The percutaneous lateral retinacular release.

This study reviews 34 of 37 original patients who were reviewed in 1981 after previously undergoing percutaneous lateral retinacular release. Short-term satisfactory results regarding reduction of pain and prevention of dislocation were maintained on this second followup (average, 48 months). With this increase in postoperative time, patients with recurrent subluxation developed an increase in patellar slipping. Patients with chondromalacia and degenerative joint disease are less suited for long-term treatment. Long-term problems occurred in 1 of 34 patients (3%).

The quadriceps function in patellofemoral disorders. A radiographic and electromyographic study.

The influence of quadriceps contraction on patellar alignment was investigated in tangential views of 31 patients with patellofemoral complaints and compared with 25 controls. The congruence angle decreased significantly following quadriceps contraction in patients with patellar subluxation in contrast to those with chondromalacia and normals. Electromyography (EMG) of the vastus lateralis (VL) and vastus medialis obliquus (VMO) during maximal quadriceps contraction was obtained prior to and after 3 months of isometric quadriceps exercises in 15 knees with patellar subluxation and in 11 with i

Splicing Alterations Associated with Multiple Homozygous TNXB Variants in a Patient with Suspected Classical-Like Ehlers-Danlos Syndrome.

Ehlers-Danlos syndrome (EDS) comprises a heterogeneous group of heritable connective tissue disorders characterized by joint hypermobility, skin hyperextensibility, and tissue fragility. Classical-like EDS (clEDS) is typically associated with pathogenic biallelic variants in TNXB. The patient showed generalized joint hypermobility involving the knees, fingers, elbows, and feet, along with soft, pliable skin. She had a history of three dislocations of the right elbow and one dislocation of the left elbow. She also reported orthostatic dizziness and chronic constipation, suggesting possible auto

Open Reduction and Internal Fixation of Acute Lisfranc Fracture-Dislocation with Use of Dorsal Bridging Plates.

Lisfranc injuries consist of a wide spectrum of injuries, ranging from subtle injuries to severe fracture-dislocations. Injuries with instability of the tarsometatarsal, intercuneiform, or naviculocuneiform joints should be treated with anatomic reduction and stable fixation. The best method of fixation is debated. Transarticular screw fixation has the disadvantage of damaging the tarsometatarsal joints. Bridging the tarsometatarsal joints with use of low-profile locking plates avoids the placement of screws through the joint and potentially reduces the risk of posttraumatic arthritis. Primary

Dural ectasia in a child with Larsen syndrome.

INTRODUCTION: We present a case of an incidental finding of dural ectasia in a child diagnosed with Larsen syndrome. Larsen syndrome is a rare inherited disorder of connective tissue characterized by facial dysmorphism, congenital joint dislocations of the hips, knees and elbows, and deformities of the hands and feet. Dural ectasia is as an abnormal expansion of the dural sac surrounding the spinal cord and may result in spinal morphologic changes, instability, and spontaneous dislocation. To the best of our knowledge, the presence of dural ectasia in Larsen syndrome has not previously been re

Influence of rugby injuries on players' subsequent health and lifestyle: beginning a long term follow up.

OBJECTIVES: To describe the current rugby playing status of a cohort of 1,169 men who had previously participated in an epidemiological survey of rugby injuries during the 1993-1994 season, and assess the consequences of rugby injuries sustained. METHODS: In May 1998, 911 (78%) men completed a questionnaire reporting their current involvement in rugby and the influence that the 324 (71%) injuries they had sustained four years earlier had since had on their health and wellbeing. RESULTS: The most common reasons given by the 390 (43%) ex-players for ceasing to play rugby were family (10%), emp

Does degree of trochlear dysplasia and position of femoral tunnel influence outcome after medial patellofemoral ligament reconstruction?

BACKGROUND: The medial patellofemoral ligament (MPFL) is the main restraining force against lateral patellar displacement. It is disrupted after patellar subluxation or dislocation. Reconstruction of the MPFL is frequently performed when nonoperative management fails and the patient experiences recurrent patellar dislocation. PURPOSE: To determine the relationship between the degree of trochlear dysplasia and femoral tunnel position and outcome after MPFL reconstruction. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 68 patients (72 knees) with recurrent dislocation of

Equivalence of Outcomes Between Ambulatory and Hospital-Based Arthroplasty in Younger and Older Adults.

BACKGROUND: Total joint arthroplasty (TJA) has increasingly shifted toward ambulatory surgery centers (ASCs), yet data supporting the safety of this setting for older patients remain limited. This study compared 90-day perioperative outcomes of TJA performed at ASCs versus inpatient hospital settings, stratified by patients younger than 75 and those 75 years or older. METHODS: We conducted a retrospective cohort study of 10,804 patients undergoing elective primary total hip or knee arthroplasty between 2021 and 2023 at a single academic health system. Patients were stratified by surgical sett

Evaluating the Safety of Prescription Sleep Medications in Patients Undergoing Total Knee Arthroplasty: A Propensity Matched Analysis.

