Anterior capsular reconstruction (ACR) using human dermal allograft (HDA) is an innovative surgical technique for managing irreparable subsc … ning, and structural support provided by this technique make ACR with HDA a valuable alternative to tendon transfers, preserving native shoulder biomechanics and offering a viable non-arthroplasty solution for severe anterior capsular deficiency.

The treatment of an irreparable rotator cuff tear is an enigma. There are so many treatment options, with different experts recommending different treatments. These include physical therapy, partial repairs, tendon transfers such as the latissimus dorsi and the lower trapezius, biceps tenotomy, tissue augmentation with allografts or autografts, balloon spacers, and finally a reverse prosthesis. It is an easy decision when the patient is physiologically old with many medical comorbidities and arthritis. It is much more difficult when the patient is younger with no arthritis. Many patients with …

The thumb frequently is involved in rheumatoid arthritis and often is a source of significant functional loss, pain, and deformity. Surgical intervention in patients with rheumatoid arthritis of the thumb should be based on the degree of radiological destruction according to Larsen, the natural course of the rheumatoid hand, the nature and stage of deformity as well as the status of tendons, ligaments, and adjacent joints. The goals of surgery are to relieve pain, increase motion, and restore thumb function. The timing for shoulder surgery should be early in the course of the disease, since it

The aim of this study was to determine the anatomical restraints to dislocation of the second metatarsophalangeal joint and to assess the bi … d load of five to ten newtons. The metatarsophalangeal joint was then repaired with use of a flexor tendon transfer in all twenty feet. This repair technique restored the load-displacement curves to that of the normal toe.

OBJECTIVE: To explore early curative effect of upper joint capsule reconstruction combined with biceps tendon transposition in treating irreparable rotator cuff tears.
METHODS: From October 2019 to March 2021, 16 patients with irreparable rotator cuff tear were underwent arthroscopic autogenous semitendinosus tendon transplantation for upper articular capsule reconstruction combined with biceps tendon transposition, included 12 males and 4 females, aged from 53 to 72 years old with an average of (62.13±5.35) years old; 3 patients on the left side and 13 patients on the right side. All patient

Multiple cranial nerve palsies often lead to complex clinical presentations. We report 2 cases in which a combination of multiple palsies resulted in paralytic esotropia with the medial rectus being the sole functioning rectus muscle. Both cases were treated with temporal transposition of split medial rectus.

Children with cerebral palsy often have spasticity of both the extensors and flexors, but how and why a flexion contracture of the wrist will develop during growth is not thoroughly understood. In order to understand the muscle adaptations that occur during contracture formation, the relationship between intraoperative sarcomere length and the extent of contracture was measured in 23 children (average age, 14.3 +/- 2.9 years) undergoing tendon transfers involving the flexor carpi ulnaris (FCU) or extensor carpi radialis brevis (ECRB) muscles. For both ECRB and FCU, sarcomere lengths measured i…

Isolated traumatic dislocation of the trapeziometacarpal joint is rare compared with fracture-dislocation of the joint. The mechanism of injury is usually axial loading on a flexed thumb metacarpal, leading to dorsal dislocation of the joint. Closed reduction with immobilization is an acceptable method of treatment if the joint is stable after the reduction. Otherwise, early ligamentous reconstruction is recommended to reduce the likelihood of secondary arthritis. Various surgical techniques have been used to reestablish the ligamentous integrity of the joint; however, these techniques usually

Following augmentation of the upper extremity muscles necessitated by damage to the brachial plexus, special post-operative care programs are required to provide the patient with the best possible results. Proper care requires cooperation between the orthopedic surgeon, physical therapists and nurses. Only then can postoperative rehabilitation and functional control of the extremity improve. We describe interdisciplinary postoperative care programs for augmentation of the deltoid muscle with trapezius transfer, arthrodesis of the glenohumeral joint, biceps augmentation with the latissimus dors

The authors evaluate the efficiency of the surgical technique of split anterior tibialis transfer, describe the long term functional results of the operation, and discuss the indications of this method of treatment. The results were very satisfactory: after surgery, 46 of 53 patients had normal shoes, 33 patients had improvement of their level of autonomy. The morbidity was very low, no patient had aggravation of the neurologic disease or general complications. The authors conclude that this operation should be employed more frequently in these disabled patients who can find definite improveme

We report the results of a new surgical procedure for spastic equinovarus deformity due to cerebral palsy. This is the transfer of the anterior half of the split tibialis posterior to the dorsum of the foot through the interosseous membrane. We performed the operation on 23 feet in 18 children. All patients were assessed before operation and at follow-up at a mean of 8.4 years postoperatively. Using the criteria of Kling et al (1985), excellent results were obtained in 14 feet, good results in eight, and a poor result in only one.

The surgical correction of 14 feet with spastic planovalgus in the neurologically impaired adult is reviewed. Evaluation of the patterns of lower extremity muscle activity preoperatively by dynamic EMG showed overactivity of the peroneus longus. A new gait pattern which has not been previously reported was observed. This "combination foot" deformity, noted in six patients, consists of equinovarus in swing, and planovalgus in stance during the gait cycle. The remaining eight patients exhibited planovalgus in swing and stance. Transfer of the peroneus longus tendon to either the cuboid or navicu

To clarify the role the flexor tendon to extensor hood transfer (Girdlestone-Taylor procedure) may have in correction of clawtoes in myelodysplasia, 20 patients with residual paralysis from myelomeningocele who underwent the procedure between 1976 and 1985 were reviewed. They were evaluated for the effectiveness of the procedure and occurrence of late deformities. A total of 25 feet with 65 toes were evaluated in patients ranging in age from 0.6 to 38.8 years. Results were classified as good if toes were straight, fair if mild deformity was present, and poor if definite clawing was noted. The

The authors describe their technique and results in the treatment of pes cavovarus by transposition of extensor hallucis brevis to the first metatarsal (Jones operation) in association with arthrodesis of the interphalangeal joint of the hallux. In cases of structural deformity it is also combined with osteotomy of the base of the first metatarsal.

The purpose of this paper is to present a case of the combined volar and radial ligament instability of a thumb metacarpophalangeal joint secondary to trauma. A successful reconstruction of the volar capsule and the radial collateral ligament was achieved using the tendon of the extensor pollicis brevis.

We studied the results of surgical treatment for rotational deformities of the lower extremity in fifty children. The preoperative rotational deformities were subdivided as follows: (1) external rotation deformity of the hip (nine hips), (2) external tibial torsion (twenty-nine limbs), and (3) toeing-in gait (thirty-one limbs). These were further analyzed as to imbalance between the forces of the medial and lateral hamstrings, that imbalance associated with fixed internal tibial torsion, and isolated internal tibial torsion. The average length of follow-up was four years and six months. For th
