Vestibular evoked myogenic potentials using simultaneous binaural acoustic stimulation.

The aim of this study is to investigate the feasibility of recording vestibular evoked myogenic potential (VEMP) using simultaneous binaural acoustic stimulation (B-VEMP), and compare it with that using monaural acoustic stimulation (M-VEMP). Seven healthy volunteers were evoked by initial B-VEMP test and subsequent M-VEMP test, whereas vice versa in another 7 volunteers. All 14 subjects demonstrated both B-VEMPs and M-VEMPs, without significant difference in the latencies of p13 and n23. When using interaural amplitude difference (IAD) ratio for interpreting amplitude, B-VEMPs did not differ

Motion sickness and vestibular hypersensitivity.

OBJECTIVE: Motion sickness is poorly understood, although it has been recognized for years as debilitating. Vestibular function is required for motion sickness to occur, but motion sickness can also be brought on without body motion. The aim of this study was to see if there was a correlation between caloric response and motion sickness susceptibility. DESIGN: One experiment was a prospective study carried out on 200 patients. A second prospective study was carried out on 121 patients. SETTING: Patients referred to our tertiary/quaternary care dizziness clinic. METHODS: In experiment 1, cal

Vestibulocollic reflexes: normal values and the effect of age.

OBJECTIVES: To define normal values and examine the influence of ageing on vestibulocollic reflexes (VCR). METHODS: Vestibulocollic responses to 100 dB (normal hearing level; NHL) clicks, forehead taps and galvanic stimulation were measured in 70 healthy adults aged 25-85 years. RESULTS: Click- and galvanic-evoked responses were present bilaterally in all subjects below 60. Average click-evoked response amplitudes decreased with age, with a pronounced decline of 25-30% per decade from the 6th decade. The average click thresholds increased from 85 dB in the third decade to 96.5 dB in the 8th

Effect of the character of autonomic response and the emotional and personality features of a human being on reactions of the vestibular system.

A moderate degree (R = 0.37-0.45) of correlation between the values of vestibular reactivity, autonomic response, and emotional and personality characteristics was revealed. A conclusion was made that the following features of the background functional organization of the vestibular system, autonomic and emotional and personality characteristics are space sickness predictors: (1) a high coefficient of enhancement of the vestibuloocular reflex (VOR) and its asymmetry with an excess of the physiological norm limits, (2) the presence of depressive manifestations, (3) the impulsive and autistic pe

Subjective visual horizontal during follow-up after unilateral vestibular deafferentation with gentamicin.

The subjective visual horizontal (SVH) was measured by means of a small, rotatable, luminous line in darkness in the upright head and body position and at 10, 20 and 30 degrees of tilt to the right and left before, and repeatedly during a follow-up period of 1 year after intratympanic gentamicin instillations in 12 patients with recurrent vertigo attacks. This treatment caused a loss of the bithermal caloric responses on the diseased side. Shortly after treatment there was a significant tilt of SVH towards the treated side (group mean = 10.6 degrees). Repeated testing made it possible to chara

Autonomic reaction to vestibular damage.

The vestibular system provides inputs to many neurons in the brain stem that participate in autonomic control. This multiplicity of vestibular-autonomic connections plays a variety of roles. Whereas it has been known for decades that unilateral vestibular lesions can result in motion sickness, recent data suggest that the vestibular system participates in making adjustments in blood pressure and respiration that are necessary to maintain homeostasis during movement and changes in posture. Animals with bilateral vestibular lesions are more susceptible to posturally related hypotension than vest

Brainstem interneurons necessary for vestibular influences on sympathetic outflow.

The objective of this study was to determine which general brainstem regions contain interneurons that are critical for mediating vestibulo-sympathetic responses in decerebrate cats, as a prelude for future cell recording studies. Large injections of kainic acid into the lateral reticular formation at levels caudal to the obex abolished sympathetic nerve responses elicited by electrical stimulation of the vestibular nerve. The same lesions also diminished excitatory and some inhibitory components of somato-sympathetic responses elicited by stimulation of the sciatic nerve, raising the possibil

[CT (high-resolution) in 60 cases of stapediovestibular otosclerosis with surgical indications].

