Delayed cerebral ischemia (DCI) is an important complication after aneurysmal subarachnoid hemorrhage (aSAH). Early identification of cerebrospinal fluid (CSF) markers is helpful for warning of impending DCI. This study assessed whether early high CSF glutamate levels can be observed in aSAH patients who later developed DCI. In this prospective clinical study, patients with normal pressure hydrocephalus or aSAH were enrolled. We found that the early CSF levels of glutamate were significantly elevated in aSAH patients compared to patients with normal pressure hydrocephalus. There was a signific

The present study reports the case of an 81-year-old woman who underwent lumboperitoneal shunt (LPS) placement for idiopathic normal-pressure hydrocephalus. After LPS placement, the patient presented with radicular pain. A computed tomography scan revealed that the intradural lumbar catheter was tangled at the T11 vertebra; therefore, we decided to remove the catheter under local anesthesia. When 1 cm of the lumbar catheter was withdrawn, the patient suddenly complained of complete loss of bilateral leg sensation and muscle strength. Emergency magnetic resonance imaging revealed that the lumba

UNLABELLED: Hindfoot and distal leg neuropathic fracture collapse secondary to normal pressure hydrocephalus is a very rare clinical pathology. The authors present a case of a 69-year-old woman who sustained a distal tibiofibular fracture that resulted in a recurvatum deformity with idiopathic neuropathy and gait instability on initial presentation. A subtalar and ankle joint arthrodesis was performed achieving rectus alignment of the lower extremity with no postoperative complications. Her neuropathic etiology was negative for common causative factors, including diabetes, infection, nutrition

Twenty-three elderly patients were found to have a consistent pattern of leukoencephalopathy by computed tomography and nuclear magnetic resonance imaging. Eight patients presented with vague, nonspecific symptoms and had no neurologic deficits. The other 15 patients had neurologic deficits that presented in one of three ways: stroke, seven patients; slowly progressive dementia and gait disturbance, five patients; or slowly progressive dementia alone, three patients. Risk factors for arteriosclerosis (hypertension, diabetes) were present in 18 patients (78%). The necropsy of one patient reveal

Periodic leg movements (PLM) in sleep are supposed to constitute a frequent cause of insomnia. There is some controversy whether PLM are the cause of insomnia by provoking microarousals or whether they are simply and epiphenomenon not casually related to the insomnia. We examined four patients with PLM by overnight polysomnography and concomitant transcranial Doppler sonography (TCD) monitoring, 13 patients with suspected normal pressure hydrocephalus by concomitant overnight polysomnography and intracranial pressure recording, and 10 healthy volunteers by concomitant overnight polysomnography

The purpose of the Fifth International Hydrocephalus Workshop was to allow clinicians and basic science researchers to educate each other in the advances that have been and are being made in the understanding and treatment of hydrocephalus and related disorders. This vision of the meeting was the work of Dr. Anthony Marmarou, who died a few months before the meeting was held. The presentations on all aspects of the study of hydrocephalus can be roughly grouped into seven basic themes. These themes are a summary of the important lifelong work of Professor Marmarou himself, including mathematica

AIMS: AQP4 is involved in regulating brain water homeostasis and in the function of the glymphatic system. In iNPH there is an altered parenchymal expression of AQP4, contributing to glymphatic dysfunction. This study aimed to test AQP4 and AQP1 levels in cerebrospinal fluid from iNPH, NOT-NPH, and control subjects to evaluate their diagnostic utility and their correlation with clinical parameters.
METHODS: CSF samples were collected from probable iNPH patients during the Tap test (TT). Patients who responded to the TT or with a Rout ≥ 12 mmHg/mL/min, performed ventriculoperitoneal shunting (

According to glymphatic system theory, cerebrospinal fluid (CSF) perfuses the brain's interstitial space to support waste clearance, but the magnitude of this flow and the outflow pathway of interstitial fluid (ISF) in humans remain uncertain. To achieve flow quantification, we applied a compartment-model approach applied in conjunction with serial quantitative MRI data acquired after intrathecal gadolinium administration. Using the method, we estimated CSF-to-ISF inflow to 45 ± 20 mL/h, in patients with suspected idiopathic normal pressure hydrocephalus. Tissue-specific contributions were 34

Ventricular cerebrospinal fluid (CSF) flow has bi-directional flow profiles synchronized with cardiac pulsation. The increase in CSF stroke volume through the aqueduct (flow pathway between the third and fourth ventricle) is believed to be a biomarker of idiopathic normal pressure hydrocephalus (iNPH). However, several studies have reported that CSF stroke volume varies considerably even in healthy populations. To explore these variations from a fluid mechanics perspective, this study analyzed CSF flow characteristics in healthy individuals using magnetic resonance imaging (MRI)-based computat

BACKGROUND: Idiopathic Normal Pressure Hydrocephalus (iNPH) is a chronic condition affecting the elderly. It is characterized by a triad of symptoms and radiological findings. Glaucoma is the leading cause of irreversible blindness worldwide. Earlier studies have proposed that the rate of glaucoma is higher in iNPH patients, and of a possible link between ventriculoperitoneal shunt (VP) treatment and the development of glaucoma.
OBJECTIVES: This study aimed to determine the prevalence of glaucoma among iNPH patients and assess the impact of VPs on glaucoma prevalence.
METHODS: A cohort study

