Comparison of radiographic stress views for scapholunate dynamic instability in a cadaver model.

PURPOSE: Many different stress views for the diagnosis of scapholunate (SL) instability have been described in the literature. The purpose of this study is to compare these stress views and determine which view has the greatest utility for demonstrating SL gap radiographically. METHODS: We performed a literature search for articles describing SL radiographic stress views. We created SL instability in 9 cadaveric wrists by ligamentous sectioning and imaged each specimen using all radiographic views found in the literature. These included the "clenched pencil" view, clenched fist views in varyi

[Use of MRI for diagnosing scaphoid fracture].

BACKGROUND: Oslo accident and emergency clinic (Oslo skadelegevakt) treats most scaphoid fractures in Oslo. The objectives of this study were to assess the usefulness of MRI in diagnosing such fractures and to determine the number of scaphoid fractures treated at our clinic, the number of patients treated with a scaphoid cast without having a fracture, the proportion of these fractures diagnosed with conventional radiographs and that diagnosed with MRI in a given period. MATERIAL AND METHODS: The article is based on a review of medical records from all patients who had taken scaphoid radiogra

[Cast immobilisation for suspected scaphoid fractures].

BACKGROUND: Patients with suspected scaphoid fractures usually have their arm immobilised in a cast for two weeks, even in the absence of radiographic signs of fracture. After two weeks a new radiographic examination is performed. Our objective was to assess the extent of unnecessary cast immobilisation and to describe existing diagnostic practice in our hospital. MATERIAL AND METHODS: We performed a retrospective study to identify all patients treated for suspected scaphoid fracture in our hospital in the period 2001 - 2006. The extent of unnecessary immobilisation and the use of diagnostic

Long-term results of midcarpal arthrodesis in the treatment of scaphoid nonunion advanced collapse (SNAC-Wrist) and scapholunate advanced collapse (SLAC-Wrist).

UNLABELLED: Outcome evaluation of midcarpal arthrodesis in the treatment of scaphoid nonunion advanced collapse (SNAC-wrist) and scapholunate advanced collapse (SLAC-wrist). PURPOSE: Scaphoid nonunion or scapholunate ligament instability results in carpal collapse and subsequent arthrosis. These conditions, termed SLAC-wrist and SNAC-wrist, are the most common patterns of arthrosis in the wrist. The purpose of this retrospective study was the evaluation of functional outcomes following midcarpal arthrodesis and of patients' satisfaction with these outcomes. METHODS: Forty-nine patients were

[Scapho-lunate ligament reconstruction with Mitek suture anchors. Initial clinical results].

The scapholunate dissociation is an often misjudged lesion of carpal ligaments as clinical and radiological signs often don't allow a clear interpretation. Apart from chronical courses with increasing arthrosis the consequences are limited and more difficult operations. The application of Mitek suture anchors is a new and simple supplement to large-scale operative reconstructions of ligaments or partial arthrodesis. Of nine reconstructions of the scapholunate interosseous ligament using this anchor method so far only one relaps occurred.

Segmental radius and ulna fractures with scaphocapitate fractures and bilateral multiple epiphyseal fractures.

Segmental forearm fractures are rare in children, and management is controversial. Epiphyseal injuries further complicate matters. We report the case of a 15-year-old boy who had segmental radius and ulna fractures with a coronal split of a metaphyseal fragment, along with bilateral epiphyseal fractures of the distal radius and ulna as well as ipsilateral scaphocapitate fractures with perilunate dislocation. There was also a contralateral fracture through the radial neck. The patient underwent immediate internal fixation of the forearm fractures and delayed fixation of the scaphocapitate fract

Current innovations in wrist arthroscopy.

It has become clear that the stability of the scapholunate joint does not depend wholly on the scapholunate interosseous ligament, but rather on both primary and secondary stabilizers, which form a scapholunate ligament complex. Each case of scapholunate instability is unique and should be treated with tissue-specific repairs, which may partly explain why one procedure cannot successfully restore joint stability in every case. Wrist arthroscopy has a pivotal role in both the assessment and treatment of the scapholunate ligament complex derangements. Tears of the foveal attachment of the triang

[Volar transcaphocapitate perilunate disclocation of the carpus: a new variety, a new approach].

