Silent Megacolon: Fulminant Clostridioides Difficile Infection Without Diarrhea Mimicking Acute Colonic Pseudo-Obstruction.

Clostridioides difficile infection (CDI) typically presents with diarrhea in elderly or antibiotic-exposed patients. We report a 40-year-old immunocompetent man with alcohol use disorder who presented with abdominal pain and jaundice, rapidly progressing to refractory constipation and colonic dilation (14.7 cm) mimicking acute colonic pseudo-obstruction. Conservative management including bowel regimens and neostigmine (generally contraindicated in suspected toxic megacolon) were attempted. Colonoscopic decompression revealed cecal pseudomembranes whereas rest of colon was grossly normal. Stool

Endoscopic ultrasound-guided gallbladder drainage as a second option of biliary decompression after failed endoscopic retrograde cholangiopancreatography for the management of malignant distal biliary obstruction.

Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) is recommended as a rescue treatment of malignant distal biliary obstruction (MBDO) after failed ERCP and endoscopic ultrasound-guided biliary drainage (EUS-BD). A 64-year-old male was admitted for painless obstructive jaundice and anemia. For religious reasons, he refused any blood transfusions. Abdominal computed tomography scan showed a pancreatic tumor with dilation biliary tree and liver metastasis. ERCP failed and advanced biliary cannulation technique such as precut were avoided due to a high risk of bleeding. We avoided the tw

A Case of Concurrent Gastric and Pancreatic Plasmacytomas in a Patient With Multiple Myeloma: An Extremely Rare Entity.

Multiple myeloma (MM), a plasma cell tumor, is primarily a disease of the bone marrow. Extramedullary plasmacytoma, also a plasma cell tumor, is very rare in the gastrointestinal tract and the pancreas, and only a handful cases have been documented till now. Gastric and pancreatic plasmacytomas are usually seen in elderly patients; however, cases in patients as young as 32 years of age have been reported. Commonly, patients with gastric plasmacytoma present with nonspecific symptoms like epigastric pain, abdominal fullness, anorexia, and weight loss, or serious conditions like massive upper ga

Temporary endosonography-guided biliary drainage for transgastrointestinal deployment of a self-expandable metallic stent.

Endosonography-guided biliary drainage (ESBD) is now gaining acceptance as a useful alternative for the management of obstructive jaundice.(1) At present, ESBD is used mainly to establish an anastomosis between the biliary tree and the duodenum, stomach, jejunum, or esophagus by placing a stent so as to bridge the bile duct and alimentary tract. We herein report a new application of ESBD, that is, its temporary use for gaining access to the bile duct in order to deploy a self-expandable metallic stent (SEMS) via the transhepatic route. In a patient with pylorus stenosis due to advanced gastric

Pancreaticoduodenectomy of ampullary carcinoma.

Thirty-two consecutive patients with adenocarcinoma of the ampulla of Vater who had curative resection by pancreaticoduodenectomy were analyzed to determine the accuracy of preoperative investigations and factors that influenced survival. Obstructive jaundice was present in 31 patients, and most patients had pain and weight loss. Ultrasound was more useful than CT in identifying biliary obstruction, whereas CT was more accurate in demonstrating pancreatic duct dilatation and an ampullary mass. Endoscopic retrograde cholangiopancreatography with biopsy and brush cytology was the most accurate i

Sepsis associated with transhepatic cholangiography.

A retrospective study was carried out of 74 elderly patients with obstructive jaundice undergoing percutaneous transhepatic cholangiography (PTC) and/or percutaneous biliary drainage (PBD) in order to assess the effect of prophylactic antibiotics on the incidence of fever and sepsis complicating these procedures. Seventeen patients underwent PTC alone, while 57 had both PTC and PBD. Fifty-three patients had either primary or metastatic malignancy. In the other patients with benign disease, choledocholithiasis was the most common reason for undertaking these procedures. Prophylactic antibiotics

[Mirizzi's syndrome].

Six cases of Mirizzi syndrome are reported. The syndrome consists of a special anatomical variant of the cystic duct, which has a low opening but runs side-by-side with the common bile duct, associated with entrapment of a gallstone in the cystic duct or the neck of the gallbladder, partial or total obstruction of the hepatic duct by the stone and by inflammatory lesions, and recurrent cholangitis. Clinical signs are non-specific and suggest at first sight an obstructive jaundice. Pre-operative morphological examination seldom provide a diagnosis before surgery. In the most typical cases ultra

Interpretative problems in endoscopic retrograde cholangiopancreatography.

1. Of forty-one patients with biliary tract problems studied by endoscopic retrograde choledochopancreatography, interpretation of ductograms or endoscopic observations in ten could not be accomplished with ease for one or several reasons. 2. Although incomplete filling of the bile ducts usually signifies organic obstruction, this occasionally is not true. The specific origin and histologic character of the lesion causing bile duct obstruction are not delineated by retrograde endoscopic cholangiography. 3. Other occasional limitations of endoscopic cholangiography include difficulties in demon

Massive Upper Gastrointestinal Bleeding From Ruptured Gastric Varices: A Rare Presentation of Type 1 Autoimmune Pancreatitis.

