Posterior column acetabular fractures treated through the modified Stoppa approach: Clinical and radiological outcomes.

BACKGROUND: Acetabular fractures with posterior column involvement are traditionally managed with fixation strategies that may include medial buttress plating, particularly in fractures with quadrilateral plate involvement. However, the necessity of routine medial buttress support remains debated. With the evolution of intrapelvic approaches, especially the modified Stoppa approach, stable fixation may be achievable without additional medial buttress plating in selected fracture patterns. This study evaluates the clinical and radiological outcomes of posterior column acetabular fractures treat

Bone reconstruction with modified Masquelet technique in open distal femoral fractures: a case series.

BACKGROUND: Large bone defects require complex treatment, multidisciplinary resources, and expert input, with surgical procedures ranging from reconstruction and salvage to amputation. The aim of this study was to provide the results of a case series of open comminuted intra-articular distal femoral fractures with significant bone loss that were managed by early fixation using anatomical plates and a modified Masquelet technique with the addition of surgical propylene mesh. METHODS: This retrospective study included all patients referred to our institution with OTA/AO C3 distal femur open fra

Atypical fractures at non-classical sites associated with anti-resorptive therapy: a systematic review.

Osteoporosis is common, affecting more than 20% of women and 6% of men globally over the age of 50. Anti-resorptive drugs, bisphosphonates and denosumab, have been effective treatments for osteoporosis for more than 30 years. Rare complications of anti-resorptive therapy include medication-related osteonecrosis of the jaw and atypical femur fractures (AFF). The American Society for Bone and Mineral Research (ASBMR) proposed a case definition for these atypical femoral fractures in 2010, which was updated in 2013. However, atypical fractures at non-classical sites have been increasingly describ

[Distal Humerus Fracture in the Elderly].

Distal humerus fractures show increasing incidence with age. Due to preexisting osteoporosis, the surgeon is regularly confronted with complex comminuted intraarticular fractures. The maintenance of autonomy in daily life and avoidance of permanent care dependency is a dogma in the care of geriatric patients. Accordingly, a stable post-operative situation that allows early functional post-operative treatment is mandatory, especially in the elderly patient. Open reduction and stable internal fixation using double plate osteosynthesis are the current osteosynthetic standard of treatment. Modern

A comparison of locked versus nonlocked Enders rods for length unstable pediatric femoral shaft fractures.

BACKGROUND: Stainless steel flexible Enders rods have been used for intramedullary fixation of pediatric femur fractures with good success. Despite intraoperative anatomic alignment, length unstable femur fractures can present postoperatively with fracture shortening. The purpose of this study was to review all length unstable pediatric femoral shaft fractures in which Enders rods were used and compare those that were locked to those that were not locked. METHODS: A retrospective clinical and radiographic review of all patients at a single institution undergoing flexible intramedullary fixati

Imaging in traumatic mandibular fractures.

A fracture of the mandible is a common trauma presentation amongst young males and represents one of the most frequently encountered fractured bones within the viscerocranium. Historically, assault was the dominant contributing factor but now due to the increased number of vehicles used per capita, motor vehicle accidents are the primary cause. Mandibular fractures can be classified anatomically, by dentition, by muscle group and by severity. The fracture may also be closed, open, comminuted, displaced or pathological. It is important that the imaging modality used identifies the classificatio

Pediatric hand fractures: a review.

OBJECTIVE: To identify the incidence, causes, and details of hand fractures in children. DESIGN: A retrospective chart review. METHODS: Records of children under 16 years of age who had sustained a hand fracture within the last 5 years were collected from the patient population of British Columbia's Children's Hospital. A total of 242 hand fractures in 232 patients were documented. These patients were reviewed for age at time of injury, gender, location of the incident, mechanism of injury, number of radiographs taken, and fracture specifics. Radiographs with obscure details or incomplete fo

Type III odontoid fractures: A subgroup analysis of complex, high-energy fractures treated with external immobilization.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: Type III odontoid fractures are classically treated nonoperatively, yet, the current literature on Type III odontoid fractures includes fractures of multiple etiologies and fracture morphologies. We hypothesize that a subgroup of complex, Type III fractures caused by high-energy mechanisms are more likely to fail nonoperative treatment. MATERIALS AND METHODS: Acute Type III odontoid fractures were identified at a single institution from 2008 to 2015. Fractures were categorized as high- or low-energy fracture with high-energy fractures defi

Complex fractures, do we operate on enough to gain and maintain experience?

Complex fractures are generally assumed by our profession to require adequate training and continuing practice to treat optimally. The quantity of complex fractures treated in individual hospitals and by or under the care of individual orthopaedic consultants may have implications regarding the quality of care for particular patients and also for the training of specialist registrars.A complex fracture was defined as a comminuted peri- or intra-articular fracture or segmental shaft fracture: fractures acknowledged at specialist fracture courses and by special trauma surgeons to require particu

Delayed flexor tendon entrapment in callus after an open comminuted distal radius physeal injury in a child: A case report.

