Traumatic bone fractures are often debilitating injuries that may require surgical fixation to ensure sufficient healing. Currently, the most frequently used osteosynthesis materials are metal-based; however, in certain cases, such as complex comminuted osteoporotic fractures, they may not provide the best solution due to their rigid and non-customizable nature. In phalanx fractures in particular, metal plates have been shown to induce joint stiffness and soft tissue adhesions. A new osteosynthesis method using a light curable polymer composite has been developed. This method has demonstrated

BACKGROUND: To potentially limit peri-implant fractures our institution commonly implements a "stress-taper" fixation construct in which the screw lengths towards the proximal end of a construct are incrementally decreased, in order to avoid a focal stress-riser when loaded. To assess this construct, we asked: 1) Does the stress taper strategy increase torsional strength than the bicortical locking construct when biomechanically tested in a cadaveric femur model? 2) Does it fail in a less comminuted fracture pattern?
METHODS: Seven matched pairs of cadaveric femora were randomly assigned to o

Clavicle fracture is a common injury due to its subcutaneous and relatively anterior position. Fractures affecting the middle third account for majority of all clavicular fractures. Both non-operative and surgical methods have been described for the management of this injury. However, there is no uniform consensus on the definite choice of treatment. Hence, this study was undertaken to compare conservative approach with primary internal plate fixation in mid-shaft clavicular fractures in terms of subjective outcome, functional outcome, the rates of nonunion and malunion and other local complic

The Locking Compression Plate (LCP) and the Less Invasive Stabilisation System (LISS) are new implants with angular stability developed by the AO/ASIF. They offer an alternative for internal fixation in complex intra-articular fractures and high-energy diaphyseal fractures of the long bones. The new system was used in a prospective study to treat 113 patients (76 women, 37 men; average age: 53.1 years) with 122 injuries (multifragment shaft fractures or complex intra-articular, delayed/ non-union, malunion) from October 2002. Of these, 13 affected the humerus, 20 the radius, 2 the ulna, 21 the

Seventy-five adults who sustained 76 tibial plateau fractures were treated according to a prospective protocol using instability in extension as the principal indication for operative fixation. Patients showing instability underwent closed manipulative reduction under fluoroscopic guidance. If significant joint depression persisted after reduction, elevation of the fracture was performed either from below using bone punches through a cortical window or via limited arthrotomy. Iliac crest bone graft was used to buttress depressed fractures. Fixation was then secured using 7-mm cannulated screws

Fractures on the base of the first metacarpal are uncommon lesions, affecting young subjects and have major social repercussions (an average of 3.5 months off work). The authors have analysed the clinical and radiological results of a series of 138 recent fractures of the base of the first metacarpal with a mean follow-up of 7 years (12 month-14 years). They used a classification into 5 types (Bennett's with a large fragment = 22%, Bennett's with a small fragment = 20%, Rolando = 15%, extraarticular fractures = 36%, Comminuted fractures = 6%) which are easy to recognize on standard x-rays, or

BACKGROUND: The Chaput fragment, a bony avulsion of the anterolateral margin of the distal tibia, is a less commonly discussed fracture pattern in ankle injuries. Its significance in ankle fractures and the optimal fixation technique remains unclear due to limited literature. This study aims to describe the morphology of ankle fractures with Chaput fragment and introduce a new classification system.
MATERIALS AND METHODS: We retrospectively analyzed 33 patients with ankle fractures with associated Chaput fragment treated at our institute over a 3-year period. Data on patient demographics, fra

BACKGROUND: Fractures of the distal radius are the most common fractures in the upper extremity, and their incidence is increasing with the aging of the population. Despite anatomical reduction of the bones, many patients complain of residual pain. A reason for this may be ligament injury not addressed during surgery or conservative treatment. Radiographic measurements may allow assessment of ligament integrity but they may be population specific and differ among races.
OBJECTIVES: To assess radiographic wrist measurements in an Israeli population and to compare them to existing values.

OBJECTIVES: The goal of this study is to compare the fatigue strength of a locking intramedullary nail (LN) construct with a double locking plate (DLP) construct in comminuted proximal extra-articular tibia fractures.
METHODS: Eight pairs of fresh frozen cadaveric tibias with low bone mineral density [age: 80 ± 7 (SD) years, T-score: -2.3 ± 1.2] were used. One tibia from each pair was fixed with LN, whereas the contralateral side was fixed with DLP for complex extra-articular multifragmentary metaphyseal fractures (simulating OTA 41-A3.3). Specimens were cyclically loaded under compression si

Fractures of the proximal humerus are common and the treatment for both displaced and comminuted variants remains controversial. Treatment options initially consisted of closed reduction, traction, casting, and abduction splints. In the early 1930s, operative treatment for displaced fractures gained popularity, which continued in the 1940s and 1950s. Humeral head replacement for severely displaced fractures of the proximal humerus was introduced in the 1950s. In the 1970s, the Association for Osteosynthesis/Association for the Study of Internal Fixation popularized plates and screws for fractu

