Instability of the metacarpophalangeal joint of the thumb. Reconstruction of the collateral ligaments using the extensor pollicis brevis tendon.

Reconstruction of the collateral ligaments of the metacarpophalangeal joint of the thumb was performed for fourteen ruptures of the ulnar and two ruptures of the radial collateral ligament, one to eighteen months after injury. The extensor pollicis brevis tendon was used to reinforce the resutured ligament. Symptoms were relieved in all cases after follow-ups ranging from one to seven years, even in two thumbs with mild degenerative arthritis of the metacarpophalangeal joint.

[Treatment of flexion-pronation contracture of the wrist in hemiplegic children].

Thirty-three cases of infantile spastic hemiplegia have been treated surgically for wrist deformity in flexion, pronation and ulnar deviation. The authors used Green's technique in most of the cases (transplantation of flexor carpi ulnaris to the extensores carpi) either in isolation or in association with lengthening of the pronator teres. The indications for surgery are given and the details of surgical technique and postoperative care are described. The complications are described and the factors affecting prognosis are analysed. Half of the cases had good or excellent results; five were fa

Chronic Longitudinal Instability of the Forearm Treated With a Combination of Ulnar Shortening Osteotomy, Pronator Teres Transfer, and Tightrope Technique.

Restoration of longitudinal stability of the forearm continues to pose a difficult challenge for the hand surgeon, and no technique has demonstrated success above others. Longitudinal stability to the forearm is conferred by 3 structures: the radial head, which acts as a primary stabilizer, the interosseous membrane, more specifically, the central band and, the distal radioulnar ligaments which are part of the triangular fibrocartilage complex. A combination of techniques is described in this article to address chronic longitudinal instability of the forearm: (1) ulnar shortening osteotomy to

Helical sub-structures in energy-storing tendons provide a possible mechanism for efficient energy storage and return.

The predominant function of tendons is to position the limb during locomotion. Specific tendons also act as energy stores. Energy-storing (ES) tendons are prone to injury, the incidence of which increases with age. This is likely related to their function; ES tendons are exposed to higher strains and require a greater ability to recoil than positional tendons. The specialized properties of ES tendons are thought to be achieved through structural and compositional differences. However, little is known about structure-function relationships in tendons. This study uses fascicles from the equine s

Pectoralis major to scapula transfer for patients with serratus anterior palsy.

Serratus anterior palsy can be a very disabling condition. However, not much has been written about many other upper limb palsies and the surgical approaches for them. In contrast, not much has been published about this particular muscle palsy. Most investigators engaged in the treatment of peripheral nerve palsies are focused on the restoration of elbow flexion/extension and shoulder abduction/external rotation. Nevertheless, scapulothoracic stability is of utmost importance for shoulder function inasmuch as it offers a stable base for arm motion--mostly forward flexion. As a result, serratus

Olecranon fractures.

Several options exist for the management olecranon fractures. These include tension band, plate and intramedullary fixation techniques as well as fragment excision with triceps advancement and non-operative management. No one technique is suitable for the management of all olecranon fractures. In deciding how to treat this common trauma presentation, the surgeon needs a good understanding of the anatomy, different fracture morphologies, surgical options and potential complications. With appropriate management and early mobilisation good functional results can be expected in the majority of pat

Restoration of thumb opposition by transposing the flexsor pollicis brevis muscle: thirteen-year clinical application.

BACKGROUND: To repair late median nerve injury, many methods have been used in the past years. The aim of this study was to review a thirteen-year experience in restoration of thumb opposition by transposing flexor pollicis brevis muscle. METHODS: From July 1992 to August 2005, 63 patients without thumb opposition because of late median never injury were treated by transposing the flexor pollicis brevis muscle. All the patients had received primary nerve repair after the jnjury. The interval between the injury and the second operation was (1.87 +/- 2.31) years (6 months to 4.2 years). The pat

[Chronic instability after posterior cruciate ligament injury. Tactics, techniques, and results].

This article reviews the surgical treatment of chronic posterior knee instability. The treatment rationale includes exact definition of the instability pattern ("envelope-of-motion" of the tibia) by clinical examination, arthrometry and stress radiography. Exact evaluation of the osseous anatomy is mandatory to identify an eventual varus morphotype. This osseous variant in combination with posterior/posterolateral instability should be treated by an osteotomy in every case. The technique of additive osteotomy to correct varus and increase the sagittal tilt of the proximal tibia is described. L

Interposition of the split flexor hallucis longus tendon after resection of a coalition of the middle facet of the talocalcaneal joint.

