Treatment of a case of serious occlusal instability associated with TMDs through a modern mini-invasive approach.

BACKGROUND: The latest trends in restorative dentistry are guided both by esthetic and mini-invasive procedures. New ceramic materials, such as lithium disilicate, allow the clinician to obtain invisible restorations with a minimum preparation thickness. SUMMARY: A young female patient presented at our practice with left temporomandibular closed locking and uncomfortable occlusal instability. After manual reduction, medical therapy, and reversible treatment with a stabilization splint, a mini-invasive lithium disilicate occlusal inferior posterior rehabilitation was conceived and performed. Cone-beam computed tomography evaluation of periodontal and bone support loss in extraction cases.

BACKGROUND: The aim of this study was to evaluate, in particular, whether bone resorption occurred at the extraction sites of a group of patients under orthodontic treatment, and, in general, whether extraction treatment predisposes patients to a greater degree of root resorption. METHODS: The study group comprised 12 class II division 1 malocclusion patients who underwent orthodontic treatment and extraction, and the control group comprised 10 class II division 1 patients who underwent orthodontic treatment without extraction. In both groups, treatments were carried out by the same operator

Management of occlusal canting with miniscrews.

A 14-year-old boy with a skeletal Class II malocclusion and open bite whose chief complaint was a posterior crossbite showed a canted occlusal plane with asymmetric gummy smile and mandibular deviation at clinical examination. The treatment with miniscrews focused on the bilateral intrusion of the maxillary posterior teeth and, after resolving the open bite, a new biomechanical technique involving joined miniscrews was applied for an en masse intrusion of the left side. This treatment strategy achieved optimal occlusion with improvements to the sagittal, vertical, and transverse relationships

[Optimizing orthodontic treatment in patients taking bisphosphonates].

Bisphosphonates have unique pharmacological characteristics unlike those of any other drug group. Millions of adults take oral bisphosphonates for long-term treatment of osteoporosis and osteopenia; some of these people will most likely also seek orthodontic treatment. Adverse dental effects from bisphosphonates have been reported, including decreased tooth movement, impaired bone healing, and osteonecrosis in the mandible and the maxilla. Osteonecrosis has been rarely observed after bisphosphonate use for osteoporosis. However, adverse drug effects might occur more frequently in orthodontic p

Early headgear effects on the eruption pattern of the maxillary canines.

OBJECTIVE: To test the null hypothesis that early headgear (HG) treatment has no effect on the eruption pattern of the maxillary canines in the early mixed dentition. MATERIALS AND METHODS: Sixty-eight children (40 boys and 28 girls) with a Class II tendency in occlusion and moderate crowding of the dental arches were randomized into two groups. HG treatment was initiated immediately in the first group. In the second group only minor interceptive procedures were performed during the first follow-up period of 2 years. Orthopantomograms were taken at the baseline, three times at 1-year interval

Biodegradable polylactide plates and screws in orthognathic surgery: technical note.

In orthognathic surgery, the bone fragments are usually fixed with metallic plates and screws. Metallic devices other than titanium plates are usually removed after the osteotomy has consolidated, which often requires general anaesthesia. Titanium plates, supposed to be biotolerable, have been introduced in order to overcome this need for secondary intervention. However, due to corrosion, titanium particles have been found in scar tissue covering these plates and in locoregional lymph nodes. Therefore, their removal is also advocated. Self-reinforced poly (L-lactide) homopolymer (PLLA) and pol

Three-Dimensional Change of Molar Position During Alignment and Leveling With PASS and MBT: A Randomized Controlled Trial.

BACKGROUNDS: With the development of straight wire appliance (SWA), especially the rountine use of nickel-titanium (NiTi) archwires in the initial stage has largely eliminated the need for bending up. However, engaging fine NiTi wire into the molar buccal tube with built-in forward tipping prescription can induce unintended mesial tipping and mesialization of maxillary molars, leading to early anchorage loss. This single-center, two-arm parallel randomized clinical trial aimed to compare the physiologic anchorage spee-wire system (PASS) with the Mclaughlin-Bennet-Trevici straight-wire system (

Craniofacial growth in children with nasal septum deviation: a cephalometric comparative study.

OBJECTIVE: Nasal-breathing impairment has been described as a possible determinant of maxillofacial development in children with adenoids/tonsils hypertrophy. However little is known about the possible influence of nasal septum deviation on craniofacial growth in childhood. We conducted a multicenter cephalometric study to compare skeletal and dental features in children with chronic nasal-breathing obstruction secondary to nasal septum deviation and nose-breathing controls. METHODS: Ninety-eight children (59M, 39F; mean age 8.8 years; age range 7-12 years) with obligate mouth-breathing secon

The preoperative condition of the temporomandibular joint and role of disc perforation in postoperative condylar resorption in Class II orthognathic surgery patients.

This study was performed to investigate whether preoperative disc perforation is associated with postoperative condylar resorption (assessed as the 1-year change in mandibular ramus height) in skeletal Class II patients, and to analyse the preoperative temporomandibular joint (TMJ) morphology and synovial biomarkers in Class II and III patients. Orthognathic surgery patients treated between 2020 and 2022, who had undergone preoperative arthroscopy, MRI, CT, and synovial fluid analysis, were included. Three groups of patients were investigated: Class II with disc perforation (Class II/Pf(+); ni

Orthodontic skeletal anchorage using a palatal external plate.

We have developed the Anchor-Lock external plate system, which is fitted on the palate for the purpose of orthodontic skeletal anchorage. The aim of this study was to introduce the Anchor-Lock and assess its success rate. The Anchor-Lock is composed of titanium screws of 2·0-mm diameter and a titanium plate of 1·0-mm thickness. The external plate is rigidly interlocked with the heads of the screws, which are implanted trans-mucosally into palatal bone. Three types of Anchor-Lock are available. These were applied to 137 orthodontic patients (104 females and 33 males) aged 10-54 years. Two types

The interdisciplinary management of a Class II division 1 malocclusion with an associated mandibular defect following surgical excision of fibromatosis: a clinical report.

Surgical excision of malignant or nonmalignant tumors of the maxillofacial region can result in significant anatomical loss/compromise. Correction of the residual defect can be complicated by the presence of an underlying malocclusion and can be a significant clinical challenge if large numbers of teeth have been lost in surgery. This clinical report discusses the interdisciplinary treatment to rehabilitate a patient with a history of surgical removal of fibromatosis. Treatment involved the use of osseointegrated implants to facilitate orthodontic correction of an underlying malocclusion. The
