Aetiology of childhood pneumonia in low- and middle-income countries in the era of vaccination: a systematic review.

BACKGROUND: This systematic review aimed to describe common aetiologies of severe and non-severe community acquired pneumonia among children aged 1 month to 9 years in low- and middle-income countries. METHODS: We searched the MEDLINE, EMBASE, and PubMed online databases for studies published from January 2010 to August 30, 2020. We included studies on acute community-acquired pneumonia or acute lower respiratory tract infection with ≥1 year of continuous data collection; clear consistent case definition for pneumonia; >1 specimen type (except empyema studies where only pleural fluid was requ

Circumscribed myositis ossificans of the masseter muscle: report of a case.

BACKGROUND: Necrotizing fasciitis (NF) is a rapidly progressive soft-tissue infection associated with high mortality even when recognized early. Empyema necessitans (EN) is an uncommon complication of pleural empyema in which purulent material extends beyond the thoracic cavity into surrounding soft tissues. Although classically linked to Mycobacterium tuberculosis and Actinomyces, EN due to Streptococcus anginosus is distinctly rare. In exceptional cases, extra thoracic spread of empyema can track along fascial planes and evolve into NF. CASE REPORT: A 36-year-old previously healthy woman, r

Current indications and results for thoracoplasty and intrathoracic muscle transposition.

OBJECTIVES: Thoracoplasty has lost much of its popularity and is being supplanted by space-reduction operations using muscle flaps. Our purpose is to retrospectively study the remaining indications and the evolving modifications of this ancient technique in our current surgical practice. PATIENTS AND METHODS: From 1994 to 2008, 35 patients underwent a thoracoplasty procedure in a single thoracic surgery centre for treatment of infectious complications of previous thoracic surgery. The number and length of ribs excised were dictated by the size and location of the thoracic cavity to obliterate

A diagnostic trap: Malignant pericardial effusion mimicking tuberculous pericarditis.

A middle-aged male with a history of treated pulmonary tuberculosis and industrial asbestos exposure complained of increasing dyspnea and chest pain for 15 days. Upon examination, he was tachycardic (100/min), tachypneic (24/min), hypotensive (100/60 mmHg), with raised jugular venous pressure and muffled heart sounds. A respiratory examination revealed decreased breath sounds in the right lung fields. Chest radiography (Fig. 1) demonstrated gross cardiomegaly with a globular cardiac silhouette suggestive of massive pericardial effusion, along with calcified pleural plaques on the right and pos

Post-Tuberculosis Lung Disease: Clinical Spectrum, Management Principles, and Lifelong Consequences.

Post-tuberculosis lung disease (PTLD) is an increasingly recognized consequence of pulmonary tuberculosis (TB) disease, affecting millions of survivors worldwide and imposing substantial clinical, functional, and socioeconomic burdens. This narrative review synthesizes current evidence across epidemiology, definitions, clinical phenotypes, assessment, and management, with dedicated consideration of children and adolescents. PTLD is common: Studies report abnormal spirometry in ∼60% of TB survivors and persistent, activity-limiting dyspnea in a sizeable minority. Outcomes are generally worse in

Diagnostic accuracy of microscopic Observation Drug Susceptibility (MODS) assay for pediatric tuberculosis in Hanoi, Vietnam.

INTRODUCTION: Microscopic [corrected] Observation Drug Susceptibility (MODS) has been shown to be an effective and rapid technique for early diagnosis of tuberculosis (TB). Thus far only a limited number of studies evaluating MODS have been performed in children and in extra-pulmonary tuberculosis. This study aims to assess relative accuracy and time to positive culture of MODS for TB diagnosis in children admitted to a general pediatric hospital in Vietnam. METHODS/PRINCIPAL FINDINGS: Specimens from children with suspected TB were tested by smear, MODS and Lowenstein-Jensen agar (LJ). 1129 s

Clinical features of recently diagnosed pulmonary paragonimiasis in Korea.

STUDY OBJECTIVE: Paragonimiasis is a typical food-borne parasitic disease that is common in Southeast Asia, the Far East, Latin American, and Africa. Recently, however, this disease has been seen in many parts of the world, largely due to increases in the numbers of immigrants and overseas travelers. The purpose of this study was to evaluate the clinical and radiologic features of recently diagnosed pulmonary paragonimiasis. PATIENTS: We retrospectively analyzed the clinical and radiologic characteristics of 36 patients (21 men and 15 women; median age 48 years; range, 19 to 75) with pulmonar

Plasma ferritin, C-reactive protein, and adenosine deaminase levels in tuberculous lymphadenitis and pleuritis and their role in monitoring treatment response.

