[Treatment of empyema of the gallbladder by endoscopic drainage].

Medical history of an old female patient is described. Gallstones were extracted from the main bile duct after endoscopic papillotomy when she was 83 ys old. Eighteen months later, she presented an obstruction of the cystic duct with a consecutive empyema of gallbladder. The obstruction was relieved mechanically by inserting a catheter through guide-wire in the cystic duct and moving the impacted stone into the gallbladder. A naso-cholecystic drain was placed and kept in the gallbladder for six days to assure free passage of bile and to administer local antibiotic treatment. The patient quickl

Postoperative Abscesses in the Liver Resection Plane in Patients with a History of Biliary Tract Treatments: A Multicenter Cohort Study.

OBJECTIVE: The effect of previous biliary reconstruction (BR) or endoscopic sphincterotomy (EST) on post-hepatectomy bile leakage and abscess formation remains controversial. This study aimed to investigate the incidence, clinical findings, and risk factors of postoperative abscesses in the liver resection plane in patients with such histories. METHODS: This multicenter retrospective study included 5,742 patients who underwent liver resection between 2011 and 2020 (38 with a history of BR, 44 with EST, and 5,660 with no history of biliary treatment). The incidence, clinical characteristics, a…

Acute Cholangitis Secondary to a Clogged Biliary Stent: A Review on the Cause of Clogging and the Appropriate Time of Replacement.

Choledocholithiasis is preferably treated by endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy and stone removal, to reduce the risk for acute cholangitis. Frequently, patients who are ill, surgically unfit, or unable to undergo stone extraction during the index procedure underwent antibacterial treatment and temporary biliary stenting via ERCP to prevent biliary sepsis and septic shock. After a period of convalescence, a repeat ERCP is scheduled to clear the bile duct and remove the stent, followed by laparoscopic cholecystectomy to complete the treatment circuit. Chol…

Long-term survival after palliative argon plasma coagulation for intraductal papillary mucinous neoplasm of the bile duct.

Intraductal papillary mucinous neoplasm of the bile duct (IPNB) is an epithelial tumor that can cause obstructive jaundice and cholangitis d … Surgery was not recommended because of the extensive intrahepatic spread of the lesion. Endoscopic sphincterotomy, endoscopic papillary large balloon dilation, and insertion of a metallic stent could not resolve the obstructive jaundice. Finally, argon plasma coagulation with percutaneous cholangioscopy was performed 3 times over 1 month. After treatment, obstr …

Sigmoid Diverticulitis and Perforation Secondary to Biliary Stent Migration.

INTRODUCTION: Biliary stent migration occurs in 5-10% of patients. Generally, this is a benign process and stents pass or are retrieved endoscopically. In rare instances, intestinal perforation has occurred. PRESENTATION OF CASE: A 79-year-old female presented with a one-day history of abdominal pain. She had undergone an ERCP four weeks previously for primary choledocholithiasis during which time a sphincterotomy and sphincteroplasty were performed, and stents were placed in the common bile duct. CT scan of the abdomen and pelvis demonstrated a biliary stent that had migrated into the sigmoi…

[Therapy of chronic persisting biloma after liver resection by enteral drainage].

Postoperative bile leaks represent a typical complication in liver surgery with a frequency ranging from 5 % to 12 % in large series. The treatment of choice is usually conservative. Using sufficient transcutaneous drainage with flushing of the biloma cavity and endoscopic retrograde cholangiography (ERC) with sphincterotomy and possibly stenting, the cure rate of bile leaks is approximately 95 %. In very rare cases all of these measures remain unsuccessful especially in cases of leakage from separated liver segments without connection to the main bile duct system. In relevantly separated live…

Gallstone Ileus following Endoscopic Stone Extraction.

An 85-year-old woman was an outpatient treated at Tokyo Rosai Hospital for cirrhosis caused by hepatitis B. She had previously been diagnosed as having common bile duct stones, for which she underwent endoscopic retrograde cholangiopancreatography (ERCP). However, as stone removal was unsuccessful, a plastic stent was placed after endoscopic sphincterotomy. In October 2012, the stent was replaced endoscopically because she developed cholangitis due to stent occlusion. Seven days later, we performed ERCP to treat recurring cholangitis. During the procedure, the stone was successfully removed by…

Topical nitrate drip infusion using cystic duct tube for retained bile duct stone: A six patients case series.

A retained bile duct stone after operation for cholelithiasis still occurs and causes symptoms such as biliary colic and obstructive jaundice. An endoscopic retrograde cholangiopancreatography with endoscopic sphincterotomy (EST), followed by stone extraction, are usually an effective treatment for this condition. However, these procedures are associated with severe complications including pancreatitis, bleeding, and duodenal perforation. Nitrates such as glyceryl trinitrate (GTN) and isosorbide dinitrate (ISDN) are known to relax the sphincter of Oddi. In 6 cases in which a retained stone was…