Alzheimer's disease (AD) is a complex and heterogeneous neurodegenerative disorder with contributions from multiple pathophysiological pathways. One of the long-recognized and important features of AD is disrupted cerebral glucose metabolism, but the underlying molecular basis remains unclear. In this study, unbiased mass spectrometry was used to survey CSF from a large clinical cohort, comparing patients who are either cognitively unimpaired (CU; n = 68), suffering from mild-cognitive impairment or dementia from AD (MCI-AD, n = 95; DEM-AD, n = 72), or other causes (MCI-other, n = 77; DEM-othe

BACKGROUND: Levels of the biomarkers amyloid-β 1-42 (Aβ42), tau and phosphorylated tau (p-tau) are decreased in the cerebrospinal fluid (CSF) of patients with idiopathic normal pressure hydrocephalus (iNPH). The mechanism behind this is unknown, but one potential explanation is dilution by excessive CSF volumes. The aim of this study was to investigate the presence of a dilution effect, by studying the relationship between ventricular volume (VV) and the levels of the CSF biomarkers.
METHODS: In this cross-sectional observational study, preoperative magnetic resonance imaging (MRI) and lumbar

BACKGROUND: The timed-up-and-go test (TUG) is a popular test for mobility, but it only measures the total time to complete tasks. Recent advances in smartphone technology enables measuring respective times of 6 components in the test (Stand, Go, Turn1, Come, Turns and Sit). We examined availability of these data for assessing different gait patterns.
METHODS: This study enrolled 32 patients with probable idiopathic normal-pressure hydrocephalus and 87 age-matched active participants as controls. All the patients responded positively to cerebrospinal fluid (CSF) removal, among whom 19 underwen

Multiple simultaneous intracerebral hemorrhages (ICH) are uncommon. We report the case of an 80-year-old woman with previous diagnosis of normal pressure hydrocephalus and who was brought to our hospital with altered mental status and urinary incontinence. Medical history of hypertension, hematological disorders or severe head trauma was absent. Platelet count and coagulation profile were unremarkable. An initial head computed tomography (CT) showed sulcal enlargement and ventricular dilatation, but no evidence of ICH. A tap test indicated as a guide to case selection for shunt surgery acciden

Meningeal Carcinomatosis (MC) is rare (4 to 5% of patients with solid tumors). We report two cases. The first case is a 53 year-old man presenting flaccid paraplegia and the second is a 76 year-old man presenting a clinical picture suggestive of normal pressure hydrocephalus. In the two cases, the diagnosis of MC was achieved by the demonstration of malignant cells in the CSF. Prognosis was poor in the two cases. The clinical presentation of MC is non specific and the diagnosis is only confirmed by demonstrating carcinomatous cells in CSF.

A 44-year-old Japanese man was diagnosed as having late adult-onset dentatorubral-pallidoluysian atrophy (DRPLA), whose CAG repeats in the DRPLA gene were 60 and 15. He developed gait disturbance, limb ataxia, pyramidal tract signs, dementia, and psychiatric symptoms including character changes within a few years of the above diagnosis. His T 2-weighted brain MRI showed symmetric high-signal lesions in the cerebral white matter and brain stem, in addition to cerebellar, brain stem, and cerebral cortical atrophy. Since the results of RI cisternography indicated that he manifested the clinical f

The author discusses the current state of clinical and pathological knowledge regarding chronic brain disease, focusing particularly on the dementias. His review of clinical studies deals with diagnostic issues and methods, etiology, and treatment. More basic research on brain alterations with aging, their relation to clinical manifestations of dementia, and studies of specific disorders are also reviewed. These disorders have been receiving increasing attention from psychiatrists, who are becoming more aware of the importance of organic cerebral factors in their patients' complaints. The need

PURPOSE: Optic nerve head (ONH) atrophy is frequently associated with hydrocephalic conditions. Cerebrospinal fluid (CSF)-containing meninges form a subarachnoid space that terminates at the ONH, which physically impacts it. This study aims to characterize optic neuropathy in congenital hydrocephalic mice with genetic disruption of the Ccdc13 gene.
METHODS: The ccdc13 germline knockout mice were generated. The hydrocephalus phenotype and subarachnoid space surrounding the optic nerve were evaluated using routine histology and Evans blue stain. Optic neuropathy was examined with immunohistoche

The 'cerebrospinal fluid (CSF) accumulator' plays an important physiological role in the healthy central nervous system as both a shock absorber and regulator of drainage. In the presence of hydrocephalus, this function is compromised by resistance to the flow of CSF. It is further compromised by the placement of a standard shunt which allows only for one-way directional diversion of the fluid. Complications associated with shunt dependency may be a direct consequence of both the condition, and a failure of existing systems to restore the normal accumulator function. This article reviews the p

The clinical significance of white matter changes including periventricular hyperintensity (PVH) and deep white matter hyperintensity (DWMH) on MRI in patients with normal pressure hydrocephalus is unclear. We investigated the correlation between ventricular size and white matter changes in healthy individuals. We performed MRI in 683 (mean age 59; 55% men) neurologically normal adults without a history or radiological findings of brain disease or trauma, who were undergoing brain health screening. The correlation between Evans' index and severity of white matter changes was analyzed. Multiple
