Structural odontoid lesions in craniovertebral tuberculosis: a review of 15 cases.

STUDY DESIGN: A retrospective chart review. OBJECTIVE: To describe the presentation and the rationale for management of pathological odontoid fracture and complete odontoid destruction in craniovertebral junction tuberculosis (CVJ TB). SUMMARY OF BACKGROUND DATA: Presentation of CVJ TB ranges from minor osteomyelitic changes to severe structural damage leading to instability. Structural damage to the odontoid process is poorly characterized in the literature. Inadequate knowledge about the radiological presentations has led to controversy in the management of CVJ TB. METHODS: The cohort con

Multifocal musculoskeletal tuberculosis mimicking multiple bone metastases: a case report.

BACKGROUND: The occurrence of non-contiguous, multiple, and remote involvement tuberculous spondylitis is rare. The clinical presentation in patients with multifocal musculoskeletal tuberculosis may closely mimic that in patients with multiple bone metastases, which makes the accurate clinical diagnosis challenging. Herein, we report a multifocal musculoskeletal tuberculosis case that was misdiagnosed for 8 months as multiple bone metastases. CASE PRESENTATION: A 63-year-old male farmer of Chinese Han ethnicity presented to us with pain in left side of the neck, right side of the chest and th

A case of skeletal tuberculosis and psoas abscess: disease activity evaluated using (18) F-fluorodeoxyglucose positron emission tomography-computed tomography.

BACKGROUND: Psoas abscess complicating tuberculous spondylitis is a rare morbidity in extrapulmonary tuberculosis. There are no established guidelines for evaluating the clinical response of psoas abscess. Although several studies have shown that positron emission tomography-computed tomography with 18 F-fluorodeoxyglucose can play a potential role in diagnosing multifocal tuberculosis and monitoring the clinical response of pulmonary tuberculosis, to our knowledge, this is the first report demonstrating that positron emission tomography-computed tomography is useful for evaluating local infla

The surgical treatment and related management for post-tubercular kyphotic deformity of the cervical spine or the cervico-thoracic spine.

PURPOSE: The outcomes of surgical treatment and related complications of post-tubercular kyphotic (PTK) deformity of the cervical spine or the cervico-thoracic spine were evaluated. METHODS: From January 2005 to October 2010, 12 cases with PTK (7 males, 5 females) with an average age of 30 years (range 21-43 years) formed the study group. There were ten patients with cervical deformities and two with cervico-thoracic kyphosis. Neurological function of all the patients was evaluated by the Japanese Orthopaedic Association (JOA) score and visual analogue scale (VAS) score. Two patients with sev

Clinical and radiographic evaluation of posterior surgical correction for the treatment of moderate to severe post-tuberculosis kyphosis in 36 cases with a minimum 2-year follow-up.

OBJECT: The object of this study was to compare the clinical and radiographic outcomes of 36 patients with posttuberculosis kyphosis who underwent one of two types of osteotomy. METHODS: Each patient underwent single-stage correction via a posterior surgical approach. A modified pedicle subtraction osteotomy (mPSO) was performed when the kyphotic deformity was less than 70° (7 cases), whereas a posterior vertebral column resection (VCR) was performed when the kyphotic deformity exceeded 70° (29 cases). Full-length standing radiographs were obtained before surgery and at follow-up visits. Thes

One-stage surgical management for thoracic tuberculosis by anterior debridement, decompression and autogenous rib grafts, and instrumentation.

BACKGROUND CONTEXT: Patients with thoracic tuberculosis have been not always successfully treated by radical debridement and bone grafting with or without supplementary posterior instrumentation and fusion, although most surgeons use posterior instrumentation to support anterior strut grafts. PURPOSE: To determine the efficacy of anterior instrumentation after radical debridement and autogenous rib grafts in patients with thoracic tuberculosis over a 6-year period at a single institution. STUDY DESIGN: A retrospective clinical study of one-stage surgical management for thoracic tuberculosis

Transpedicular decancellation osteotomy in the treatment of posttuberculous kyphosis.

