Evaluation of artificial nerve conduit and autografts in peripheral nerve repair in the rat model of sciatic nerve injury.

OBJECTIVE: To investigate the therapeutic effect of artificial nerve conduit in the sciatic nerve injury and repair in the rat model. METHODS: A total of 60 adult male Sprague Dawley rats were evenly randomized into five groups to build the model of sciatic nerve injury and perform the injury repair experiment. The five groups were: group A which was treated with artificial nerve conduit, group B which was treated with common carotid artery (CCA) autograft, group C which was treated with sciatic nerve autograft, group D which was treated with sham operation, and group E as the normal control.

Expression of developmentally regulated cytoskeleton and cell surface proteins in childhood spinal muscular atrophies.

Expression of some developmentally regulated cytoskeleton components (desmin, vimentin and myosin heavy chain isoforms) and cell surface proteins (including neural cell adhesion molecule (NCAM), its polysialylated (PSA) isoform and CD24) have been studied by immunohistochemical detection in a series of 23 infantile spinal muscular atrophies (SMA). According to the clinical classification established by Byers and Banker in 1961, 8 cases were type I SMA (Werdnig-Hoffmann's disease), 10 cases were type II (intermediate form), and 5 cases were type III (Kugelberg-Welander's disease). In 15 cases,

[A patient with mesenteric lymphoma who developed amyotrophic lateral sclerosis and sepsis].

We treated a patient with mesenteric lymphoma who concomitantly developed amyotrophic lateral sclerosis (ALS). The patient died of urinary tract infection nine months after the onset of ALS. We herein report the changes in the patient's condition and the sequence of events until death from the viewpoint of a physiotherapist. The patient was a 69-year-old woman who developed mesenteric lymphoma in September of X year and perceived weakness in the toes in November of X year. She showed signs of upper and lower motor neuron disorders, and electrophysiologic testing revealed denervation in three a

Facial Paralysis Treatment and Facial Symmetrization with Botulinum Neurotoxin: A Narrative Review with Illustrative Clinical Cases.

Facial paralysis leads to static and dynamic asymmetry, hyperkinesis of the non-paralyzed side, and synkinesis, with major functional and psychosocial consequences for patients. Botulinum neurotoxin serotype A (BoNT-A) injections are an effective therapeutic option since they denervate overactive muscles, thereby reducing hyperkinesis or synkinesis and eventually improving asymmetry. This narrative review summarizes relevant literature on the use of BoNT-A for facial paralysis. It provides a summary of dosing strategies and treatment plans, discusses the use of functional scales for assessing

Diagnostic Pitfalls in Adult-Onset Spinal Muscular Atrophy Type 4: When Incidental Lumbar Stenosis Confounds the Clinical Picture.

Spinal muscular atrophy type 4 (SMA‑4) represents a rare, late‑onset motor neuronopathy with insidious proximal limb weakness and preserved walking ability. Its slow progression and vague initial symptoms often cause years of diagnostic uncertainty. We describe a 38‑year‑old man who experienced slowly worsening, fatigue‑related proximal leg weakness over three years. Initial suspicion fell on a metabolic myopathy, and later, lumbar spine magnetic resonance imaging (MRI) showed multilevel degenerative changes with canal stenosis, findings that were initially attributed to the cause but did not

Improved Motor Neuron Preservation and Axonal Recovery Following Experimental Sciatic Nerve Repair With Heterologous Fibrin Biopolymer.

Peripheral nerve injuries severely impair motor function. Although neurorrhaphy is the gold-standard treatment, recovery is often incomplete. The heterologous fibrin biopolymer (HFB), a nonhuman-derived and biocompatible sealant, has shown regenerative potential in neural tissues, but its effects on the spinal cord microenvironment after peripheral nerve injury remain unclear. This study evaluated the chronic regenerative effects of HFB combined with neurorrhaphy after sciatic neurotmesis in rats. Forty adult male Wistar rats were divided into four groups: control, denervated, neurorrhaphy, an

A Rare Case of Transepiphyseal Distal Femur Fracture Dislocation With Delayed Recovery of a Common Peroneal Nerve Injury.

The aim of this study is to bring attention to a unique case and our approach to treatment in this context. We describe a case of an 11-year-old male who presented to us with an injury to his left knee following trauma with pain, swelling, shortening and deformity for one day. An X-ray revealed a transepiphyseal fracture dislocation of the left distal femur (Salter-Harris type 1 injury) and neurovascular examination was conclusive of foot drop which pointed towards injury to common peroneal nerve (CPN). The patient was taken up for closed reduction with percutaneous pinning under mobile C-arm

Chitosan Nerve Grafts Incorporated with SKP-SC-EVs Induce Peripheral Nerve Regeneration.

BACKGROUND: Repair of long-distance peripheral nerve defects remains an important clinical problem. Nerve grafts incorporated with extracellular vesicles (EVs) from various cell types have been developed to bridge peripheral nerve defects. In our previous research, EVs obtained from skin-derived precursor Schwann cells (SKP-SC-EVs) were demonstrated to promote neurite outgrowth in cultured cells and facilitate nerve regeneration in animal studies. METHODS: To further assess the functions of SKP-SC-EVs in nerve repair, we incorporated SKP-SC-EVs and Matrigel into chitosan nerve conduits (EV-NG

Aligned Graphene Mesh-Supported Double Network Natural Hydrogel Conduit Loaded with Netrin-1 for Peripheral Nerve Regeneration.

