Treating body dysmorphic disorder with medication: evidence, misconceptions, and a suggested approach.

Body dysmorphic disorder (BDD) is a relatively common and often disabling disorder with high morbidity and mortality. Both psychotropic medication and cognitive behavioral therapy (CBT) are considered first-line treatments for BDD, and medication treatment is often essential for more severely ill and suicidal patients. In this practical overview of the pharmacotherapy of BDD, we briefly describe BDD's clinical features, associated morbidity, and how to recognize and diagnose BDD. We describe the importance of forming a therapeutic alliance with the patient, the need for psychoeducation, and ot

Self-reported systemic, immune-mediated disorders in patients with and without proplast-teflon implants of the temporomandibular joint.

PURPOSE: This study investigates the self-reported immune-related health status of patients exposed to Proplast-Teflon (P/T) temporomandibular joint (TMJ) implants, comparing their health status with a group of patients who were not exposed to any alloplastic TMJ implants. It also compares those whose implants were removed with those in whom they were retained. PATIENTS AND METHODS: Patients seen in a single oral and maxillofacial surgical practice completed a detailed self-report questionnaire about physical symptoms and disorders. Sixty-four had received P/T implants, and 22 were unexposed

[Prevalence of upper abdominal complaints and their effect on the quality of life and utilization of medical resources. Swiss Primary Care Group].

The authors present the results obtained in Switzerland, as part of an international survey (DIGEST), on 3 months' prevalence of upper digestive symptoms (UDS) and their influence on quality of life and consumption of medical services. 514 randomized adults from the general population in 8 different cities were interviewed. In these interviews data were recorded concerning demographic and socio economic aspects, quality of life, severity and frequency of UDS, consultations and medication. The sub-population with relevant UDS (i.e. UDS at least once a week and/or of moderate to severe degree) w

Early onset of obsessive-compulsive disorder and associated comorbidity.

BACKGROUND: Previous studies have aimed to identify subtypes of obsessive-compulsive disorder (OCD) based on their age of onset (AOO). Obsessive-compulsive spectrum disorders (OCS disorders) such as tic disorders have been particularly associated with an early onset in some studies. However, subtypes of early- and late-onset OCD are unevenly determined, and the biological and the clinical validity of these subtypes are unknown. This study was undertaken to discriminate the subtypes of OCD in different AOO levels and to test the hypothesis that different AOO bands are associated with a differen

Three forms of somatization presenting in primary care settings in Spain.

The objective of this paper is to study the prevalence and clinical characteristics of functional, hypochondriacal, and presenting somatization (FSTS, HSTS, and PSTS, respectively) defined by standardized criteria, as well as the validity of their distinction in primary care in Spain. A two-stage epidemiological study of a representative sample (N = 1559) of primary care patients was carried out. In the first phase, the validated Spanish versions of General Health Questionnaire, Mini-Mental State Examination, and CAGE were used. In the second phase, the Standardized Polyvalent Psychiatric Inte

[Sickle cell disease and depression: preliminary study using the Hamilton rating scale for depression].

Prolonged latency in the appearance of REM sleep as a marker of depression has been demonstrated in patients with the sickle-cell disease. To detect the possible existence of depressive disturbances in patients with sickle-cell disease, the Hamilton rating scale for depression (17 items) was used in 30 patients with homozygote sickle-cell disease and 31 carriers of the sickle-cell trait, treated or not with vasodilator drugs. None of the 61 subjects studied presented a score of 18 or more on the Hamilton rating scale, this being the threshold value for confirming the existence of moderate depr

Distressed high utilizers of medical care. DSM-III-R diagnoses and treatment needs.

Among a sample of 767 high utilizers of health care, 51% were identified as distressed by an elevated score on the SCL anxiety and depression scales, the SCL somatization scale, or by their primary-care physician. These distressed high utilizers were found to have a high prevalence of chronic medical problems and significant limitation of activities caused by illness. In the prior year, they made an average of 15 medical visits and 15 telephone calls to the clinic. The Diagnostic Interview Schedule was completed on 119 distressed high utilizers randomly assigned to an intervention group in a c

Pharmacological relapse prevention of alcoholism: clinical predictors of outcome.

OBJECTIVE: The efficacy of pharmacological relapse prevention in alcoholism with acamprosate and naltrexone has been supported by several controlled trials. It remains uncertain whether any differential indication for treatment exists. METHODS: We evaluated outcome data of a controlled trial on acamprosate and naltrexone in patients with low vs. high baseline somatic distress, depression and anxiety (Symptom Checklist-90, SCL-90), low vs. high baseline craving, and according to typological differentiation as proposed by Cloninger and Lesch. These variables have previously been suggested to be

Somatic symptom burden and distress in post COVID-19 and persistent physical symptoms: Evidence from combined SSD-12 and PHQ-15 factor analysis.

