Don't Miss This! Red Flags in the Pediatric Eye Examination: Introduction and Essential Concepts.

Although the pediatric eye examination can be made more challenging by inattention, poor cooperation, sleep, or just the wiggles, these should never be justification for an inadequate assessment of the child. In fact, a challenging patient should make the examiner ever more careful to get a good history and to look carefully for potential red flags that should be examined in greater detail. Missing a history of diplopia, or not noticing an abnormal pupil, or blurred disc margins, to name a few, may delay the diagnosis and treatment of potentially life-threatening entities. In this symposium, r

Transcranial direct-current stimulation (tDCS) improves detection of simple bright stimuli by amblyopic Long Evans rats in the SLAG task and produces an increase of parvoalbumin labelled cells in visual cortices.

In this work visual functional improvement of amblyopic Long Evans rats treated with tDCS has been assessed using the "slow angled-descent forepaw grasping" (SLAG) test. This test is based on an innate response that does not requires any memory-learning component and has been used before for measuring visual function in rodents. The results obtained show that this procedure is useful to assess monocular but not binocular deficits, as controls and amblyopic animals showed significant differences during monocular but not during binocular assessment. On the other hand, parvoalbumin labelling was

Congenital nasolacrimal duct obstruction: common management policies among pediatric ophthalmologists.

PURPOSE: To study common management policies of congenital nasolacrimal duct obstruction (CNDLO) among pediatric ophthalmologists. METHODS: A 21-question survey was sent to members of the American Association for Pediatric Ophthalmology and Strabismus in April 2014. The questions focused on treatment of CNLDO during the first year of life, primary and secondary surgical interventions, surgical techniques, and amblyopia assessment. RESULTS: One hundred twenty-seven members completed the survey and 121 responses were analyzed after replies of 6 candidates in training were excluded. Eighty-two …

Inferior oblique transposition for large hypertropia after vertical rectus transposition.

Consecutive vertical deviations occur in 13% to 30% of cases after full tendon vertical rectus transposition (VRT) surgery in adult patients with abducens nerve palsy. We report a 7-year-old boy with congenital abducens nerve palsy who developed a large hypertropia following VRT with posterior fixation sutures. During subsequent surgery, extensive scarring of the vertical rectus muscles was observed, and an inferior oblique nasal anterior transposition was performed. The patient was orthotropic in primary position through 18 months of follow-up. The causes and management options for vertical d

Crowding, reading, and developmental dyslexia.

We tested the hypothesis that crowding effects are responsible for the reading slowness characteristic of developmental dyslexia. A total of twenty-nine Italian dyslexics and thirty-three age-matched controls participated in various parts of the study. In Experiment 1, we measured contrast thresholds for identifying letters and words as a function of stimulus duration. Thresholds were higher in dyslexics than controls for words (at a limited time exposure) but not for single letters. Adding noise to the stimuli produced comparable effects in dyslexics and controls. At the long time exposure th

Sensitivity of photoscreening to detect high-magnitude amblyogenic factors.

PURPOSE: To determine the sensitivity of a unique pupil-size based set of referral criteria of the MTI PhotoScreener(Medical Technology and Innovations, Inc, Cedar Falls, Iowa) to detect high magnitude refractive error. METHODS: The photoscreening photographs of 949 preschool children previously analyzed were reevaluated with the new referral criteria. The original photographs had been obtained from pediatricians' offices and public health and Women, Infants, and Children's (WIC) clinics. The results of this analysis were compared with the gold standard clinical examination and cycloplegic re

Acuity and contrast sensitivity in monkeys after neonatal intraocular lens implantation with and without part-time occlusion of the fellow eye.

PURPOSE: The authors used a monkey model to evaluate intraocular lenses (IOLs) for the treatment of infantile cataract in humans. Specifically, they sought to assess the effectiveness of IOLs, with and without occlusion therapy, in preventing amblyopia. METHODS: A diffuser contact lens was placed on one eye each of 11 neonatal monkeys to simulate an infantile cataract. A unilateral lensectomy, combined with the implantation of an IOL, was performed on the same eye 1 to 2 weeks after birth. Clear contact lenses were used to adjust the optical correction of the pseudophakic eyes to a near point…

The value of visual evoked potentials in the evaluation of periorbital hemangiomas.

PURPOSE: To present a case of a child with an extensive facial hemangioma in whom the suspicion of intracranial involvement was raised by visual evoked potential (VEP) evaluation. DESIGN: Observational case report. METHODS: VEP analysis was performed using flash and pattern stimuli to determine whether treatment intervention other than occlusion therapy was needed. RESULTS: VEP analysis indicated a marked transoccipital asymmetry in the flash and pattern VEP suggestive of right hemispheric dysfunction. Neuroimaging showed intracranial extension of the hemangioma. CONCLUSION: The VEP evalua

Prenatal tobacco exposure and visual disorders in children.

Maternal smoking during pregnancy is associated with adverse fetal and perinatal outcomes and may also affect visual system development in o … restriction, neonatal morbidity, and residual confounding. Evidence regarding refractive errors and amblyopia is less consistent, while data on structural visual pathway abnormalities remain limited. Proposed mechanisms include vascular dysfunction, altered cholinergic signaling, oxidative stress, inflammation, and impaired neurodevelopment. Conclusion: Pren …

Oral cyclophosphamide as part of the treatment regimen in patients with nonparaneoplastic autoimmune retinopathy secondary to birdshot chorioretinopathy.

The purpose of this case series is to delineate the rationale for employing oral cyclophosphamide along with rituximab in three patients with nonparaneoplastic autoimmune retinopathy (npAIR) secondary to birdshot chorioretinopathy. Case 1: A 50-year-old male with a history of amblyopia in the right eye (OD) presented with floaters and reduced vision in the OD. He was diagnosed with birdshot chorioretinopathy (BSCR) in both eyes (OU). He was successfully treated with cyclosporine and mycophenolate mofetil; however, later, he developed npAIR. He started rituximab infusion and oral cyclophosphami…
