Locked posterior dislocation of the shoulder.

Of forty patients with forty-one locked posterior dislocations of the shoulder, the diagnosis had been missed by the initial physician in the majority. A motor-vehicle accident, a seizure, an alcohol-related injury, or electroshock therapy had caused the dislocation in these patients, and the average interval from injury to diagnosis was one year. Twenty-five of the forty-one dislocations had been diagnosed in less than six months. An axillary radiograph confirmed the diagnosis in all shoulders and demonstrated the approximate size of the impression defect. There were no associated displaced f

Partial tibial nerve transfer to the tibialis anterior motor branch to treat peroneal nerve injury after knee trauma.

BACKGROUND: Injuries to the deep peroneal nerve result in tibialis anterior muscle paralysis and associated loss of ankle dorsiflexion. Nerve grafting of peroneal nerve injuries has led to poor function; therefore, tendon transfers and ankle-foot orthotics have been the standard treatment for foot drop. QUESTIONS/PURPOSES: We (1) describe an alternative surgical technique to obtain ankle dorsiflexion by partial tibial nerve transfer to the motor branch of the tibialis anterior muscle; (2) evaluate ankle dorsiflexion strength using British Medical Research Council grading after nerve transfer;…

Defining Endogenous Mitochondrial Transfer in Muscle After Rotator Cuff Injury.

BACKGROUND: Rotator cuff muscle degeneration leads to poor clinical outcomes for patients with rotator cuff tears. Fibroadipogenic progenitors (FAPs) are resident muscle stem cells with the ability to differentiate into fibroblasts as well as white and beige adipose tissue. Induction of the beige adipose phenotype in FAPs has been shown to improve muscle quality after rotator cuff tears, but the mechanisms of how FAPs exert their beneficial effects have not been fully elucidated. PURPOSE: To study the horizontal transfer of mitochondria from FAPs to myogenic cells and examine the effects of β

Lower Extremity Salvage in a Diabetic Patient With Cutaneous Mucormycosis and COVID-19 After Open Patella Fracture.

Background: Cutaneous mucormycosis, while less common than sinonasal or pulmonary infections, can cause widespread tissue necrosis after seemingly innocuous encounters. The most common location of cutaneous mucormycosis is the extremities, and extensive infection has been reported after trauma or orthopedic procedures. Case Report: A 60-year-old female with poorly controlled type 2 diabetes mellitus sustained an open patella fracture after a fall. She underwent washout and internal fixation with cannulated screws and cable tension band wiring. The patient's recovery was complicated by asymptom

Free flaps to preserve below-knee amputation stumps: long-term evaluation.

Five patients with insufficient soft-tissue coverage on below-knee amputation stumps have been treated with free-tissue transfer surgery to preserve a functional below-knee prosthetic level. The flaps employed include one latissimus dorsi myocutaneous flap, two latissimus dorsi muscle-skin graft flaps, one groin flap, and one foot-fillet flap. All five flaps survived; one patient required early venous anastomosis reexploration and revision. The patients have been followed for a mean duration of 5.5 years (range 3 to 8 years). The mean duration to first ambulation with a prosthesis was 3.6 mont

Posterior interosseous neuropathy: distinguishing from a proximal radial neuropathy.

The posterior interosseous nerve is the terminal motor branch of the radial nerve that innervates the extensor carpi ulnaris and the extenso … hse and leash of Henry. The sites were decompressed and concurrent salvage secondary reconstructive tendon transfers were required in view of the severe axonal loss with minimal chance of functional reinnervation.

Combined index finger pollicization with an anterolateral thigh flap for thumb reconstruction.

A severe crush injury to the hand is devastating to patients. Under conditions in which the crush force is too great, the digits are not viable candidates for replantation. We present two cases in which the patients suffered from loss of the thumb ray at the first carpometacarpal joint and skin defect at the radial side. The tendons, radial nerve and metacarpal bone of the index finger were injured, and the second metacarpal head was retained. There was a comminuted fracture of the trapezium in both patients. The treatment protocols consisted of the index finger pollicization and the free ante

Chronic glenohumeral dislocation.

The evaluation and management of chronic glenohumeral dislocations can be challenging. By definition, chronic glenohumeral dislocations repr … y outcomes can be achieved by using a variety of techniques, including open reduction combined with tendon transfers, allograft reconstruction, disimpaction and bone grafting and prosthetic replacement. Equally important, however, is recognizing patients in whom successful outcomes can be achieved with nonsurgical management.

Functional reconstruction of the upper extremity in tetraplegia. Application of Möberg's and Allieu's procedures.

The authors conducted a retrospective study to evaluate the benefit of 43 reconstructive procedures and tendon transfers, performed in 37 sessions to restore hand and arm function in 25 tetraplegic patients. These operative procedures were either single or multiple, depending on the patients' needs for restoration. The initial clinical situation was assessed based on a simplified version of the international classification of Giens, modified in Edinburgh. The results were evaluated through clinical assessment and simple functional testing. Gestural ability was improved in more than 80% of the …

Patella baja after total knee arthroplasty: is it really patella baja?

