frequent clinical manifestation of invasive intestinal infection due to Entamoeba histolytica and a common cause of diarrhoea worldwide. Since higher transmission rates are usually related to poor health and exposure to unhygienic conditions, cases reported in Europe usually involve immigrants and international travellers. We describe a case of a young returning traveller who contracted amoebic dysentery while visiting India. She presented to a major London Hospital several months later with features suggestive of amoebic liver abscesses, a known sequelae of amoebiasis. MRI with intravenous contrast demonstrated an area of likely occlusion of the portal vein. The clinical and microscopic appearances of inflammatory bowel disease may be very similar to those of amebic colitis. The coexistence of invasive amebiasis with inflammatory bowel disease may have disastrous results. Patients with inflammatory bowel disease have a greater prevalence of amebiasis, but this association is more significant for ulcerative colitis. There have been very few reports in the literature presenting the superimposition of amebiasis on Crohn's disease. In this report, a rare case of Crohn's colitis with superimposed amebiasis resulting in colonic perforation is presented.

One of the main drawbacks of experimental amebiasis is the lack of an adequate animal model for invasive intestinal lesions. Mongolian gerbils are useful because both intestinal and hepatic amebiasis can be produced experimentally with Entamoeba histolytica trophozoites. A 28-year-old stewardess was admitted to hospital with violent cramp-like pain in the right upper abdomen, which was accompanied by a single episode of watery pasty diarrhea. Over the next few days her symptoms persisted, she developed a high fever, and her general state of health deteriorated. Ultrasonography repeated on the fourth day then revealed a small space-consuming lesion in the right lobe of the liver. While the laboratory investigations initially remained negative, the suspected amebic abscess and colitis were subsequently also confirmed serologically.

The enzymatic activity of inorganic pyrophosphate (PPi) dependent phosphofructokinase became manifest in the supernatant obtained by centrifugation in a homogenate of E. histolytica strain HMI-IMSS at 700,000 g.
