Functional and fixed orthodontic treatment in a child with cerebral palsy.

Cerebral palsy is a permanent neuromuscular motor disorder that results from injury in the developing brain during the prenatal or postnatal period. Common functional and craniofacial problems related to cerebral palsy include impaired swallowing, chewing, and speech; maxillary transverse deficiency; excessive anterior facial height; and Class II malocclusion. This article reports the treatment of a 12-year-old girl with ataxic cerebral palsy; she had a dental and skeletal Class II malocclusion, maxillary transverse deficiency, and severe crowding in both arches. Treatment included rapid maxil

Camouflage treatment of severe bialveolar protrusion in skeletal Class II using miniscrew anchorage.

The aim of this case report was to demonstrate the efficient use of miniscrew as anchorage in a 12-year-old male patient with skeletal and dental Class II and severe biprotrusion and lips procumbency. It was proposed a compensating orthodontic treatment with 4 premolar extraction and maximum anchorage. Since the patient abandoned treatment after one premolar extraction, loss of anchorage occurred and treatment plan was modified to maximize lower retraction with miniscrew anchorage and distalization of the upper arch using two methods: Class II elastics from lower miniscrews to the sliding jig

Soft tissue response to mandibular advancement using 3D CBCT scanning.

This prospective longitudinal study assessed the 3D soft tissue changes following mandibular advancement surgery. Cranial base registration was performed for superimposition of virtual models built from cone beam computed tomography (CBCT) volumes. Displacements at the soft and hard tissue chin (n = 20), lower incisors and lower lip (n = 21) were computed for presurgery to splint removal (4-6-week surgical outcome), presurgery to 1 year postsurgery (1-year surgical outcome), and splint removal to 1 year postsurgery (postsurgical adaptation). Qualitative evaluations of color maps illustrated th

Tooth-borne distraction of the lower anterior subapical segment for correction of class II malocclusion, subsequent to genioplasty.

INTRODUCTION: Alveolar distraction is mainly used to increase height and width of the alveolar crest. This technique, however, is not typically used for lengthening the perimeter of the dental arch or improving teeth axes. We applied alveolar distraction in a tooth-borne manner in the second stage of our original method and obtained favorable results. We therefore present an outline of this method. CASE REPORT: Genioplasty was first performed to create an infrastructure for sequential advancement of the subapical alveolar segment. After bone union, anterior subapical alveolar osteotomy was pe

Masticatory performance is not associated with diet quality in Class II orthognathic surgery patients.

This study evaluated the association between masticatory function, diet, and digestive system problems in 59 Class II patients 5 years after bilateral sagittal split osteotomy. Dietary intake data were recorded in 4-day diet diaries and analyzed for overall diet quality (Healthy Eating Index) and selected dietary components. Masticatory function was assessed through measurements of masticatory performance, maximum bilateral bite force, and chewing time and number of chewing strokes until the subject felt that the bolus was ready to swallow. Self-reported frequency of digestive system problems

Hard- and soft-tissue changes after adult Class II Division 1 orthodontic treatment: a retrospective CBCT-based observational study.

BACKGROUND: Restoring facial balance is now central to adult orthodontic care, yet evidence describing hard-soft tissue coupling and short-term retention stability in non-growing adults with Angle Class II Division 1 malocclusion remains limited. METHODS: In this observational study, 236 consecutively treated adults (≥18 years) underwent clinically archived cone-beam computed tomography (CBCT) before treatment (T0) and within 2 weeks after debonding (T1). Ten hard- and five soft-tissue variables were measured on the T0 and T1 three-dimensional reconstructions and used for the treatment-change

Effect of multiple hook heights and positions during en masse maxillary distalization using infrazygomatic crest miniscrew- single and double points of force application: a finite element analysis study.

BACKGROUND: This study aimed to simulate maxillary dentition distalization as one unit anchored to infrazygomatic crest (IZC) miniscrew using different hook positions and lengths. MATERIALS AND METHODS: Eleven finite-element models (FEM) were constructed from a cone beam computed tomography scan of a patient with Class II malocclusion. Different force vectors to the IZC miniscrew were simulated using one point of force application either mesial to the canine or mesial to the first premolar, using different hook lengths (0, 2, 4, and 6 mm). In the novel approach, two point-force system was con

Simultaneous Surgical Correction of Skeletal Class III Dentofacial Deformity During Acute Management of Facial Fractures: Patient Selection and Surgical Technique.

BACKGROUND: Mandibular prognathism is a common dentofacial deformity in Asia. Treatment of such condition may vary from orthodontic camouflage to surgical orthodontics with orthognathic surgery depending on the severity of the condition. Because of the prominent position of the mandible, fractures involving different locations of the mandible commonly occur in maxillofacial trauma. Anatomical reduction of maxillofacial fractures and restoration of the pretraumatic occlusion are the primary goals of acute management of facial fractures. In patients with dentofacial deformity, simultaneous surgi

The Loudon Chateau repositioning appliance.

The LCR Appliance if fabricated and used properly, has many advantages over the Herbst and Twin Block appliances in solving tongue thrust, mandibular deficiency and repositioning with proper mandibular advancement and mandibular growth results. The regular Chateau appliance was named in 1904 after Dr. Chateau in Franc. It was originally used in Europe but was an uncomfortable removable appliance with wires used in the mandibular anterior lingual area to reposition the mandible.
