Ectopia lentis may cause a reduction in visual acuity, which varies with the type and degree of dislocation and the presence of other ocular abnormalities. Retinoscopy may reveal a significant refractive error, usually myopia and astigmatism. Occasionally, an accurate refraction may be difficult because of tilting or dislocation of the lens. If aphakic refraction improves visual acuity, an aphakic correction should be considered.

We report 13 cases of corneal ulcers caused by gram-negative bacteria. These cases occurred in aphakic eyes wearing hydrophilic contact lenses on an extended-wear basis. The risks for infection in the elderly aphakic eye with extended-wear hydrophilic lenses are increased by delay in removal of the lens. Caution in the use of these lenses is urged. The ability of either the patient or an attendant to remove the lens as soon as redness, diminished vision, or pain presents itself is essential in minimizing the risks for serious infection.

Corneal swelling was monitored for nine hours in 14 unilateral aphakic patients who wore +13.25-diopter extended-wear lenses. Sixty-five patients less than 10 years of age had cataract surgery on 99 eyes. The visual prognosis depended on the cataract type and whether the treatment was undertaken during the critical period for development of the fixation reflex. The penetration of a pseudophakic eye by a high-speed metallic foreign body is reported. The path of the steel missile was interrupted by the intraocular lens, which was fixed within the capsular remnant, preventing missile penetration into the posterior segment. Although the foreign body was removed from the anterior chamber without immediate sequelae, several months later a peripheral retinal detachment occurred. Prompt recognition and treatment resulted in the retention of excellent pseudophakic vision. The importance of postoperative protective eyewear is emphasized.
