Factors predicting complication rates after primary shoulder arthroplasty.

HYPOTHESIS: Shoulder arthroplasty is an effective treatment for arthritic conditions and intraarticular fractures of the proximal humerus. Treatment options include total and hemiarthroplasty of the shoulder. They hypothesis of this study was that a mandatory statewide discharge database could identify the epidemiology of primary shoulder arthroplasty, 90 day complication rates, implant survival rates, and patient and hospital characteristics associated with complications. MATERIALS AND METHODS: We identified patients undergoing primary total shoulder replacement and hemiarthroplasty between

Hill-sachs lesions in shoulders with traumatic anterior instability: evaluation using computed tomography with 3-dimensional reconstruction.

BACKGROUND: In patients with traumatic anterior shoulder instability, a large Hill-Sachs lesion is a risk factor for postoperative recurrenc … the mean length, width, and depth of the lesions (calculated as a percentage of the diameter of the humeral head) were 9.0%, 5.3%, and 2.1%, respectively, while the corresponding numbers for primary dislocation were 73.3%, 27.7%, 14.8%, and 7.0%, all showing statistically significant differences. Among all 142 shoulders, the corresponding numbers were, respect …

CONCLUSION: Both strategies, including bone grafting with conventional implants and the use of custom implants for structural glenoid defects, achieved MCIDs and meaningful improvements in patient function and pain. The Evolving Role of Artificial Intelligence in Fracture Diagnosis and Surgical Planning in Orthopaedics: Current Insights and Future Directions.

Orthopaedic fracture diagnosis and surgical planning are being revolutionised by artificial intelligence (AI) and machine learning (ML). These technologies enable the automated detection and classification of fractures and provide surgeons with AI-assisted pre-operative decision-making support at multiple body sites. The objective of this review is to synthesise current evidence (from 2016 to 2026) related to the application of AI/ML technologies in orthopaedic fracture diagnosis and surgical planning, to summarise the primary clinical findings from these studies, and to describe the challenge

Converting to reverse shoulder arthroplasty from primary anatomic shoulder arthroplasty and fracture hemiarthroplasty: a radiographic and clinical outcome analysis at 8-years.

INTRODUCTION: Revision shoulder arthroplasty can be challenging. One of the main considerations for surgeons is the type of implant that was placed in the initial surgery. Anatomic shoulder arthroplasty (ASA) is used for cases of osteoarthritis as well as for fractures of the humeral head. Hemiarthroplasty can be used for complex proximal humerus fractures. The purpose of this study is to determine whether there is a difference in clinical and radiographic outcomes between patients that failed primary fracture hemiarthroplasty (FHA), or ASA for osteoarthritis and then required reoperation with…

It is usually the result of a … additional fractures of the tuberosities are rare and severe injuries requiring open surgery as the humeral head may remain locked prohibiting closed reduction. Even after the reduction, the stable fixation of the tuberosities is crucial for shoulder stability and postoperative clinical and functional results.

Pathological unstable dislocation of shoulder secondary to septic arthritis: A case report.

INTRODUCTION: Pathological unstable dislocation of the glenohumeral joint following septic arthritis is a rare condition. We report a case o … ulder. He also complained of severe pain over that region. X-ray showed anteromedial dislocation of humeral head. Aspiration of joint was done and the aspirate was sent for culture and sensitivity, which revealed Staphylococcus aureus. Incision and drainage was performed, about 500 ml of straw-colored pus mixed with synovial fluid extending up to sternum, post …

Critical Glenoid Bone Loss in Posterior Shoulder Instability.

BACKGROUND: There is currently no consensus regarding the amount of posterior glenoid bone loss that is considered critical. Critical bone l … force was applied to the glenohumeral joint, and the peak force that was required to translate the humeral head posteriorly and the lateral displacement that occurred with translation were measured. The defect size was measured as a percentage of the glenoid width. Testing was performed in 11 conditions: (1) intact glenoid and labrum, (2) simulated reverse Ba …

Isolated coronoid fracture: Assessment by magnetic resonance imaging for concomitant injuries.

BACKGROUND: Ligamentous injury associated with isolated coronoid fracture had been sparingly reported. Concealed or unclear fractures and ligamentous or articular cartilage lesions are promptly acknowledged by magnetic resonance imaging (MRI) but cannot be entirely pictured in regular radiological assessments. In isolated coronoid fracture, the fragment size is very small and due to the complex anatomy surrounding the coronoid radiographic imaging may not be sufficient. The purpose of this study was to evaluate the incidence of combined osteochondral and ligamentous injuries by magnetic resona

Scapulothoracic and Glenohumeral Kinematics During Daily Tasks in Users of Manual Wheelchairs.

BACKGROUND: Rates of shoulder pain in individuals who use manual wheelchairs (MWCs) as their primary means of mobility have been reported to be as high as 70% during activities of daily living. Current prevailing thought is that mechanical impingement of the soft tissues that reside within the subacromial space between the humeral head and coracoacromial arch is a major contributor to the shoulder pain in users of MWCs. The subacromial space size is directly related to the kinematics at the shoulder joint. Yet to be answered are questions about which common daily tasks are characterized by the…

Outcomes after salvage procedures for the painful dislocated hip in cerebral palsy.

BACKGROUND: The painful dislocated hip in the setting of cerebral palsy is a challenging problem. Many surgical procedures have been reported to treat this condition with varying success rates. The purpose of this study is to retrospectively evaluate and compare the outcomes of 3 different surgical procedures performed at our institution for pain relief in patients with spastic quadriplegic cerebral palsy and painful dislocated hips. METHODS: A retrospective chart review of the surgical procedures performed by 5 surgeons for spastic, painful dislocated hips from 1997 to 2010 was performed. Th

A calibration phantom for direct, in vivo measurement of 241Am in the axillary lymph nodes.

A calibration phantom was developed at the University of Cincinnati (UC) to determine detection efficiency and estimate the quantity of activity deposited in the axillary lymph nodes of a worker who had unknowingly sustained a wound contaminated with 241Am at some distant time in the past. This paper describes how the Livermore Torso Phantom was modified for calibrating direct, in vivo measurements of 241Am deposited in the axillary lymph nodes. Modifications involved milling a pair of parallel, flat bottom, cylindrical holes into the left and right shoulders (below the humeral head) of the Li…

Pullout strength of suture anchors in comparison with transosseous sutures for rotator cuff repair.

Suture anchors are increasingly gaining importance in rotator cuff surgery. This means they will be gradually replacing transosseous sutures. The purpose of this study was to compare the stability of transosseous sutures with different suture anchors with regard to their pullout strength depending on bone density. By means of bone densitometry (CT scans), two groups of human humeral head specimens were determined: a healthy and a osteopenic bone group. Following anchor systems were being tested: SPIRALOK 5.0 mm (resorbable, DePuy Mitek), Super Revo 5 mm (titanium, Linvatec), UltraSorb (resorba…

Comparison of outcomes of 2 surgical treatments for proximal humerus giant cell tumors: a multicenter retrospective study.

BACKGROUND: The incidence of giant cell tumors in the proximal humerus is low. We evaluated 2 surgical treatments for giant cell tumors of the proximal humerus and postoperative upper-extremity function. METHODS: This study retrospectively analyzed the clinical data of 27 cases of giant cell tumors of the proximal humerus at 4 Chinese medical centers specializing in bone oncology collected between January 2002 and June 2015.