BACKGROUND: Comminuted radial head fractures are challenging to treat with open reduction and internal fixation. Radial head arthroplasty is a favourable technique for the treatment of complex radial head fractures. The purpose of this study was to evaluate the functional and radiological outcomes of radial head arthroplasty using modular pyrocarbon radial head prosthesis.
METHODS: We retrospectively reviewed 21 consecutive patients requiring radial head arthroplasty for unreconstructible radial head fractures between July 2003 and July 2009. Patients completed a Short Form-36 (SF-36), the Di

BACKGROUND: Supracondylar humeral fractures are the most common lesions in childhood. Severely displaced fractures are commonly treated by crossed K-wire osteosynthesis. Such stabilized fractures require a cast and often involve postoperative complications such as iatrogenic lesions of the n. ulnaris and secondary displacements, sometimes leading to a consecutive cubitus varus. This study analyzed possible advantages of elastic stabile intramedullary nailing (ESIN) with postoperative release of motion.
PATIENTS AND METHOD: All children aged 1-14 years suffering from a supracondylar humeral fra

BACKGROUND: The role of plate fixation in the management of fresh displaced midclavicular fractures is unsettled. The objective of this study was to evaluate the drawbacks and pitfalls of this treatment method.
METHODS: We analyzed the complications encountered in 103 consecutive adult patients with severely displaced fresh fractures of the middle third of the clavicle who were treated by open reduction and internal fixation using AO/ASIF plates. These 103 patients accounted for 9.5% of the 1,081 patients with fresh midclavicular fractures seen between 1989 and 1995. The mean age of the 103 p

In this report, we detail a 69-year-old female who sustained a comminuted intra-articular left distal radius fracture that failed to heal with bridge plate fixation. Given the patient's poor subchondral bone stock and refusal of bone autograft, we designed a construct using a dorsal spanning plate and an intramedullary fragment-specific plate as a volarly placed strut in combination with viable bone allograft and cancellous bone chips to treat this nonunion. This case demonstrates an option for distal radius non-union treatment and highlights the importance of ingenuity that orthopedic surgeon

CASE: A twenty-eight-year-old man presented with a Pipkin type-III femoral head fracture and open floating knee injury. Initially debridement was performed, during which the extensor apparatus could not be salvaged. The top priority in management of the floating knee was achievement of adequate soft-tissue coverage followed by skeletal stabilization as staged procedures. Because the femoral head was split, dislocated, comminuted, and deemed nonreconstructible, arthroplasty was considered the best option. The extensor mechanism damage was addressed by reconstruction using an iliotibial tract fl

Despite the increasingly widespread use of the volar plate, the pinning of the distal radius keeps its place as a basic technique in the treatment of extra-articular and intra-articular fractures with dorsal displacement and little comminution. To date, there are 2 types of pinning recognized as stable and reliable: the "cross-pinning" with 2 styloid pins and a dorso-ulnar one and the multiple mixed pinning, which is an improved version of the classical technique of Kapandji. This article offers a modified technique of mixed multiple pinning: the pinning type Fort-de-France, which offers a mor

When performing a revision total hip replacement complicated by infection, severe osteolysis, comminuted periprosthetic fracture, and/or extensive bone loss, a single-stage procedure may not be feasible. This study reports four cases of femoral intramedullary nailing as an interim or salvage technique during complicated total hip replacement. This reconstruction provides axial and rotational stability of the femur while maintaining femoral alignment. Furthermore, this reconstruction facilitates early mobilization and rehabilitation of the patient. This interim reconstruction can be converted t

High-energy tibial plateau fractures are often the result of blunt trauma. In addition to extensive bony injuries, soft-tissue problems are very common. Preoperative planning for surgical repair of these injuries is invaluable. Contemporary internal fixation techniques using ligamentotaxis, percutaneous fixation, and antiglide techniques provide the basis for internal fixation of these injuries. When extensive comminution and damaged soft tissues prohibit their use, the circular external fixator provides an excellent option for management of these complex injuries. Whether internal or external

Intra-articular fractures of the lower end of the tibia are an interesting challenge. The best functional results in the past series were observed in patients treated according to the following 4 sequential principles: (1) reconstruction of the correct length of the fibula; (2) anatomical reconstruction of the articular surface of the tibia; (3) insertion of a cancellous autograft to fill gaps left by impaction and comminution; (4) stable internal fixation of the fragments by a plate placed on the medial aspect of the tibia. Seventy-five cases had a good or excellent late result (on average 6

This systematic review and meta-analysis aimed to compare the outcomes of intramedullary nailing (IMN) and open reduction with internal fixation (ORIF) in treating traumatic humeral shaft fractures in adults (18 years and above). A comprehensive literature search was conducted in databases including PubMed, Google Scholar, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL). The primary outcome was time to union, while secondary outcomes included non-union rates, incidences of iatrogenic radial nerve palsy, surgical site infections, and intra-operative comminution.
