A Long-Term Complication with Increasing Incidence: Intravesical Hem-o-Lok® Clip Migration after Robot-Assisted Laparoscopic Radical Prostatectomy.

Background: Hem-o-lok® clips (HOLC) (Weck Surgical Instruments, Teleflex Medical, Durham, NC) are commonly used to achieve athermal hemostasis and control the lateral vascular pedicles of the prostate, especially in nerve-sparing surgery during robot-assisted laparoscopic radical prostatectomy (RALP). However, an increasing number of studies have demonstrated intravesical HOLC migrations. This study aims to describe the clinical presentation, symptoms, management, and outcomes of intravesical HOLC migration to the vesicourethral anastomotic site (VUAS). Methodology: A retrospective analysis wa

Late Conversion of a Distally Migrated Abdominal Aortic Endograft in a Symptomatic Patient.

OBJECTIVE: To identify predictive factors of clinical outcome of subacute thyroiditis. DESIGN: Retrospective case series of 56 consecutive patients treated in 3 outpatient clinics between 1999 and 2005. Medical records were reviewed for demographic data, seasonal disease distribution, laboratory and clinical course, treatment, and short-term outcome. MAIN OUTCOME: Mean age was 48.6+/-12 yr; 70% were females. Twenty-five percent had antithyroid antibodies and 9% had recurrent disease. Differences in occurrence by season were not significant (p=0.28). Ultrasound, performed in 35 patients, reve

Gastropleurocutaneous fistula secondary to cement prosthesis migration: A rare late complication of chest wall sarcoma reconstruction.

BACKGROUND: Gastropleurocutaneous fistula (GPCF) is a rare, potentially life-threatening condition defined by an abnormal connection between the stomach and the pleural cavity with an accompanying cutaneous tract. It is distinct from a gastropleural fistula, which lacks an external cutaneous extension. To our knowledge, no prior reports have described GPCF caused by migration and erosion of a cement chest-wall prosthesis. CASE PRESENTATION: We present the case of a 40-year-old female with a history of anterior chest wall sarcoma resection and cement prosthesis reconstruction performed 23 year

A rare complication of tension band fixation of olecranon osteotomy: Distal migration of K-wire.

Tension band wiring (TBW) is one of the most commonly used fixation techniques to fix olecranon osteotomies. Hardware prominence has been the most commonly reported complication of TBW. However, distal migration of Kirschner (K)-wire after TBW fixation for olecranon osteotomy has not been reported. In this case report, we presented distal migration of K-wire detected nine months after initial surgery in a 46-year-old male patient. The patient was operated on for an intraarticular distal humerus fracture using an olecranon osteotomy. The osteotomy was fixed with TBW fixation. The patient missed

Intrathoracic Sleeve Migration Following Sleeve Gastrectomy: Incidence and Outcomes.

PURPOSE: With laparoscopic sleeve gastrectomy (LSG) taking its place as the primary metabolic bariatric procedure in the past decade, de novo or persistent gastroesophageal reflux disease (GERD) has come into the spotlight as one of the most debilitating postoperative complications. Among the causes of GERD, intrathoracic migration (ITM) of the sleeve has become an understated yet significant phenomenon. This study aims to analyze the actual incidence of spontaneous ITM at our center, as well as its relationship to the baseline characteristics of patients and perioperative outcomes. MATERIALS

Successful Management of Myval Transcatheter Heart Valve Embolization Into Abdominal Aortic Aneurysm During Transcatheter Aortic Valve Replacement.

Transcatheter heart valve embolization is a serious and rare complication of transcatheter aortic valve replacement. Having a strategy for promptly managing transcatheter heart valve embolization is crucial to avoid emergency conversion from transcatheter aortic valve replacement to open-heart surgery. Many cases of transcatheter heart valve embolization occurring with balloon-expandable prostheses such as the SAPIEN 3 (Edwards LifeSciences Corporation) valve and self-expandable prostheses such as the ACURATE neo (Boston Scientific Corporation) valve have been reported in the literature. Here,

Is the French Paradox cementing philosophy superior to the standard cementing? A randomized controlled radiostereometric trial and comparative analysis.

