Hydrocephalus following toxoplasmosis.

Between 1966-1983 ten children with hydrocephalus following toxoplasmosis were shunted. Hydrocephalus was diagnosed at ages ranging from birth to one year. Except for one with stenosis of the foramen of Monro all had aqueductal obstruction. A high ventricular fluid pressure and distinct cerebral mantle reduction were always present. Intracerebral calcifications were common. Only 1 had normal eyes. All 9 survivors remained shunt dependent during a mean follow-up period of 9.6 years. One fifth of revisions were related to problems specific for toxoplasma-associated hydrocephalus. A temporary ext

Nonlinear poroplastic model of ventricular dilation in hydrocephalus.

OBJECT: The mechanism of ventricular dilation in normal-pressure hydrocephalus remains unclear. Numerical finite-element (FE) models of hydrocephalus have been developed to investigate the biomechanics of ventricular enlargement. However, previous linear poroelastic models have failed to reproduce the relatively larger dilation of the horns of the lateral ventricles. In this paper the authors instead elaborated on a nonlinear poroplastic FE model of the brain parenchyma and studied the influence of the introduction of these potentially more realistic mechanical behaviors on the prediction of t

Reversible intracranial hypertension following treatment of an extracranial vascular malformation: case report.

Pediatric hydrocephalus is a well-studied and still incompletely understood entity. One of the physiological means by which hydrocephalus and intracranial hypertension evolve is through perturbations to normal vascular dynamics. Here the authors report a unique case of an extracranial vascular anomaly resulting in persistently elevated intracranial pressures (ICPs) independent of CSF diversion in a patient with a Joubert syndrome-related disorder. The patient developed worsening intracranial hypertension after successful CSF diversion of Dandy-Walker malformation-associated hydrocephalus via e

Disproportionately Enlarged Subarachnoid Space Hydrocephalus (DESH) Identified on I-123 Ioflupane SPECT/CT.

A patient with gait instability underwent I-123-ioflupane SPECT/CT imaging for suspected Parkinson's disease. DaTQUANT analysis was negative for Parkinson's disease; however, evaluation of the SPECT images revealed low counts within the bilateral parietal region. Further review of the CT images demonstrated the classic appearance of disproportionately enlarged subarachnoid space hydrocephalus (DESH), a supportive imaging feature of idiopathic normal pressure hydrocephalus (iNPH), highlighting the importance of correlation of cross-sectional CT and DaT SPECT imaging.

Dementia of different etiologies: vasoactive intestinal polypeptide in CSF.

Patients with normal-pressure hydrocephalus, multi-infarct dementia, and recent cerebral infarction had significantly lower vasoactive intestinal polypeptide (VIP) levels than age-matched controls (11 +/- 3 pmol/l, 17 +/- 4 pmol/l, 21 +/- 4 pmol/l, and 33 +/- 4 pmol/l, respectively). Three months after a shunt operation, the VIP levels had increased significantly in patients with hydrocephalus (54 +/- 13 pmol/l). VIP concentration in patients with senile dementia did not differ from that of controls.

The Dilemma of Hydrocephalus in Prolonged Disorders of Consciousness.

Prolonged disorders of consciousness (DOC) are considered to be among the most severe outcomes after acquired brain injury. Medical care for these patients is mainly focused on minimizing complications, given that treatment options for patients with unresponsive wakefulness or minimal consciousness remain scarce. The complication rate in patients with DOC is high, both in the acute hospital setting, as in the rehabilitation or long-term care phase. Hydrocephalus is one of these well-known complications and usually develops quickly after acute changes in cerebrospinal fluid (CSF) circulation af

Rapid deterioration of primary fourth ventricular outlet obstruction resulting in syndrome of inappropriate antidiuretic hormone secretion.

Fourth ventricular outlet obstruction (FVOO) is a rare cause of obstructive hydrocephalus. Although FVOO accompanied by malformative syndrome and secondary causes of obstruction are common, there are few reports of primary FVOO (PFVOO). The syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a rare presenting feature of hydrocephalus. A 1-year, 8-month-old boy with a normal head circumference developed SIADH accompanied by rapid deterioration of symptoms of intracranial hypertension. PFVOO was diagnosed because magnetic resonance imaging revealed an enlarged ventricular system

Index of cerebrospinal compensatory reserve in hydrocephalus.

OBJECTIVE: An index of cerebrospinal compensatory reserve (RAP) has been introduced as a potential descriptor of neurological deterioration after head trauma. It is numerically computed as a linear correlation coefficient between the mean intracranial pressure and the pulse amplitude of the pressure waveform. We explore how RAP varies with different forms of physiological or nonphysiological intracranial volume loads in adult hydrocephalus, with and without a functioning cerebrospinal fluid (CSF) shunt. METHODS: A database of intracranial pressure recordings during CSF infusion studies and ov

A case report of mental disorder caused by shunt blockage after hydrocephalus surgery.

INTRODUCTION: Hydrocephalus is a form of communication hydrocephalus syndrome characterized by ventricular enlargement and normal intraventricular pressure. The primary clinical manifestations include gait disturbances, cognitive dysfunction, urinary incontinence, and either elevated or normal intracranial pressure. This paper presents a case of a mental disorder resulting from inadequate drainage following a ventriculoperitoneal shunt procedure for hydrocephalus. The case report aims to enhance clinicians' understanding of such organic brain lesions, which are prone to misdiagnosis and inappr

Extreme sensitivity of hearing to decreases of ICP in Menière's disease.