OBJECTIVE: Although the transpedicular decancellation osteotomy is a salvage technique for reconstruction of complex spinal deformities, it is not a procedure used exclusively for patients with kyphosis occurring as a sequel of treated tuberculosis. In this study, 16 adult patients with kyphosis underwent transpedicular decancellation osteotomy between 1993 and 1999. Pain, kyphosis angle, sagittal balance, and functional and neurologic status were the main parameters used for the clinical and radiologic assessment. METHODS: Sixteen patients with angular kyphotic deformity underwent transpedic

Easy drainage of presacral abscess.

Presacral region is one of the difficult regions for drainage of abscess as the approaches described for this carry a lot of morbidities. Transpedicular approach is described for the drainage of presacral abscess. Further, tuberculous abscess as a cause of cauda equina syndrome is reported. Patient had complete neurological recovery within weeks of drainage of abscess. Transpedicular drainage of presacral abscess is a safer option for recently developed neurological deficit. Late presentation may need anterior approach, which is associated with increased morbidity.

An unusual spinal arachnoiditis.

Cryptococcal spinal arachnoiditis occurs in patients with meningitis and usually when they are immunocompromised. Spinal symptoms in cryptococcosis are rare and a very exceptional entity in the immunocompetent population. We present a young immunocompetent male who developed progressively increasing paraparesis due to primary cryptococcal arachnoiditis, who showed significant improvement after antifungal therapy. Although extremely rare, spinal arachnoiditis in an immunocompetent individual can be caused due to cryptococcus, as in our case. This case illustrates and emphasizes the necessity fo

Late onset Pott's paraplegia.

BACKGROUND: Pott's disease may cause late neurological involvement due to development of sharp kyphosis. Anterior decompression and fusion is the treatment of choice for this disorder. OBJECTIVE: To determine the mid-term clinical results of patients with late onset Pott's paraplegia, who underwent anterior decompression and grafting after neurological deterioration. SETTING: A university hospital in Istanbul, Turkey. METHODS: Eight patients who developed late onset paraplegia with a mean period of 24.6 years (range, 9-46 years) after the active disease were treated with anterior decompress

[Possibilities of reducing the duration of the treatment of patients with tuberculous spondylitis].

In the article are described the results of operative treatment of 185 patients with tuberculous spondylitis. In order to restore stability there was carried out posterior spondylidesis after Genle with removal o ffoci by means of posterior access in 70 patients. 115 patients were subjected to anterolateral spondylidesis, 55 of them--by generally accepted method and 60--with creation of trapezoid lock between the body of a vertebra and transplants. In this group of patients was used removable jacket made of polivike before the patient's elevation. In this case the term of the stationary treatm

Intracranial tuberculomas: the Hofuf, Saudi Arabia experience.

Tuberculosis usually involves the brain through haematogenous spread. The mass lesion caused by tuberculosis in the brain is called tuberculoma, which is a conglomerate of tubercles. Tuberculomas may also be found in the spinal cord. Because of their slow growth they often become calcified. The study was conducted at the King Fahad Hospital, Hofuf, Al Hassia, Saudi Arabia between 1992 and 1998. It was a prospective study of all patients which had intracrainal mass lesions which showed typical ring-enhancement on brain CT scan with contrast. Twenty patients satisfied inclusion into the study. O

Anterior approach to the cervicothoracic junction without sternotomy: a report of 37 cases.

STUDY DESIGN: Description of a new anterior approach to the cervicothoracic junction and retrospective study of results. OBJECTIVE: To propose a less invasive anterior approach and report the preliminary results of this technique. SUMMARY OF BACKGROUND DATA: Different partial sternotomies with or without associated clavicular resection have been proposed but expose patients to the risk of pseudarthrosis. A less invasive technique should reduce the morbidity of this surgery. METHODS: From 2000 to 2004, 37 patients were operated between T1 and T4. Whenever possible, access was limited to a le

Tuberculosis in children undergoing hemodialysis.

UNLABELLED: Tuberculosis (TB) remains a public health problem in Tunisia. Its incidence is higher in immunocompromised hosts than in the general population. In children and during hemodialysis, TB is characterized by the frequency of extrapulmonary localizations and diagnostic difficulties. The aim of this retrospective study is to evaluate the incidence of TB in Tunisian children undergoing hemodialysis and to determine its clinical features as well as the results of chemotherapy. METHOD: This retrospective study includes seven TB children among 112 children on hemodialysis at the pediatric
