Central Inflammatory Cytokines in Tuberculous Meningitis: A Systematic Review and Meta-analysis.

No formal agreement exists regarding central inflammatory cytokine aberrations in tuberculosis (TB). We undertook a systematic review and meta-analysis of studies comparing cytokine levels in cerebrospinal fluid (CSF) from patients with TB compared with controls. We searched PubMed, Scopus, and Web of Science for articles published up to June 22, 2021. Studies were included in the meta-analysis if they assessed unadjusted levels of cytokines in unstimulated CSF samples and drew the comparison(s) between any of the following pairs: patients with TB versus controls without central nervous system

Differentiation Between Brucellar and Tuberculous Spondylodiscitis in the Acute and Subacute Stages by MRI: A Retrospective Observational Study.

RATIONALE AND OBJECTIVES: The aim of this study was to reveal the distinctive features of magnetic resonance imaging (MRI) for distinguishing brucellar spondylodiscitis (BSD) from tuberculous spondylodiscitis (TSD) in the acute and subacute stages. METHODS: This study involved 14 patients with BSD and 18 patients with TSD from May 2011 to January 2015. BSD was diagnosed based on ≥1/160 titers of a Brucella agglutination test or isolation of Brucella spp. TSD was diagnosed based on the isolation of tuberculosis bacteria. All patients underwent T1- and T2-weight imaging (T1WI and T2WI) and fat

Clinical presentation of pediatric tuberculous spondylitis in high TB burden setting before and during the COVID-19 pandemic.

INTRODUCTION: Tuberculous spondylitis (TBS) in children can be severe, non-specific, and slowly progressive. Disruptions in tuberculosis (TB) services were observed amid the coronavirus disease (COVID-19) pandemic, prompting a closer examination of its impact on TBS patients. This study compared the presenting symptoms of TBS in children before (A) and during (B) the pandemic. METHODOLOGY: An analytic retrospective study was conducted using medical charts and the pediatric respirology registry of all patients (aged ≤ 18 years) diagnosed with TBS before and after the pandemic. Demographic data

Postoperative spondilodiscitis.

INTRODUCTION: Postoperative spondylodiscitis is a primary infection of the nucleus pulposus with secondary involvement of the cartilaginous endplate and vertebral bone. Although uncommon, postoperative spondylodiscitis causes major morbidity and may be associated with serious long-term sequelae. Several risk factors had been identified, including immunosuppression, obesity, alcohol, smoking, diabetes and malnutrition. MATERIALS AND METHODS: A review of the literature was done to analyse the diagnosis, treatment and prevention of postoperative spondylodiscitis. RESULTS: We found that the prin

Pedicle subtraction osteotomy for rigid post-tuberculous kyphosis.

We studied 15 patients with healed tuberculosis of the spine and a resultant kyphosis. We selected only those with no neurological deficit and performed a wedge resection of the vertebra using a transpedicular approach. The wedge was removed from the apex of the deformity. For those with a neurological deficit, we chose the conventional anterior debridement and decompression with 360 degrees circumferential fusion. At a mean follow-up of 26.8 months (8 to 46) the outcome was good with an increase in the mean Oswestry Disability Index from 56.26 (48 to 62) pre-operatively to 11.2 (6 to 16) at t

Use of fluorine-18 fluoro-2-deoxy-D-glucose positron emission tomography in assessing the process of tuberculous spondylitis.

Fluorine-18 fluoro-2-deoxy-D-glucose positron emission tomography can be used to quantify the pathologic increase in glucose metabolism of inflammatory processes. Preliminary studies indicate a high level of sensitivity and specificity in detecting and identifying chronic osteomyelitis. This case study shows that positron emission tomography can be used to assess the process of inflammatory activity in tuberculous spondylitis. This technology also has the advantage of higher spatial resolution compared with other nuclear medicine procedures. In addition, it can differentiate between bone and s

[Magnetic resonance imaging in the complex radiation diagnosis of tuberculous spondylitis in adults].

The paper analyzes X-ray study and MR imaging of 36 adult patients with tuberculosis spondylosis having an active process developed in maturity (n = 20) and sequelae of childhood spondylitis (n = 16). X-ray study is shown to retain its basic value in the diagnosis of spondylitis. MRI is the optimum technique for early diagnosis of inflammatory changes in the spine and soft tissues, the method of choice in neurological disorders.

Bone marrow examinations as final clue to diagnosis of hypercalcemia: report of two cases.

Two young men with severe hypercalcemia in association with renal failure (one acute and one chronic) are reported in whom usual diagnostic tests failed to reveal an etiology, and the final diagnoses were given by bone marrow examinations. Early bone marrow examinations in specific patients with hypercalcemia of undetermined origin sometimes are vital as shown by our two patients.

Tuberculous spondylitis as a cause of inflammatory spinal pain: a report of 4 cases.

Patients are said to have inflammatory spinal pain if they fulfill at presentation 4 of the following 5 criteria: duration of spinal discomfort for at least 3 months, spinal morning stiffness, age less than 40, insidious onset of symptoms, and no relief from pain with rest, but improvement with exercise. Inflammatory spinal pain is typical of the spondylarthropathies. Only in a minority of the cases it is found in other rheumatic disorders such as rheumatoid arthritis, fibromyalgia or infectious spondyilitis. Tuberculous spondylitis is rarely mentioned as a possible cause of inflammatory spina

A case of Pott's disease of the spine eroding into the suprarenal aorta.

Spinal tuberculous osteomyelitis eroding into the aorta is an extremely rare entity. The case of an 80-year-old man with a large proximal anastomotic pseudoaneurysm above an aortobiiliac prosthesis involving the suprarenal aorta is reported. A periaortic and perigraft fluid collection was found to be contiguous with an osteolytic process of the anterior bodies of the T12 and L1 vertebrae. Staged extraanatomic axillobifemoral bypass, graft excision, and viscerorenal revascularization with superficial femoral-popliteal veins were performed. The pathologic diagnosis of spinal tuberculous osteomye

[Pott's disease in children].

Twenty seven cases of Pott's disease in children between 6 months and 14 years of age are reported. They were observed during a two years period, with a follow-up time till 18 months. In report to their ages 67% were lower than 6 years old and two of them lower than one year old. There was a male sex predominance. The 'hump' sign was the most frequently observed. The most frequent damage localization was at dorsal level. All patients received treatment with four drugs, 14 of them needed orthopedic methods, and only 6 were submitted to surgical correction. In more than 50% of the patients there

Unilateral palsy of the hypoglossal nerve in a patient with tuberculosis of the first cervical vertebra.

A 26-year-old female developed a unilateral hypoglossal nerve palsy in association with tuberculosis of the first cervical vertebra. The extensive soft tissue and bony abnormalities were demonstrated only by computerized axial tomography. Involvement of the hypoglossal nerve within the skull by tuberculous granulation tissue was the most likely mechanism for the nerve palsy.

Prediction of the angle of gibbus deformity in tuberculosis of the spine.

The cases of ninety patients who had tuberculous lesions in the thoracic and thoracolumbar spine were studied. A comparison of the angles of the gibbus deformity at onset and six years later showed statistically significant differences between the patients who underwent treatment by radical surgery and those who underwent treatment by one of two non-surgical regimens. Analysis of the amount of the initial loss of vertebral body and of the final angle of the gibbus deformity at six years showed a correlation coefficient of 0.83. Using the formula y = a + bx, the final angle of the gibbus deform
