Evaluation of a new real-time blood continuous glucose monitoring system in pregnant women without gestational diabetes. A pilot study.

The incidence of gestational diabetes mellitus (GDM) has reached epidemic proportions in the United States, affecting 200,000 pregnant women annually. Controversies exist regarding whether the glucose levels used to diagnose GDM in pregnancy should be lowered since similar fetal complications are seen in women with GDM as well as in nondiabetic women. Although GDM has been extensively studied, the effects of the earlier stage of glucose intolerance are not well understood. The objectives of this descriptive, comparative, correlational pilot study include assessment of real-time blood glucose c

Posthospitalization breastfeeding patterns of moderately preterm infants.

BACKGROUND: Little is known about how preterm infants make the transition from breast-feeding and bottle feeding to exclusive breast-feeding in the weeks following hospital discharge. This study examined the breastfeeding patterns of preterm infants born at 30 to 35 weeks' gestation over a 4-week period following hospitalization. METHOD: Daily feeding diaries were completed by 53 mothers. These diaries were used to describe the proportion of breast milk feeds and feeds directly at breast. RESULTS: Infants received a high proportion of breast milk feeds, with 60% receiving breast milk exclusi

Alloimmune thrombocytopenia in the fetus: current management theories.

Fetal alloimmune thrombocytopenia (AIT) affects only a small number of fetuses but the consequences may be devastating. Outcomes range from mild thrombocytopenia without signs or symptoms to intracranial hemorrhage (ICH) and fetal or neonatal death. Antenatal screening is available but not used routinely in the United States. The most frequent method of AIT identification is its diagnosis in the neonatal period (index neonate). AIT may be diagnosed antenatally if an ICH is noted on routine obstetric ultrasound. There are accurate predictors of AIT, as well as antenatal therapies, to prevent th

Hypertension in neonates causes and treatments.

Neonatal hypertension, defined as systolic blood pressure greater than 95th percentile for age, size, and gender, is an uncommon but significant problem. Hypertension may develop secondary to thromboembolic complications of central lines, steroid use, or be a sign of an underlying renal or cardiac problem. Pharmacologic management of hypertension in the neonate includes use of beta blockers, angiotensin-converting enzyme inhibitors, calcium channel blockers, and/or diuretics. This article discusses issues in assessment and management of neonatal hypertension and reviews the pharmacologic agent

Neonatal transitional physiology: a new paradigm.

Early clamping of the umbilical cord at birth, a practice developed without adequate evidence, causes neonatal blood volume to vary 25% to 40%. Such a massive change occurs at no other time in one's life without serious consequences, even death. Early cord clamping may impede a successful transition and contribute to hypovolemic and hypoxic damage in vulnerable newborns. The authors present a model for neonatal transition based on and driven by adequate blood volume rather than by respiratory effort to demonstrate how neonatal transition most likely occurs at a normal physiologic birth.

Eating disorders, fertility, and pregnancy: relationships and complications.

Women are becoming heavier with each generation although the ideal female image emphasizes slimness. This focus results in the development of eating disorders in a significant number of women. The most common eating disorders are anorexia nervosa and bulimia nervosa. Eating disorder behaviors during pregnancy are associated with complications such as preterm delivery, low birthweight, intrauterine growth restriction, Caesarean birth, and low Apgar scores. Increasing the understanding of eating disorders assists health care professionals to accurately assess and intervene to improve a woman's n

The case against elective cesarean section.

Prominent American and British obstetricians have been advocating for performing more Cesareans. They argue that Cesarean section is as safe or nearly as safe as vaginal birth, eliminates pelvic floor damage and the consequent symptoms caused by vaginal birth, is safer for the infant, and is desired by many women; however, abundant evidence in the medical literature refutes the validity of those claims.

Cocaine: effects of in utero exposure on the fetus and neonate.

In North America, an increasing number of babies are prenatally exposed to cocaine, yet the implications of cocaine use during pregnancy are not fully understood. The effects of cocaine are exerted primarily by its influence on aminergic receptors in the central and peripheral nervous systems. Developmental, physiological, and behavioral problems in infants and children are likely outcomes of maternal cocaine abuse, but these findings are confounded by concomitant use of other drugs such as marijuana and cocaine and by factors such as time, dosage, and route of cocaine intake. Different screen

Prominence of withdrawal symptoms in polydrug-exposed infants.

The problem of licit and illicit drug use during pregnancy continues to be a major perinatal health issue in this country. It is estimated that 200,000 infants are born each year to women who used illegal drugs while they are pregnant. Much information is available regarding the physiologic and neurobehavioral signs and symptoms of withdrawal expressed in infants exposed in utero to individual substances such as heroin, marijuana, methadone, alcohol, barbiturates, and PCP. However, little information is available related to the signs and symptoms of withdrawal observed in infants exposed to ma

Intrahepatic cholestasis of pregnancy: a critical review.