BACKGROUND: Impaired sleep is reported in over 50% of patients after a total knee arthroplasty (TKA). Clinicians often treat postoperative sleep disturbances with medications; however, the risks associated with the use of sleep medications after TKA are not well understood. We assessed the association between sleep medication use after TKA and postoperative complications. METHODS: Using a national administrative claims database, we identified 101,806 patients undergoing primary TKA from 2011 to 2022 who were taking sleep medications within 90 days after surgery and 509,030 patients who were n

A Comparison of 1.5-Stage and Two-Stage Revisions for Prosthetic Joint Infection in Total Hip and Knee Arthroplasty: A Meta-Analysis of Outcomes.

Prosthetic joint infection (PJI) is a devastating complication of total hip and knee arthroplasty. Whilst two-stage revision has long been considered the gold standard treatment, the 1.5-stage revision has emerged as a viable alternative. However, the optimal surgical strategy remains a subject of debate. The aim of this meta-analysis was to evaluate the comparative outcomes of 1.5-stage versus two-stage revision for PJI. A systematic search of electronic data sources and bibliographic reference lists was conducted. All studies reporting comparative outcomes of 1.5-stage versus two-stage revis

Early institutional adoption and outcomes from 154 consecutive medial mobile-bearing unicompartmental knee arthroplasties: a single-center experience after implant introduction.

INTRODUCTION: Medial unicompartmental knee arthroplasty (UKA) has become an established alternative to total knee arthroplasty (TKA) for isolated medial compartment osteoarthritis, offering faster recovery and better functional outcomes. However, results during the early phase of institutional adoption may vary depending on surgeon experience and procedural standardization. This study aimed to evaluate early clinical outcomes, complication rates, revision profiles, and the learning curve associated with the initial institutional adoption of medial mobile-bearing UKA in a high-volume, multi-sur

Morphological study of the vastus medialis oblique in recurrent patellar dislocation based on magnetic resonance images.

BACKGROUND: To investigate the morphological parameters of the vastus medialis obliquus (VMO) muscle and delineate its importance in the maintenance of patellofemoral joint stability. METHODS: The magnetic resonance imaging data of seventy-five knees (fifty-four patients) with recurrent lateral patella dislocation (LPD) and seventy-five knees (seventy patients) without recurrent LPD were retrospectively analysed. Five morphological parameters related to the VMO (elevation in the sagittal plane and coronal plane, craniocaudal extent, muscle-fibre angulation, cross-sectional area ratio) and two

New standardization method of tibial tubercle-posterior cruciate ligament distance according to patient size in patients with patellofemoral instability.

BACKGROUND: Large differences in tibial tubercle-posterior cruciate ligament (TT-PCL) distance were described in several reports between countries, suggesting that abnormal TT-PCL distance is variable due to individual patient size. This study aimed to clarify the relationship between TT-PCL distance and patient size, and to determine a method for describing individualized TT lateralization. METHODS: We analyzed 41 patients with recurrent patellar dislocation (RPD) and 41 age-matched patients without patellar instability who underwent primary anterior cruciate ligament reconstruction (control

[The use of the sequential mathematical analysis for the determination of the driver's seat position inside the car passenger compartment from the injuries to the extremities in the case of a traffic accident].

The objective of the present study was to determine the diagnostic coefficients (DC) for the injuries to the upper and lower extremities of the vehicle drivers inflicted inside the passenger compartment in the case of a traffic accident. We have analysed the archival expert documents collected from 45 regional bureaus of forensic medical expertise during the period from 1995 to 2014 that contained the results of examination of 200 corpses and 300 survivors who had suffered injuries in the traffic accidents. The statistical and mathematical treatment of these materials with the use of sequentia

Comparison of clinical outcomes between mobile- and fixed-bearing unicompartmental knee arthroplasty for medial compartment osteoarthritis: a retrospective study.

BACKGROUND: Unicompartmental knee arthroplasty (UKA) is an established treatment for isolated medial compartment osteoarthritis (MCOA). The optimal choice between mobile-bearing (MB) and fixed-bearing (FB) designs remains debated, particularly in Asian populations where treatment costs and insurance policies influence surgical decisions. OBJECTIVE: To compare short-term clinical and radiographic outcomes between MB and FB UKA and to evaluate potential implications for healthcare resource allocation. METHODS: Sixty-four patients who underwent medial UKA between January 2022 and October 2024 w