BACKGROUND AND AIM: Post COVID-19 condition (PCC) and persistent physical symptoms (PPS) may involve overlapping symptom presentations. We examined whether symptom-related dimensions differ or overlap between PCC and functional somatic disorders by comparing Somatic Symptom Disorder-B Criteria Scale (SSD-12; cognitive-affective distress) and the Patient Health Questionnaire-15 (PHQ-15; somatic symptom burden), and by performing a joint factor analysis of their items. METHODS: Two cohorts at Helsinki University Hospital were analysed: the Sympa cohort (2020-2024), comprising patients with PPS,

New Perspective on Psychosocial Distress in Patients with Dysphonia: The Moderating Role of Perceived Control.

OBJECTIVES: Although an association between psychosocial distress (depression, anxiety, somatization, and perceived stress) and voice disorders has been observed, little is known about the relationship between distress and patient-reported voice handicap. Furthermore, the psychological mechanisms underlying this relationship are poorly understood. Perceived control plays an important role in distress associated with other medical disorders. The objectives of this study were to (1) characterize the relationship between distress and patient-reported voice handicap and (2) examine the role of per

Association between functional gastrointestinal diseases and exposure to abuse in teenagers.

Abdominal pain-predominant functional gastrointestinal diseases (AP-FGD) are common in children and commonly attributed to exposure to child abuse. However, this relationship has not been studied in teenagers, and the main objective of the current study was to assess it. Teenagers were recruited from four randomly selected schools in Western province of Sri Lanka. Data were collected using a validated self-administered questionnaire. AP-FGD were diagnosed using Rome III criteria. A total of 1850 teenagers aged 13-18 years were included. Three hundred and five (16.5%) had AP-FGD. AP-FGD were si

Age-related differences in the course of alcohol withdrawal in hospitalized patients.

The aim of this study was to compare the course of alcohol withdrawal (AW) syndromes in different age groups of hospitalized patients. Medical records of 892 patients treated for AW in Nowowiejski Hospital in Warsaw, Poland from 1973 to 1987 were reviewed using a structured questionnaire; a further 321 patients were observed on a prospective basis in the years 1990-1999. We compared severity of the symptoms and the course of AW episodes in five age groups: <30, 30-39, 40-49, 50-59 and > or =60 years old. Although the age groups did not differ in respect of gender, there were significant differ

What are the real problems for patients with functional dyspepsia?

BACKGROUND: Patients with functional dyspepsia (FD) have more complaints than just the dyspepsia. METHOD: One hundred FD patients were assessed with regard to psychologic, medical, and social factors, before randomization, in a study of cognitive therapy. They were asked to list their main problem areas or 'target complaints'. RESULTS: Dyspepsia was the third most frequent target complaint (26 patients), and anxiety was the most frequent one (65 patients). Patients identifying dyspepsia as a target complaint differed from the other FD patients in several aspects. They had significantly more

Psychological profiles of patients with upper gastrointestinal symptomatology induced by non-steroidal anti-inflammatory drugs.

Eighty three patients with classical or definite rheumatoid arthritis taking non-steroidal anti-inflammatory drugs (NSAIDs) were studied in an attempt to determine whether a patient's personality and psychological profile might contribute to the development of NSAID induced gastrointestinal symptomatology. It was found that the personality profile of the group of 45 asymptomatic patients was similar to that of a previously reported control group. In contrast, the 37 patients with gastrointestinal symptoms attributed to NSAIDs had a significantly higher mean score for free floating anxiety, dep

Somatization disorder in a community population.

The data recently collected in the Piedmont region of North Carolina as part of the National Institute of Mental Health Epidemiologic Catchment Area Program were used to examine somatization disorder in a community population. The authors found an adjusted prevalence of 0.38% for somatization disorder diagnosed according to the Diagnostic Interview Schedule and DSM-III. Respondents with somatization disorder demonstrated demographic characteristics (female, unmarried, nonwhite, from a rural area, less educated) that are consistent with findings from past clinical and epidemiologic studies of s

Cardiologic perspectives of chest pain in childhood: a referral problem? To whom?

Discussed are the diagnostic and therapeutic issues of cardiac disease in the pediatric age group that have the potential to result in ischemic ventricular dysfunction. The discussion then turns to the nontraumatic thoracic, gastrointestinal, and psychogenic disturbances that are likely to produce symptoms of chest pain. In this context, the question of when to refer, when to reassure, when to begin a diagnostic evaluation, and when to institute longitudinal follow-up are addressed.