Intrahepatic cholestasis of pregnancy (ICP) is a disease predominantly of the third trimester of pregnancy, characterized primarily by pruritus, biochemical disturbances in liver enzymes, and less frequently jaundice. Although maternal pruritus can be severe, overall maternal morbidity and mortality associated with ICP is low. However, fetal morbidity and mortality are significant with associated risks for meconium-stained amniotic fluid, acute onset of fetal compromise, spontaneous preterm labor, and intrauterine fetal demise. Current literature recommends obstetric management that includes f

Metabolic crisis: hyperemesis gravidarum.

Nausea and vomiting during pregnancy affect approximately 50% to 70% of all pregnant women. Although most cases of nausea and vomiting in pregnancy resolve spontaneously and are not associated with compromised nutritional status, a small percentage of cases progress to hyperemesis gravidarum (severe nausea and vomiting during pregnancy). Hyperemesis gravidarum is a serious disorder that can lead to weight loss, dehydration, electrolyte disturbances, and occasionally death if improperly treated or left untreated. The article summarizes recent research on hyperemesis gravidarum, focusing on the

Varicella infection and toxoplasmosis in pregnancy.

Varicella occurring in pregnancy can be dangerous for the fetus, the mother, and the newborn. The fetus may experience multiple system damage. The mother and newborn are at increased risk for varicella pneumonia with a 9% and 20% fatality rate, respectively. The recent introduction of the varicella vaccine will affect the occurrence of gestational infection. Toxoplasmosis is rarely dangerous for the pregnant woman, yet the fetus and newborn may be at risk for chorioretinitis, hydrocephalus, intracranial calcifications, and convulsions. The greatest challenge in the management of toxoplasmosis

Myasthenia gravis and pregnancy.

Pregnancy is certainly an option open to patients with myasthenia gravis. Preconceptional counseling is the ideal, with the goal of discontinuing or reducing corticosteroid medications in patients requiring those agents. Alterations in physiology will require ongoing and careful assessment of the pregnant myasthenic or cholinergic crises. Antenatal testing for fetal well-being may be difficult to interpret. A small number of fetuses with fetal myasthenia gravis will be diagnosed by ultrasound. Attempts may be made to decrease the amount of circulating antibody crossing to the fetus. Interventi

Perinatal superior sagittal sinus venous thrombosis.

The initial clinical findings that form the basis for the diagnosis of puerperal SSSVT are frequently quite subtle. It is essential to a favorable outcome that clinicians maintain a high index of suspicion in relation to physical complaints of puerperal women. Meticulous, comprehensive, multisystem assessment is indicated to explore fully the presentation of headache, weakness, or other subjective data when discovered. Headaches can easily be attributed to a multitude of factors associated with birth. These include fatigue, stress, complications of anesthesia, fluid and electrolyte imbalances,

Gestational diabetes mellitus screening tests: a review of current recommendations.

Early recognition and management of gestational diabetes decreases the incidence of macrosomia. Infant morbidity and mortality are also decreased. While there continues to be little agreement regarding precise methods of screening and diagnostic criteria for GDM, most specialists recommend screening with the 50-g oral glucose load followed by a 3-hour GTT. Screening only those women with select risk factors may result in 30% to 50% of women with gestational diabetes being overlooked. Careful explanation of testing methods and implications may result in improved patient understanding of the imp

Diabetic ketoacidosis during pregnancy.

Pregnancy increases the chances for the woman with type 1 diabetes to develop DKA, especially if the patient is noncompliant with glycemic control, if a concomitant infection occurs, or if tocolysis with a beta-adrenergic agent is necessary for preterm labor. Prompt recognition of the crisis state of DKA with immediate intervention and management is a challenge for the health care team. Approaches to correct the fluid imbalance and to restore glucose homeostasis are needed to stabilize the patient and to provide an optimum outcome for mother and fetus. Care of the pregnant patient with a pacemaker.

This article has reviewed cardiovascular adaptations to pregnancy and parturition. It has presented a basic overview of pacemaker supported cardiovascular function and related that to pregnancy. It has summarized 22 previously reported cases of pregnancy among women with pacemakers and reported yet another such case. The following conclusions can be drawn: The pregnant woman with a pacemaker, especially one that is not rate-responsive, faces risks for destabilized cardiovascular adaptation during the course of pregnancy, labor, and delivery. Selective reduction in multiple gestation.

As new advances in the treatment and management of infertility become available, it is hoped that selective reduction procedures will no longer be necessary. In the interim, however, it is imperative that nurses be knowledgeable about the options available to parents experiencing multifetal pregnancy, including the choice of selective reduction procedures.