In the prosthetic knee, narrowing of the space between the patella and the tibial polyethylene can be due to distal positioning of the patella or to a proximal transfer of the joint line. In the former case, one is dealing with patella baja. In the latter case, the patella is in its normal position relative to the femur, a situation we call pseudo-patella baja. The therapeutic options for patella baja and pseudo-patella baja are different because the former is due to a short patellar tendon, and the latter is due to a raised joint line. Eversion of the extensor mechanism during revision surger

Neuroarthropathy: an overuse injury of the shoulder in quadriplegia.

The shoulder joint of the spinal cord injured patient is the primary joint used for transfer and wheelchair propulsion. In addition to ordinary use, the shoulder is subject to overuse injury with resultant functional impairment. Frequent overuse injuries of the shoulder include subdeltoid bursitis, bicipital tendonitis, rotator cuff tear and secondary degenerative arthritis. Neuroarthropathy of the shoulder, an overuse injury of an insensate shoulder girdle, has been rarely described in the literature. The case presented is that of an active 47 year old, right hand dominant, C7 complete quadri

The rotator crescent and rotator cable: an anatomic description of the shoulder's "suspension bridge".

Twenty fresh frozen cadaver shoulders were dissected in order to study the rotator cable-crescent complex. The rotator crescent is a term that we have used to describe the thin, crescent-shaped sheet of rotator cuff comprising the distal portions of the supraspinatus and infraspinatus insertions. The crescent was found to be bounded on its proximal margin by a thick bundle of fibers that we have called the rotator cable. This cable-crescent configuration was found to consistently span the insertions of supraspinatus and infraspinatus tendons. The dimensions of the rotator cable and crescent we

Median nerve injuries and their management.

Injuries of the median nerve cause two major syndromes. Interosseous nerve injury as it branches from the median just below the elbow produces a characteristic deformity. Treatment involves neurolysis, or primary or secondary repair. Various tendon transfers can be used to compensate for lost function. The other syndrome is produced by median nerve injury at the wrist and consists of thenar muscle paralysis and a sensory deficit on the radial aspect of the palm and radial 3 1/2 digits. The carpal tunnel syndrome is the most common cause. Treatment consists of incising the volar carpal ligament…

A Mixed Methods Approach as a Channel to Interpret Outcomes Research and Lived Experience Enquiry of Upper Extremity Elective Surgery for Tetraplegia.

Cervical spinal cord injury (SCI) causing tetraplegia is extremely disabling. In such circumstances, restoration of upper extremity (UE) function is considered the highest priority. The advent of early nerve transfer (NT) procedures, in addition to more traditional tendon transfers (TT), warranted in-depth consideration given the time-limited nature of NT procedures. Potential surgery candidates may not yet have come to terms with the permanence of their disability. A mixed methods convergent design was utilized for concurrent analysis of the Aotearoa/New Zealand upper limb registry data from …

Deltoid flap reconstruction for massive rotator cuff tears: mid- and long-term functional and structural results.

BACKGROUND: Various open and arthroscopic techniques for irreparable massive rotator cuff tears have been described. Muscle-tendon transfers may be considered in order to improve range of motion and strength. MATERIALS AND METHODS: We assessed retrospectively the functional and structural results of 31 deltoid flap transfers for massive postero-superior rotator cuff tears at 2 time points: Mid- and long-term follow-up exams were performed after a mean of 53 and 175 months, respectively. RESULTS: Functional gains were minor, but improvement in pain and patient satisfaction was high. The mean …

Allograft Prosthetic Composites in Complex Primary and Revision Shoulder and Elbow Arthroplasty.

Substantial bone loss at the time of complex primary and revision shoulder or elbow arthroplasty is challenging. Large bone defects compromise component support and important muscle-tendon units. Megaprosthesis, osteoarticular allografts, vascularized bone transfers, fusions, and allograft prosthetic composites (APCs) have all been described for reconstruction in these difficult situations. APCs increase bone stock and provide a better opportunity for soft-tissue reconstruction; however, they are prone to other complications, including graft resorption, nonunion, and fracture. Editorial Commentary: Shoulder Superior Capsular Reconstruction Remains to Be Proven Superior to Partial Repair of the Rotator Cuff for Active Patients With Irreparable Tears and Without Osteoarthritis.

The management of irreparable rotator cuff tears in active patients without severe osteoarthritis is challenging. Retracted tears of the superior cuff result in devastating glenohumeral kinematics and decreased shoulder function. Surgical solutions such as superior capsular reconstruction (SCR) or tendon transfers may improve shoulder function. Regarding SCR, the superior capsule has been described as a static stabilizer allowing for the centering of the humeral head. However, some bases for SCR are based on biomechanical studies that should be regarded as time zero, absent healing, and genera…
