Outcome of very severe birth asphyxia.

The aim of this study was to establish the outcome of very severe birth asphyxia in a group of babies intensively resuscitated at birth. 48 infants, born between 1966 and 1971 inclusive, were selected; 15 were apparently stillborn and 33 had not established spontaneous respirations by 20 minutes after birth. One-half of them died, but 3 to 7 years later three-quarters of the survivors are apparently normal. Later handicap was associated with factors leading to prolonged partial intrapartum asphyxia, while acute periods of more complete asphyxia were not necessarily harmful.

Fetal asphyxia due to umbilical cord compression. Metabolic and brain pathologic consequences.

Term monkey fetus 1620 sustained 50 min of rapidly developing severe asphyxia which began immediately after its in utero version. The arterial blood pO2 decreased from a normal value of 34 to 11-12 mm Hg while the blood pH fell from 7.35 to 6.70. During this asphyxia, hemoglobin-oxygen saturations below 5% were recorded. The complete collapse of the umbilical circulation several minutes prior to the reoxygenation of the fetus added an episode of total asphyxia. With reoxygenation following delivery, fetal cardiovascular performance improved rapidly though over an hour was required for recovery

Coagulation studies in preterm infants with respiratory distress and intracranial haemorrhage.

Coagulation studies were performed on 48 preterm infants with respiratory distress or recurrent apnoea who were considered to be at risk from intracranial haemorrhage. One or more coagulation abnormalities were detected in 16 cases though only one infant had a bleeding diathesis in life. Coagulation abnormalities were more frequent in infants subsequently found to have intracranial haemorrhage (intraventricular or subarachnoid haemorrhage) than in those with hyaline membrane disease only. Of 26 infants who died, 6 had hyaline membrane disease only, 17 had intracranial haemorrhage (usually intr

Validation of neurodevelopmental assessments for early detection of high-risk infants in Zimbabwe: protocol for the Child Development Observation study (ChiDO study).

INTRODUCTION: Appropriate neurodevelopmental assessment tools are often unavailable in low- and middle-income countries (LMICs) due to lack of validation, cost and cultural-linguistic challenges. This reduces the availability of high-quality developmental evaluation, resulting in clinical gaps that impact individual child outcomes and modifiable risk detection for prevention. The objective of this study is to validate three neurodevelopmental assessment tools for early detection in low-resource settings through recruitment of infants across the neurodevelopmental impairment (NDI) risk spectrum

Comparing the efficacy in reducing brain injury of different neuroprotective agents following neonatal hypoxia-ischemia in newborn rats: a multi-drug randomized controlled screening trial.

Intrapartum hypoxia-ischemia leading to neonatal encephalopathy (NE) results in significant neonatal mortality and morbidity worldwide, with > 85% of cases occurring in low- and middle-income countries (LMIC). Therapeutic hypothermia (HT) is currently the only available safe and effective treatment of HIE in high-income countries (HIC); however, it has shown limited safety or efficacy in LMIC. Therefore, other therapies are urgently required. We aimed to compare the treatment effects of putative neuroprotective drug candidates following neonatal hypoxic-ischemic (HI) brain injury in an establi

Chest Compressions during Sustained Inflations Improve Recovery When Compared to a 3:1 Compression:Ventilation Ratio during Cardiopulmonary Resuscitation in a Neonatal Porcine Model of Asphyxia.

BACKGROUND: Recently, sustained inflations (SI) during chest compression (CC) (CC+SI) have been suggested as an alternative to the current approach during neonatal resuscitation. No previous study compared CC+SI using CC rates of 90/min to the current 3:1 compression:ventilation ratio (C:V). OBJECTIVE: To determine whether CC+SI versus a 3:1 C:V reduces the time to the return of spontaneous circulation (ROSC) and improves hemodynamic recovery in newborn piglets with asphyxia-induced bradycardia. INTERVENTION AND MEASUREMENTS: Term newborn piglets were anesthetized, intubated, instrumented, a

Prevalence of systemic air-embolism after prolonged cardiopulmonary resuscitation in newborns: A pilot study.

BACKGROUND: Chest compressions (CC) during cardiopulmonary resuscitation (CPR) are the cornerstone of adult CPR protocols and are meant to restore circulation and improve outcome. Although adverse effects such as air-embolisms have been reported, these are rare and considered to be outweighed by beneficial effect. In newborns, however, the lung tissue is more fragile. Thus, the high intra-thoracic pressures resulting from CC may make the newborns more vulnerable for air-embolisms. OBJECTIVES: We studied the postmortem prevalence of air-embolism in neonates that have received CPR. METHODS: Pr

Scale-free bursting in human cortex following hypoxia at birth.

The human brain is fragile in the face of oxygen deprivation. Even a brief interruption of metabolic supply at birth challenges an otherwise healthy neonatal cortex, leading to a cascade of homeostatic responses. During recovery from hypoxia, cortical activity exhibits a period of highly irregular electrical fluctuations known as burst suppression. Here we show that these bursts have fractal properties, with power-law scaling of burst sizes across a remarkable 5 orders of magnitude and a scale-free relationship between burst sizes and durations. Although burst waveforms vary greatly, their ave

Poor adherence to neonatal resuscitation guidelines exposed; an observational study using camera surveillance at a tertiary hospital in Nepal.

BACKGROUND: Each year an estimated 10 million newborns require assistance to initiate breathing, and about 900 000 die due to intrapartum-related complications. Further research is required in several areas concerning neonatal resuscitation, particularly in settings with limited resources where the highest proportion of intrapartum-related deaths occur. The aim of this study is to use CCD-camera recordings to evaluate resuscitation routines at a tertiary hospital in Nepal. METHODS: CCD-cameras recorded the resuscitations taking place and CCD-observational record forms were completed for each

Cesarean section en caul and asphyxia in preterm infants.

We reviewed the outcome for 211 women undergoing a planned en caul (within intact membranes) cesarean section and for 836 control women with conventional lower segment section, in the period 2001-2010 at a university-affiliated hospital in China, where the former technique has been practiced. Of the intended en caul sections there were 141 successful deliveries (66.8%), and 70 that failed and were converted to conventional lower segment cesarean section. Maternal blood loss was similar for both operation types, but the rate of asphyxia was significantly lower among preterm infants delivered by