We report the case of a Menière's disease patient affected by normal pressure hydrocephalus (NPH) who presented a cerebrospinal fluid (CSF) pressure-dependent hearing impairment after shunting. This side-effect was not only reversible and reproducible but occurred at a high opening pressure when the valve setting was lowered by only 0.7 mmHg (10 mmH(2)O). This observation suggests that hearing in Menière's disease might be very sensitive to small reductions of intracranial pressure (ICP) and that these patients should be informed of this potential risk, which can compromise the efficacy of the

Bicompartmental analysis of cerebrospinal fluid circulation. Theory and clinical applications.

A new model for cerebrospinal fluid (CSF) circulation is proposed. Specific activity/time curves for CSF kinetics determined after intraventricular injection of a radiotracer were produced by fitting a biexponential function to data points and developing a two-compartmental model. Calculation of kinetic parameters of the model provides quantitative data about CSF dynamics. The study of each compartment separately and of the intercompartmental relationship is possible with this model. Sequential scan images and graphic plots of the variations of radioactivity in both compartments, derived from

Acetazolamide in hydrocephalus management: Mechanistic insights and clinical challenges.

Hydrocephalus is a heterogeneous neurological disorder characterized by impaired cerebrospinal fluid (CSF) dynamics, resulting in ventriculomegaly, neurological dysfunction, and substantial morbidity across pediatric and adult populations. Surgical CSF diversion remains the standard treatment but risks infection, shunt malfunction, and lifelong dependency, underscoring the need for potential nonsurgical adjuncts. Acetazolamide, a carbonic anhydrase inhibitor, has been investigated as a pharmacological approach to reduce CSF production, lower intracranial pressure, and modulate cerebral blood f

[Chronic subdural hematoma as a complication of ventriculoperitoneal shunts].

Nine cases of chronic subdural hematoma occurring after the insertion of ventriculo-peritoneal shunts are described. Three patients were children, two were adults with stenosis of the Sylvian aqueduct, and the last four had normal pressure hydrocephalus. Patients with chronic hydrocephalus were initially treated with burr holes associated to transient occlusion of the distal catheter of the diversion in order to promote reexpansion of the compressed hemisphere. Two shunt-dependent patients were successfully treated with a concurrent subdural-peritoneal shunt. In two cases a higher pressure shu

Hydrocephalus resulting from superior vena cava thrombosis in an infant. Case report.

The authors report an unusual case of superior vena cava thrombosis in an infant who subsequently developed communicating hydrocephalus; they also review previously reported cases of dural sinus hypertension, and separate them into two groups. Patients in the first group develop hydrocephalus and those in the second develop a pseudotumor-like syndrome. The former patients tend to have generalized increase in intracranial venous pressure while the latter have a normal pressure in some major intracranial venous structure(s). The absence of venous cushioning of the choroid plexus pulse wave is pr

Comparison of Shunt Outcomes for Nonbacterial Infection Hydrocephalus with Common Hydrocephalus Etiologies: A Retrospective Case-Control Study.

BACKGROUND: Shunting is an established treatment for hydrocephalus, yet reports on shunt outcomes for nonbacterial infection (NBI) hydrocephalus are limited. Furthermore, comparison of mechanisms and rates of failure for shunted NBI hydrocephalus versus more typical etiologies remains undetermined. METHODS: Patients who underwent shunting for hydrocephalus at 2 centers (1995-2020) were included. Indications for shunting were grouped as "typical" (congenital, posthemorrhagic, normal pressure hydrocephalus, malignancy-related, trauma, and idiopathic) and NBI hydrocephalus (coccidioidomycosis, c

[A device to measure CSF flow in a shunt tube and its clinical application].

An implantable device for measurement of cerebrospinal fluid (CSF) flow in a shunt tube has been developed. The unit is energized by an extracorporeal high frequency generator (200 kHz), and electrolysis creates bubbles in the shunt tube. Bubble flow velocity is detected as reflected sound using a pair of ultrasonic Doppler probes (Saneisokkuki Doppler Flowmeter Type 1935) placed apart on the skin surface and in parallel with the tube. CSF flow is expressed in ml/min. by calculating velocity and tube diameter. The unit consists of a coil with a 200 kHz capacitor, a silicon diode to rectify the

[Gamma cysternographic data in hydrocephalus and post-traumatic lesions].

With pneumoencephalography and pressure perfusions tests, the gamma-cisternography is a method of choice for establishing a reliable diagnosis of Normal Pressure Hydrocephalus (NPH) which concept was introduced by HAKIM. The gammacisternography allows to establish the diagnosis of NPH when a persistent ventricular filling is present and to find, very often, the etiological factor by showing the absence of radioactivity over the subarachnoïd pathways along the convexities at any stage or delayed and abnormal CSF absorption. The gammacisternography permits also to explore with 111 In DTPA the hi

Assessment of neurological symptoms in adult hydrocephalus occlusus. A pilot study.

BACKGROUND: Comprehensively describe and compare (pre⁄postoperatively) the clinical symptomatology in adult non-communicated hydrocephalus. Associated hydrocephalus signs were analyzed with the idiopathic Normal Pressure Hydrocephalus Scale (iNPH Scale). A standardized clinical scale for non-communicated hydrocephalus is currently not in use. METHODS: Ten patients with hydrocephalus occlusus (HO) were analyzed. Hydrocephalus signs were examined with the iNPH Scale in gait, neuropsychology, continence, and balance before and three months after treatment with shunt operation or third endoscopic