The gold standard treatment for peripheral nerve injuries (PNIs) is the autologous graft, while it is associated with the shortage of donors and results in major complications. In the present study, we engineer a graphene mesh-supported double-network (DN) hydrogel scaffold, loaded with netrin-1. Natural alginate and gelatin-methacryloyl entangled hydrogel that is synthesized via fast exchange of ions and ultraviolet irradiation provide proper mechanical strength and excellent biocompatibility and can also serve as a reservoir for netrin-1. Meanwhile, the graphene mesh can promote the prolifer

Sensory neuropathy in X-linked recessive bulbospinal neuronopathy.

X-linked recessive bulbospinal neuronopathy (X-BSN) is an adult-onset spinal and bulbar amyotrophy. Neurophysiologic studies demonstrate subclinical involvement of sensory nerves with diminished or absent sensory nerve action potientials and denervation changes, indicating the involvement of sensory neurons. We report the clinical features, findings of electrophysiologic study, and results of morphometric analysis of sural nerve pathology in a patient with X-BSN. Molecular genetic studies were also performed in the patient and his three daughters. Electrophysiologic studies revealed decreased

Treatment of Breast Animation Deformity in Implant-Based Reconstruction with Selective Nerve Ablation.

Breast animation deformity is a known complication of subpectoral implant placement that is usually corrected by repositioning the implant to the prepectoral position. Other less common treatment options include performing the muscle splitting biplanar technique, triple plane technique, neuromodulator injections, and secondary neurotomies via transection of the pectoral muscle. We report a patient with animation deformity successfully treated with direct identification and ablation of the medial and lateral pectoral nerves using selective bipolar electrocautery. The patient is a woman with a h

Impact of different types of facial nerve reconstruction on the recovery of motor function: an experimental study in adult rats.

OBJECTIVE: Poor functional recovery after facial nerve reconstruction is characterized by mass movements and synkinesis. Major reasons are axonal sprouting from the regenerating axons leading to misdirected reinnervation and hyperinnervation as well as polyinnervation of the mimic muscle end plates. We analyzed whether or not the type of nerve reconstruction influenced these pathological phenomena. METHODS: The experiments were performed on 48 adult rats divided into four groups. One group served as an intact control and the experimental groups were subjected to facial-facial nerve repair (FF

Cervical dystonia: disease profile and clinical management.

Cervical dystonia, the most common focal dystonia, frequently results in cervical pain and disability as well as impairments affecting postural control. The predominant treatment for cervical dystonia is provided by physicians, and treatment can vary from pharmacological to surgical. Little literature examining more conservative approaches, such as physical therapy, exists. This article reviews the etiology and pathophysiology of the disease as well as medical and physical therapist management for people with cervical dystonia.

Early development of SI cortical barrel subfield representation of forelimb in normal and deafferented neonatal rat as delineated by peroxidase conjugated lectin, peanut agglutinin (PNA).

Development of the barrel field representation of the forelimb in the primary somatosensory cortex (SI) was studied in normal and deafferented neonatal rat pups by means of the peroxidase conjugated lectin peanut agglutinin (PNA), which most likely binds to radial glial cells within barrel boundaries. 1. Alterations in lectin binding were seen in animals sacrificed on postnatal day 8 (PND-8) if deafferentation took place on PND-1 (day of birth) through PND-6. 2. Deafferentation on PND-5 or on PND-6 had the least effect on lectin binding. In these animals, lectin binding was reduced, although t

On the nature of the oscillations of the membrane potential (slow waves) produced by acetylcholine or carbachol in intestinal smooth muscle.

1. Intracellular recording was made with glass micro-electrodes from cells of the longitudinal muscle of the guinea-pig ileum in isotonic and in hypertonic solution.2. In isotonic solution spontaneous bursts of electrical activity occurred; these consisted of a slow potential component which carried a burst of spike action potentials. Acetylcholine increased the size (and the frequency) of the slow potential component. This had the effect of first reducing and then abolishing the spike potentials; continuous slow wave activity was thus produced. Slow waves were about 1 sec in duration and up t

Animal models of degenerative neurological disease.

Amyotrophic lateral sclerosis (ALS), Parkinson's disease (PD), and Alzheimer's disease (AD) are human neurological disorders which occur in middle and late life. These three diseases share certain features: they are slowly progressive; transmitter-specific groups of neurons are selectively affected by disease processes; and affected nerve cells exhibit cytoskeletal pathology. The causes and mechanisms of cell injury are unknown, and there are no treatments which directly affect the disease process. Dysfunction and death of these specific cell groups account for different clinical syndromes. In

Role of Hypoxia-Inducible Factor-1α in Regulating Muscle Degeneration After Rotator Cuff Tears.

BACKGROUND: Secondary muscle degeneration after a rotator cuff tear (RCT) critically affects clinical outcomes. Vascular compromise after a tendon injury creates a complex microenvironment that may be associated with the degeneration of rotator cuff muscle. The role of hypoxia-inducible factor-1α (HIF-1α), a master regulator of cellular stress responses to hypoxia, in modulating muscle abnormalities after an RCT remains undefined. PURPOSE: To define the role of HIF-1α in stem cell differentiation and muscle degeneration after an RCT in a murine model. STUDY DESIGN: Controlled laboratory stud

Regenerative peripheral nerve interface following forequarter amputation: a case report.

BACKGROUND: Forequarter amputation is a radical surgical procedure for malignant bone and soft tissue tumors involving the shoulder girdle, typically indicated for cases where limb salvage is not feasible due to extensive local invasion. However, post-amputation neuroma and phantom limb pain are major complications, occurring in 80%-90% of cases and severely impairing patients' quality of life. Traditional nerve management techniques often fail to prevent recurrent neuroma formation. The Regenerative Peripheral Nerve Interface (RPNI) is an emerging technique designed to guide axonal regenerati