A 59-year-old Thai woman presented with jaundice and massive hematemesis complicated by cardiac arrest. Emergency endoscopy revealed isolated gastric varices type 1, successfully treated with cyanoacrylate injection. Axial imaging demonstrated diffuse pancreatic enlargement with splenic vein compression and distal common bile duct obstruction. Endoscopic retrograde cholangiopancreatography with biliary stenting was performed. Endoscopic ultrasound (EUS) showed diffuse mixed-echogenic pancreatic infiltration, and EUS-guided biopsy revealed dense lymphoplasmacytic infiltration with 20 immunoglob

Afferent loop obstruction induced by gallstone ileus after Whipple procedure: a case report and literature review.

Afferent loop obstruction (ALO) induced by gallstone ileus after pancreaticoduodenectomy (PD) is rare. We present a case of ALO after the Whipple procedure in a 62-year-old male with a rare cause of gallstone ileus. He was admitted to the hospital with epigastric pain, fever, and jaundice two years after experiencing intermittent abdominal pain. With the help of computed tomography (CT) and magnetic resonance cholangiopancreatography (MRCP) gallstone ileus was confirmed and the patient underwent emergency surgery. Intraoperative findings indicated that internal herniation, adhesions and a stuc

Diseases of bile duct in children.

Several diseases originate from bile duct pathology. Despite studies on these diseases, certain etiologies of some of them still cannot be concluded. The most common disease of the bile duct in newborns is biliary atresia, whose prognosis varies according to the age of surgical correction. Other diseases such as Alagille syndrome, inspissated bile duct syndrome, and choledochal cysts are also time-sensitive because they can cause severe liver damage due to obstruction. The majority of these diseases present with cholestatic jaundice in the newborn or infant period, which is quite difficult to

Titanium clip migration after cholecystectomy: original technique for laparoscopic transcystic extraction when endoscopic retrograde cholangiopancreatography fails.

Surgical clip migration into the common bile duct (CBD) is a rare complication after laparoscopic cholecystectomy (LC). Few cases of surgical clip migration have been reported in the literature, and most of them have been successfully treated with endoscopic retrograde cholangiopancreatography (ERCP). We present a 71-year-old woman with 48 h of abdominal pain, jaundice and fever 6 years after laparoscopic cholecystectomy. She was diagnosed with common bile duct obstruction from surgical clip migration. After failure of ERCP, the patient was successfully treated with an innovative approach by l

Acute cholecystitis caused by malignant cystic duct obstruction: treatment with metallic stent placement.

We report the successful management of acute cholecystitis using cystic duct stent placement in 3 patients with inoperable malignant cystic duct obstruction (2 cholangiocarcinoma and 1 pancreatic carcinoma). All patients underwent stent placement in the bile duct, using an uncovered stent in 2 and a covered stent in 1, to relieve jaundice occurring 8-184 days (mean 120 days) before the development of acute cholecystitis. The occluded cystic duct was traversed by a microcatheter and a stent was implanted 4-17 days (mean 12 days) after cholecystostomy. Acute cholecystitis was improved after the

Follicular lymphoma of the gallbladder and extrahepatic bile ducts.

We report a case of a 63-year-old female, who presented with symptoms and signs of an acute biliary tract obstruction with jaundice and pain. Ultrasound together with intraoperative examination suggested a Klatskin tumour. Following radical resection, a tumour located predominantly in the wall of gallbladder, but infiltrating extensively also the walls of cystic, common hepatic and choledochus duct, was found. On the basis of cell morphology and the results of immunohistochemical analysis, the tumour was classified as an extranodal follicular lymphoma, grade II of the gallbladder, involving al

Malrotation with recurrent volvulus presenting with cholestasis, pruritus, and pancreatitis.

A patient aged 16 years was referred following numerous episodes of intense pruritus associated with persistently altered liver function tests. Ultrasound (US) demonstrated the superior mesenteric vein positioned more to the left than usual and abnormal orientation of the duodenum, consistent with a midgut malrotation. Endoscopic retrograde cholangiopancreatography the common bile duct, suggesting a malrotation with chronic or recurrent volvulus. A classic malrotation with 180 degrees volvulus was noted at operation. A second child previously thought to have recurrent idiopathic pancreatitis w

Primary biliary cystic tumors of the liver.

Five cases of primary cystic tumors of the intrahepatic bile duct are documented over a 5-yr period. Clinically, two cases had recurrent episodes and one had a first attack of jaundice, fever, and pain in the upper abdomen; one complained of abdominal fullness and one was asymptomatic. The appearance of the tumors were single, large, multilocular, and cystic. Important radiological findings included irregular thickness of the cystic wall, internal septation, and papillary projection. Marked biliary dilation was demonstrated in four of the patients. On the cholangiogram, amorphous filling defec

Immunocytochemical studies on cholestatic factor in human liver with or without cholestasis.

The localization of the cholestatic factor (CF) was immunocytochemically investigated in liver biopsy specimens obtained from patients with various liver diseases. CF was detected in seven of nine patients with drug-induced liver injury, three of four with acute viral hepatitis, three of five with alcoholic liver injury and in the two patients with autoimmune hepatitis. Fourteen of these 15 CF-positive patients had jaundice in their clinical courses. CF was stained diffusely in the cytoplasm of hepatocytes throughout the lobules in a granular pattern. Electron-microscopically, it was localized