On the one hand, surgical treatment of dorsal perilunate fracture-dislocation is now well codified. The posterior approach is preferred by the majority of the authors. On the other hand, surgical schedule for volar perilunate dislocation is not well establish in the literature. The anterior approach is the most frequently recommended. In case of Fenton's syndrome (scaphocapitate fracture), the head of the capitatum is described with a rotation of 180 degrees. The authors reports a case of a volar transcaphocapitate perilunate fracture-dislocation of the carpus in a polytrauma. The head of the

Operative Treatment of an Isolated, Comminuted, Biarticular Trapezium Fracture: A Case Report and Review of the Literature.

BACKGROUND: Isolated, highly comminuted trapezium fractures are extremely rare. While they can be initially diagnosed with plain radiographs … ra-articular congruity of the trapezium with both the base of the first metacarpal distally and the scaphoid bone proximally ensured a favorable outcome. LITERATURE REVIEW: Various operative techniques are presented in current literature, encompassing ORIF with screws and K-wires, closed reduction and percutaneous fixation with K-wires, mini-external fixation, …

[Experimental studies of the biomechanics of scaphoid screws].

The purpose of this study was to determine the anchoring forces of four commonly used scaphoid screws in synthetic polyurethane foam. A linear relation was found between the length of the probe and the anchoring forces. The leading thread of the Herbert screw showed a anchoring force of 54 N/mm, the trailing thread 80 N/mm, and the Ulrich screw 75 N/mm. This linear relation of the shaft screws was only relevant upto a border probe length no longer that of the thread. Furthermore, it is valid for screws with continuous threads up to a probe length of 12 mm respectively. The cortical screw devel

Scaphocapitate fusion in stage III Kienböck's disease: effects of lunarectomy on postoperative pain and function.

BACKGROUND: Kienböck's disease (KD) is a progressive condition characterized by avascular necrosis of the lunate, leading to carpal collapse and wrist dysfunction. Scaphocapitate fusion (SCF) is a salvage procedure often used in advanced stages of KD to stabilize the wrist and alleviate symptoms. This study aimed to evaluate the clinical and radiological outcomes of SCF in stage III KD, with a specific focus on the impact of lunarectomy on postoperative pain and function. MATERIALS AND METHODS: This retrospective study included 25 patients with stage III KD who received SCF treatment and were

Robot-assisted percutaneous scaphoid fixation: patient-reported outcomes and learning curve at two centres.

This study describes patient-reported outcomes of robot-assisted percutaneous scaphoid fracture fixation of 62 patients from two medical centres and the learning curve of this new technique. One attempt to place the guidewire was sufficient in 97% of cases. All fractures achieved radiographic union at a mean of 9 weeks. There were no complications observed. At a mean follow-up of 36 months (range 12-68 months), the mean patient-rated wrist evaluation (PRWE) was 2 (range 0-22) and the mean Mayo Wrist Score was 96 (range 70-100). After the initial ten to 20 cases, the learning phase was reasonab

Volar locking and angular stability plate for treatment of comminuted scaphoid fractures: a case series of 44 cases.

BACKGROUND: The primary method employed worldwide for the treatment of scaphoid fractures is screw fixation. However, in unstable and comminuted fractures, percutaneous fixation could produce complications due to technical challenges, such as improper axis positioning, inaccurate screw length measurement, intra-articular screw penetration, and impingement. Alternative open approaches for the surgical management of scaphoid fractures have been proposed, and in recent years, a new specific volar locking plate for the treatment of scaphoid fractures has been developed. This study aims to present

Incidental Flexor Carpi Radialis Tendinopathy on Magnetic Resonance Imaging.

Background: Wrist pain is often nonspecific. Magnetic resonance imaging (MRI) is regularly obtained to evaluate wrist pain. Variations and pathophysiology identified on MRI may not account for patient's clinical symptoms. This study aims to quantify the prevalence of flexor carpi radialis (FCR) tendinopathy on MRI and the coexistence of trapeziometacarpal (TMC) or scaphotrapeziotrapezoid (STT) osteoarthritis. Methods: Using an institutional research database, we identified 3631 adult patients who obtained an MRI of the wrist during a 15-year period. Text search in the radiology reports identif

Does the DISI matter after distal scaphoidectomy with tendon interposition for STT osteoarthritis?

Progression to dorsal extension of the lunate after distal scaphoidectomy was described more than a decade ago. Still, this technique remains a popular choice for surgical treatment of isolated scaphotrapeziotrapezoid osteoarthritis (STT OA). This study aimed to investigate short-term postoperative function, patient satisfaction and radiographic outcomes of distal scaphoidectomy with tendon interposition for isolated STT OA in the wrist. Scaphoid resection width, amount of DISI and postoperative complications were also assessed. We evaluated all distal scaphoidectomies done at our hospital fro