BACKGROUND: We aimed to assess the plasma levels of ferritin, C-reactive protein (CRP), and adenosine deaminase (ADA) at baseline and their utility as biomarkers to monitor response to treatment in extrapulmonary tuberculosis (EPTB) patients. METHODS: Prospective measurements of ferritin, CRP, and ADA were done in unstimulated plasma samples of 92 EPTB (49 TB lymphadenitis and 43 TB pleuritis) patients registered for anti-TB treatment. Blood samples were taken at the start, 2, and 6 months of treatment, plasma levels of ferritin and CRP were measured by the enzyme-linked immunosorbent assay a

A diagnostic test that uses isothermal amplification and lateral flow detection sdaA can detect tuberculosis in 60 min.

AIMS: Tuberculosis (TB), caused by Mycobacterium tuberculosis (MTB), is now the leading cause of death from infectious disease, thus rapid diagnostic and screening techniques for TB are urgently needed. METHODS AND RESULTS: Here, a detection of MTB using multiple cross displacement amplification coupling with nanoparticles-based lateral flow device (MCDA-LFD) was developed and validated, targeting the specific sdaA gene. The whole detection procedure, including rapid genomic DNA extraction (15 min), amplification (30 min) and result reporting (2 min), was completed within 50 min. No cross-rea

Tuberculosis bronchopleural fistula treated with atrial septal defect occluder.

Bronchopleural fistula (BPF) is an uncommon and potentially fatal complication of lobectomy or pneumonectomy, particularly in tuberculosis patients. It is associated with high mortality and its treatment remains a challenge. The development of plugging technology has led to the emergence of less invasive endobronchial methods for treating BPF. We describe the successful treatment of a multidrug-resistant tuberculosis patient with BPF using an occlusion device originally designed for transcatheter closure of an atrial septal defect. Follow-up over 10 months revealed maintenance of the repair wi

Diagnostic challenge of tuberculosis of the elderly in hospital: experience at a university hospital in Saudi Arabia.

We studied retrospectively 80 elderly patients who had been admitted to hospital with tuberculosis (TB) between January 1988 and June 1993. There were 64 with pulmonary TB and 16 with miliary tuberculosis (MTB). The mean age was 70+/-7.5 years (range 60-88 years) with 56% over 70 years of age. Underlying disease preceding TB was present in 86.3% patients. In the majority of patients clinical manifestations were subtle. Chest X-ray showed involvement of lower lung fields and miliary shadowing in 71.2% (33/80) and 20% (16/80) patients, respectively. The organism was detected in expectorated sput

Impact of Implementation of an Automated Liquid Culture System on Diagnosis of Tuberculous Pleurisy.

This study was conducted to evaluate the impact of implementation of an automated liquid culture system on the diagnosis of tuberculous pleurisy in an HIV-uninfected patient population. We retrospectively compared the culture yield, time to positivity, and contamination rate of pleural effusion samples in the BACTEC Mycobacteria Growth Indicator Tube 960 (MGIT) and Ogawa media among patients with tuberculous pleurisy. Out of 104 effusion samples, 43 (41.3%) were culture positive on either the MGIT or the Ogawa media. The culture yield of MGIT was higher (40.4%, 42/104) than that of Ogawa media

Mycobacterium tuberculosis miniepidemic in a church gospel choir.

BACKGROUND: Conventional contact investigation and molecular fingerprinting of Mycobacterium tuberculosis isolates in tuberculosis (TB) outbreaks have facilitated recognition as well as application of public health control activities. Singing in a choir as an activity that promotes TB transmission has been occasionally recognized. Such avocational transmission in a middle class community can occur with attendant difficulties encountered in contact investigation. METHODS: Five cases of TB (one index case; three secondary cases; one unassociated case) were identified among members of a famous c

Non-surgical resolution of a delayed esophagopleural fistula caused by tuberculous mediastinal lymphadenitis: Diagnostic challenges and therapeutic success.

BACKGROUND: Esophagopleural fistula (EPF) is an abnormal pathological communication between the esophagus and the pleural space. While EPF is typically associated with malignancy or iatrogenic injury, tuberculosis (TB) is a rare cause, with only a limited number reported cases. Here, we present a case of TB related EPF that developed following diagnostic surgery and was successfully treated with anti-TB therapy alone, without the need for surgical intervention. CASE PRESENTATION: A 58-year-old male presented with a three-month history of a six-kilogram weight loss, chronic cough and sputum pr