Sixty patients with stapediovestibular otosclerosis (OEV) were studied by CT-scan (target program). These patients had stapedectomy indication because they had a conductive hearing loss in which one had an A/B gap of > 25 dB. The leading principle of this study was how to know the main indications of the CT-scan which makes it possible for morphologic and densitometric evaluation of the extension and probable activity of the stapedial, fenestral and fissural labyrinthine otosclerotic foci. A correlation between the CT pattern of otosclerosis and the pure or mixed hearing loss had been tried. T

Neurotrophin-4/5, brain-derived neurotrophic factor, and neurotrophin-3 promote survival of cultured vestibular ganglion neurons and protect them against neurotoxicity of ototoxins.

The ability of neurotrophin-4/5 (NT-4/5), brain-derived neurotrophic factor (BDNF), neurotrophin-3 (NT-3), and nerve growth factor (NGF) to promote survival of postnatal rat vestibular ganglion neurons (VGNs) was examined in dissociated cell cultures. Of the four neurotrophins, NT-4/5 and BDNF were equally effective but more potent than NT-3 in promoting the survival of VGNs. In contrast, NGF showed no detectable effects. As expected, TrkB-IgG (a fusion protein of extracellular domain of TrkB and Fc domain of human immunoglobulin G) specifically inhibited the survival-promoting, effects by NT-

The role of summating potential in the diagnosis and management of Menière's disease.

To assess the role of a negative summating potential (-SP) in the clinical diagnosis and management of Menière's disease, the relationship of -SP abnormality to clinical symptoms, hearing level and caloric test results was examined in 70 patients with unilateral Meniere's disease. The click-induced -SP and action potential (AP) were recorded by extratympanic electrocochleography. When SP/AP ratio exceeded 0.43, the -SP was considered to be abnormal. No significant relationship was found between abnormal -SP and clinical symptoms (the duration of cochlear and vestibular symptoms, recent hearing

Experimental endolymphatic hydrops: are cochlear and vestibular symptoms caused by increased endolymphatic pressure?

The correlation between inner ear pressure and cochlear function was investigated in guinea pigs with unilaterally obliterated endolymphatic sacs and ducts. In 11 animals that developed endolymphatic hydrops, auditory thresholds as monitored by auditory evoked action potentials had increased with recruitment. Most of these animals also experienced episodes of spontaneous nystagmus. In control ears endolymphatic pressure did not differ more than 0.5 cm H2O from perilymphatic pressure. In six of 11 hydropic ears, endolymphatic pressure was more than 0.5 cm H2O higher than perilympathic pressure;

Ototoxicity in neonates treated with gentamicin and kanamycin: results of a four-year controlled follow-up study.

This article reports the results of a four-year follow-up study initiated in 1970 on the long-term effects of gentamicin and kanamycin use in newborn infants. Audiometric, vestibular, and psychometric evaluations were performed on gentamicin-treated, kanamycin-treated, and untreated, matched control infants and children. No substantial sensorineural hearing loss or vestibular dysfunction was identified in these patients that could be attributed to aminoglycoside therapy. Generation and pathological characterization of a transgenic mouse model carrying a missense PJVK mutation.

Hearing loss is the most prevalent hereditary sensory disorder in children. Approximately 2 in 1000 infants are affected by genetic hearing loss. The PJVK gene, which encodes the pejvakin protein, has been linked to autosomal recessive non-syndromic hearing loss DFNB59. Previous clinical studies have revealed that PJVK mutations might be associated with a wide spectrum of auditory manifestations, ranging from hearing loss of pure cochlear origin to that involving the retrocochlear central auditory pathway. The phenotypic variety makes the pathogenesis of this disease difficult to determine. Si