AIMS: Highly polished stems with force-closed design have shown satisfactory clinical results despite being related to relatively high early migration. It has been suggested that the minimal thickness of cement mantles surrounding the femoral stem should be 2 mm to 4 mm to avoid aseptic loosening. The line-to-line cementing technique of the femoral stem, designed to achieve stem press-fit, challenges this opinion. We compared the migration of a highly polished stem with force-closed design by standard and line-to-line cementing to investigate whether differences in early migration of the stems

A Retrospective Case Series of a Novel Spinal Cord Stimulator Trial Technique with Less Displacement and Migration of the Trial Leads.

BACKGROUND: Spinal cord stimulation is an established treatment option for certain chronic pain conditions which have been previously unresponsive to conservative therapies or potentially for a subset of patients who have not improved following spine surgery. Prior to permanent lead implantation, stimulator lead trials are performed to ensure adequate patient benefit. During these trials, one of the most common complications and reasons for failure is the displacement and migration of the trial leads, resulting in lost therapeutic coverage. Other complications include infection and dislodged b

Endovascular Coil Migration Into the Intestinal Lumen: Two Cases of Successful Nonoperative Management.

Endovascular embolization of visceral arteries is commonly performed as treatment for aneurysms, pseudoaneurysms, and emboloradiation of liver tumors; while considered relatively safe, it is not without complications. We are reporting 2 cases of coil migration into the gastrointestinal tract. Patients were successfully managed without endoscopic or surgical coil removal. Patients were followed after discharge and noted to have no further complications from their migrated coils. These cases highlight the success of expectant management for coil migration. We recommend against invasive intervent

Prominent metalware from pelvic surgery causing dyspareunia.

We present a case of female dyspareunia secondary to metalware placement during extensive pelvic surgery following a motor vehicle accident. The patient initially had an uneventful recovery from her operations. However, she noticed pain with vaginal intercourse, due to a screw tip which was palpable on vaginal examination. X-ray imaging confirmed long screws in the medial part of an anterior column plate, which were impacting on the anterior vaginal wall. Subsequent percutaneous removal of two screws resulted in resolution of her symptoms of painful vaginal intercourse. While the pain from mec

Delayed extrusion of embolic coils into the airway after embolization of an external carotid artery pseudoaneurysm.

Carotid blowout syndrome (CBS) is a known devastating complication of head and neck surgery. The risk of developing CBS increases in the setting of radiation therapy, wound breakdown, or tumor recurrence. Traditionally, the treatment of choice for CBS is surgical ligation of the bleeding artery; however, recently, endovascular occlusion has become a more common option. If a pseudoaneurysm is present, treatment consists of trapping with endovascular coils or occlusion with a liquid embolic agent. Delayed migration of embolization coils into the airway causing acute respiratory distress is a rar

Migrating coil.

We describe a rare case of a 60-year-old man with known history of peptic ulcer disease who presented with melena and epigastric pain secondary to coil migration into duodenal mucosa 4 years after the initial therapeutic embolisation of the gastroduodenal artery. Upper endoscopy revealed oozing duodenal ulcer at the same site of the previously located duodenal ulcer 4 years ago and metal coil impacted at the duodenal mucosa. It is unclear if the coil migration is the effect or the cause of the bleeding duodenal ulcer. Our patient was treated by surgical intervention due to failed endoscopic ha

Anterior Migration After Bryan Cervical Disc Arthroplasty: The Relationship Between Hyperlordosis and its Impact on Clinical Outcomes.

BACKGROUND: Various modifications have been tested to prevent kyphosis after Bryan cervical disc arthroplasty (CDA). However, the migration of Bryan prostheses has not been systematically studied. This study investigated the cause of anterior migration (AM) and assessed the effect of AM on clinical and radiographic outcomes. METHODS: We retrospectively reviewed 46 consecutive patients who underwent modified Bryan CDA between August 2006 and December 2010. We measured functional spinal unit, angle of operative disc space, range of motion, and sagittal alignment of cervical spine preoperatively

A Novel Technique for Correcting Peritoneal Dialysis Catheter Malposition and Blockage.

Methods To investigate the safety and clinical significance of the method described in this study, we focused on 16 peritoneal dialysis patients with peritoneal dialysis (PD) catheter malposition and blockage in whom nonsurgical reposition was ineffective, who received a local incision about 5 cm below hypogastrium PD catheter insertions under local anesthesia. Tissues were separated layer by layer, 1-cm incisions were performed on the peritoneum vertically and conventionally, and then the PD catheters were pulled. Adherent mesentery was separated and the PD catheters were freed and removed su
