The HELLP syndrome is a severe and life-threatening complication of preeclampsia with typical laboratory findings. The frequency of the disease is 1 to 150-300 live births in perinatal centers. Forty-six hydropic infants with homozygous alpha-thalassaemia born during a period of 10 years have been reviewed. Measurement of free thyroxin (FT4) by a recently introduced commercial assay (Amerlex Free T4 RIA) was compared with the calculated free thyroxin index (FT4I) for serum from 104 postpartum women. In two experiments with Holstein cows effects of dietary supplementation of niacin to periparturient cattle on subsequent milk production and ketosis were examined. In Experiment 1, 20 cows were assigned to two groups. The treatment group received niacin at 6 g per head per day for 2 wk prepartum and 12 g per head per day for 4 wk postpartum. The high perinatal mortality among infants of women transferred from general practitioner to consultant care is well known, the implication being that if these women had been booked for consultant care several of the deaths might not have occurred. The findings of this study indicate that booking into a consultant unit did not reduce perinatal mortality and suggest that, where liaison between hospital and general practitioners is good and transfer easy, booking into a general practitioner maternity unit can be as safe for low-risk women as booking into a consultant unit.

Demographic, obstetric, clinical features and clinical outcome of 100 consecutive admissions to a psychiatric mother and baby unit are presented.