Ten consecutive patients (fourteen feet) who had a painful coalition of the middle facet of the talocalcaneal joint with restricted motion of that joint were managed with a resection of the coalition and interposition of a split flexor hallucis longus tendon in 1992, 1993, or 1994. Initial nonoperative treatment of all of the feet had failed. According to the ankle-hindfoot clinical rating system of the American Orthopaedic Foot and Ankle Society, there were eleven excellent results and one good, one fair, and one poor result at a mean of fifty-one months (range, thirty-two to sixty months) af

[Arthroscopic notch-plasty in persistent extensor deficit following anterior cruciate ligament-plasty].

Occasionally following ACL reconstruction, an extension deficit persists, because of impingement of the transplant in the notch. In six cases an arthroscopic notchplasty was performed, five of these cases were immediately successful. Due to initial insufficient resection of the notch, one patient required a second notchplasty to achieve full extension.

Transfer of upper pectoralis major flap for functional reconstruction of deltoid muscle.

Since January 1984, functional reconstruction of the deltoid muscle using transfer of the upper part of the pectoralis major muscle has been performed in 7 patients with deltoid dysfunction. 5-20 months follow-up (average 11) revealed marked improvement in 6 patients. During the operation, the superior and inferior parts of the pectoralis major muscle were bluntly dissected and sectioned. The upper part of this muscle was turned over and transferred to the site normally occupied by the deltoid muscle. The transposed muscle contributed to both abduction and flexion of the shoulder. The efficacy

[Surgical treatment of arthritis of the thumb saddle joint].

First the etiology, clinical signs and differential diagnosis of the basal joint of the thumb are represented. Then a short summary is given about the possibilities of conservative and operative therapy. In operative treatment, there are techniques of maintaining the flexibility of this joint as well as the arthrodesis. In our clinic we prefer operative techniques by removal of the multangulum majus or implantation of an artificial joint (Francobal-endoprosthesis). The results of our 22 patients, who had an operation of an arthritis of the basal joint of the thumb were controlled clinically an

The Grice procedure: indications and evaluation of long-term results.

The major indication for the Grice extra-articular subtalar arthrodesis is valgus deformity occurring after poliomyelitis. Unsatisfactory long-term results in valgus deformities secondary to other neuromuscular conditions, myelodysplasia and flexible flat feet would indicate that subtalar arthrodesis is not appropriate in these conditions. The Grice procedure had limited success in patients with cerebral palsy, but only in those mildly afflicted with hemiplegia. With the eclipse of poliomyelitis by other neuromuscular conditions, the extra-articular would appear to have limited indications in

Secondary reconstruction of anterior cruciate ligament in athletes by using the semitendinosus tendon. Preliminary report of 78 cases.

Secondary reconstruction of the torn anterior cruciate ligament has been accomplished by semitendinosus tenodesis in 78 athletes (average age, 20 years; range, 13 to 45 years old). This series was limited to patients who had isolated tears of the ligament or tears of the anterior cruciate ligament associated with tears of one or both menisci. Meniscectomies were performed in 71 knees and reefing of the posteromedial capsular ligament in 35 knees. After an average of 11 months of follow-up (range, 6 to 29 months postoperatively), 67 (86%) of the patients have obtained significant improvement (0

Adverse effects of adenovirus-mediated gene transfer of human transforming growth factor beta 1 into rabbit knees.

To examine the effect of transforming growth factor (TGF)-beta1 on the regulation of cartilage synthesis and other articular pathologies, we used adenovirus-mediated intra-articular gene transfer of TGF-beta1 to both naïve and arthritic rabbit knee joints. Increasing doses of adenoviral vector expressing TGF-beta1 were injected into normal and antigen-induced arthritis rabbit knee joints through the patellar tendon, with the same doses of an adenoviral vector expressing luciferase injected into the contralateral knees as the control. Intra-articular injection of adenoviral vector expressing TG

The advantages of free tissue transfer in the treatment of posttraumatic stiff knee.

BACKGROUND: Open fractures of the distal femur involving the joint, surrounding ligament, and soft tissues are among the worst types of injuries that may eventually lead to stiff knee. Release procedures have been described as the common treatment option for posttraumatic stiff knee. However, the importance of simultaneously applied free flaps to replace the pliable soft tissue around the knee joint has not been discussed previously. METHODS: Between 1996 and 2002, nine patients with posttraumatic severe stiff knees were operated on. All patients underwent release procedures, such as scar rem