Tendon entrapment within fracture callus is an uncommon complication in children, particularly after forearm fractures. We report an 8-year-old boy with an open comminuted distal radius physeal injury treated with debridement, open reduction, and K-wire fixation, who later developed an isolated flexion contracture of the index finger at the distal interphalangeal joint. Surgical exploration revealed entrapment of the flexor digitorum profundus tendon of the index finger within a bony tunnel formed by exuberant callus. The bony tunnel was excised, and tenolysis was performed. The child regained

How I do it: spine jack expansion kyphoplasty for AO spine A4 complete burst fracture with neurological deficit.

BACKGROUND: Some patients suffering from thoracolumbar complete burst fracture causing neurological deficit may be eligible for single-stage posterior-only three column reconstruction with spine jack expansion kyphoplasty. METHOD: Short segment monoaxial pedicle screws are placed. Spine jack working channels are positioned in the comminuted vertebral body. Then, the surgeon can perform jack expansion kyphoplasty and short-segment fixation and distraction in the order best suited, considering whether the collapsed vertebral body has been expanded thanks to hyperlordosis positioning. Treatment of Femoral Shaft Pseudarthrosis, Case Series and Medico-Legal Implications.

Pseudarthrosis (PSA) is a possible complication of femoral shaft fracture treatment. It is often associated with reduced bone quality and can, therefore, adversely affect quality of life. Its treatment poses a major challenge for orthopaedic surgeons. Several authors have set forth different surgical approaches for the treatment of pseudarthrosis, such as internal fixation with plate and screws, replacement of an intramedullary nail or prosthetic replacement. In cases associated with bone loss, osteopenia, or comminution of fracture fragments, autologous or homologous bone grafts may also be u

Surface-Stress Induced Embrittlement of Metals.

Environment-assisted fracture phenomena in metals are usually associated with surface energy reduction due to an adsorbed film. Here we demonstrate a unique embrittlement effect in Al that is instead mediated by surface stress, induced by an adsorbed organic monolayer. Atomistic simulations show that the adsorbate carbon-chain length lc controls the surface stress via van der Waals forces, being compressive for lc < 8 and tensile otherwise. For lc > 8, we demonstrate experimentally that the nanoscale film causes a ductile-to-brittle transition on the macroscale. Concomitant with this transitio

Reamed Exchange Nailing in Nonunion of Tibial Shaft Fractures: A Review of the Current Evidence.

One of the most dreaded complications of fracture management is a nonunion. Nonunions are usually difficult to manage and can be a source of significant mental, physical, and financial distress to the patient. The incidence of nonunion is dependent on multiple factors including degree of comminution, open versus closed, concomitant infection, and vascular status, and therefore the management of such nonunions continues to be an often debated topic. Currently, there is no clear consensus on the role of reamed exchange nailing for tibial shaft nonunions. While reamed exchange nailing for aseptic

Skull fracture caused by vacuum extraction.

BACKGROUND: The vacuum extractor is being increasingly advocated as the instrument of first choice for assisted vaginal delivery. It is widely believed that the vacuum cup will dislodge before causing serious fetal trauma. CASE: Rotational delivery of a term infant was effected using a vacuum extractor. A 6-cm Malmström metal cup with a paramedian application was in place for 12 minutes. The vacuum pressure developed was 0.8 kg/cm2. Four traction efforts with contractions were required to deliver the fetal head. A neonatal skull x-ray the following day showed a comminuted parietal bone fractu

Repair of a proximal diaphyseal femoral fracture in a calf, using intramedullary pinning, cerclage wiring, and external fixation.

Repair of a comminuted, spiral oblique, proximal diaphyseal femoral fracture in a 7-day-old calf was achieved by use of an intramedullary pin, cerclage wires, and external fixator. Six stainless steel wires were used for full cerclage to secure a long butterfly fragment and multiple incomplete and complete nondisplaced fragments to the femur in order to create 2 principal fragments. Axial alignment and resistance to bending was provided by a round, double-pointed, end-threaded intramedullary pin (6.35 mm in diameter), which was inserted in a retrograde fashion. A type-1, double-connecting-bar

Surgical anatomy of the craniofacial dysostoses: insights from CT scans.

Six patients with midface retrusion secondary to craniofacial dysostoses were evaluated by computer assisted tomography (CT) before and after surgery. Computer reformated life-size images were generated for linear measurements and for assessing the encroachment upon the orbit of the anterior and middle cranial fossae, the lateral displacement of the ethmoids, and the retrusion of the maxilla. Dynamic ocular CT demonstrated alterations in the angle of divergence of the optic nerve with globe movement which made measurements of that angle an unreliable assessment of hypertelorism.
